Efficacy analysis of radioiodine-131 for the first ablation to the postsurgical remnant of differentiated thyroid carcinoma

Abstract:Objective The effect of first ablation on the postsurgical thyroid remnant was assessed in 33 cases of differentiated thyroid carcinoma treated with different dosages of iodine-131 in order to find a reasonable iodine-131 dosage for the first therapy in these patients. Methods Iodine-131 thyroid scan and uptake rate were used as the criteria to evaluate the effect after 6 months of iodine-131 therapy. These cases were divided into three dosage groups, e.g. group 1 (2.22~2.96 GBq), group 2 (3.70 GBq), group 3 (>3.70 GBq). There were two cases groups, i.e. M (metastases) and NM (no-metastases) groups according to whether there was accompanying metastasis. The average dosage of iodine-131 was also calculated in these two groups. Results The clearance rate of post-surgical thyroid remnant after iodine-131 therapy for differentiated thyroid carcinoma in the 3 dosage groups was 33.33% (3/9), 71.42% (10/14) and 30% (3/10) respectively. Group 3 was not higher than group 2 as expected (P=0.05). In M and NM groups, the clearance rate was 23.08% (3/13) and 70% (14/20) (P=0.011) respectively. The calculated average iodine-131 dosage in M and NM group was 4.15±1.45 GBq and 3.50±0.77 GBq respectively. Conclusions A proper dosage (about 3.33GBq) of Iodine-131 has a good effect on the clearance of postsurgical thyroid remnant of differentiated thyroid carcinoma without metastases. Treatment effect can be impaired seriously if accompanied by metastases, even if enhancing the iodine-131 dosage considerably. Therefore, we suggest that the iodine-131 therapy should be given after the thyroid tissue was removed as much as possible.
