Otoendoscope-assisted repair for large tympanic membrane perforation in 26 cases

Abstract:Objective To investigate the clinical efficacy and value of otoendoscope-guided repair for large tympanic membrane perforation.Methods A retrospective analysis was conducted involving 26 patients who had undergone transcanal otoendoschope-assisted flap myringoplasty for tympanic membrane perforation between January 2010 and May 2016.The healing of tympanic membrane and the improvement of hearing were evaluated.Results Tympanic membrane grafts survived in all the 26 cases.To be specific,perforation closed in 25 cases,and shrunk in 1 case transplanted with tragus cartilage perichondrium.A crevice of 1 mm appeared anterior to the graft.According to the examination 6 months after the surgery,no recurrent perforation or obvious depression occurred in 25 cases.Rinne test for all patients showed positive results.Parameters of pure-tone threshold test showed that the average air-bone gap reduced by 10 ~ 25 dB [preoperative (56.5 ± 8.5) dB vs.postoperative (32.5 ± 6.5) dB,P < 0.05)].Acoustic immitance of type A occurred in 23 cases,type B in 2 cases.Conclusions Bearing advantages of simple procedures,wide surgical field,minimal invasion and satisfied efficacy,otoendoscope-guided repair of tympanic membrane makes bone-grinding unneeded when the surgical area is exposed.With easy access to otoendoscope,this technology is worth replicating among primary hospitals.
