Abstract:Objective:To investiga te the correlation of serum levels of MMP-2 and MMP-9 with the disease activity and progression of joint damage in early rheuma toid arthritis. Methods: Serum MMP-2 and MMP-9 in 98 patients with ear ly RA and 20 normal controls were tested with a double-antibody enzyme-linked im munosorbent assay. The correlation between serum levels of MMP-2 and MMP-9 and L arsen score (△Larsen) and Health Assessment Questionnaire (△HAQ) over the firs t 12 months was analysed. Results: Serum levels of MM P-2 and MMP-9 correlated si gnificantly with C-reactive protein (CRP) (r=0.42 and 0.49, P0.01), △ HAQ (r=0.32 and 0.30, P0.01) and △Larsen (r=0.23 and 0.32, P 0.05) respectively. Ana lysis of the patients with a normal CRP levels showed correlation of MMP-2 and M MP-9 with the presence of erosive disease. (r=0.52 and 0.65 respectively, P0.05). Logistic regression analysis in the patients who were non-erosive at presentation, showed that the Larsen score has a closer correlation with MMP-2 level which indicate the erosive progression (r=0.30, P 0.01). Conclusion: Serum MMP-2 and MMP-9 levels correlate with dis ease a ctivity and early progression of joint damage. They may be a valuable predictor of the progression of bone erosion in early rheumatoid arthritis.
