A Novel Biomarker in Differentiating MCN and MN: Urinary sCD80

Abstract:Objective To investigate the urinary soluble CD80 (sC D80) levels of minimal change nephropathy (MCN) and membranous nephropathy (MN) so as to explore the possibility to take it as a novel noninvasive measurement in differentiating MCN and MN. Methods Urinary and blood samples of the cases with confirmed diagnosis of MCN an MN were collected during the period from Feb., 2017 to Sept., 2017; the concentration of urinary sC D80 was detected by ELISA while the levels of serum albumin (Alb), urinary creatinine (Cr), urinary protein/urinary creatinine (Up/Uc) were measured by autoanalyzer.Results In total, 15 cases with MCN and 13 cases with MN were enrolled, there existed no obvious differences between them in gender, age, Alb, Cr and Up/Uc (P>0. 05); the concentration of urinary sC D80 of the cases with MCN was (721. 70±354. 84 ng/gCr), much higher than that of the cases with MN (374. 05± 162. 68 ng/gCr) (P> 0. 05); no correlation between sC D80 and Up/Uc was observed; the best threshold for urinary sC D80 as MCN diagnosis marker was 683. 43 ng/g Cr, the sensitivity was 53. 3% and the specificity was 100%. Conclusions Urinary sC D80 can be taken as a novel noninvasive biomarker in differentiating MCN and MN, which is of important clinical significance in identifying the pathological types of nephrotic syndrome.
