Cyclosporine A blood concentration monitoring and its impact on the efficacy of 70 children with aplastic anemia in Peking University People's Hospital

Abstract:Objective:To explore the effect of cyclosporine A (CsA) blood concentration monitoring on clinical efficacy in aplastic anemia (AA) children.Methods:A retrospective analysis was carried out to analyze clinical data of 70 AA children receiving CsA treatment in Peking University People's Hospital from August 2015 to July 2017.Results:The average blood concentration of CsA was(199.17+92.70) ng·mL-1,including 357 cases (49.51%) in the target concentration window.There were 33 cases (47.14%) in the target concentration window and 25 cases (35.17%) below the lower limit at the first monitoring.For the increasing number of TDM tests,the proportion of low concentration cases decreased,while the proportions of normal and high concentration cases increased.For the classification of blood concentration monitoring times,effective rate in patients at ≥ 13 times was significantly higher than those of below ≤ 6 times (x2=12.108,P=0.001),CsA dose and concentration of CsA had a weakly positive correlation (r=0.192).There was no significant difference in blood concentration among different age groups.Conclusion:There was great individual difference in CsA pharmacokinetic parameters.It is helpful to improve the success rate of treatment as carrying on regular monitoring of CsA plasma concentration,and adjusting the dosing regimen according to the results of plasma concentration monitoring.
