Gastro-Entero-Pancreatic Neuroendocrine Carcinomas: Differentiation from Adenocarcinomas with Neuroendocrine Differentiation Based on Dynamic Contrast-Enhanced CT Findings of Hepatic Metastases

Abstract:Objective To investigate the feasibility of dynamic enhanced CT features of hepatic metastases in differentiating gastro-entero-pancreatic neuroendocrine carcinomas (GEP-NECs) from adenocarcinomas (GEP-ADCs) with neuroendocrine differentiation (NED).Methods Abdominal CT scans in 49 pathologically proven cases of GEP-NECs with hepatic metastases and 17 pathologically proven cases of NED (+) GEP-ADCs with hepatic metastases were retrospectively reviewed.CT features of hepatic metastases were compared for site,number,size,shape,enhancement pattern,enhancement area,perilesional enhancement and lymphadenopathy and ascites.Results The differences of the site,number,size,shape,enhancement pattern,enhancement area,lymphadenopathy and ascites between the two groups had no statistical significance (P > 0.05).The perilesional enhancement was more found in GEP-NECs than in NED (+) GEP-ADCs group (34.7% vs.11.8%),also x2 test didn't show statistical significance.But the logistic regression analysis reviewed that perilesional enhancement in arterial phase was the independent factor for predicting GEP-NECs (P =0.048).Through ROCcurves,the AUC was 0.705.Conclusion Perilesional enhancement in arterial phase had potential feasibility in differentiating the hepatic metastases of GEP-NECs from NED(+) GEP-ADCs.
