Linagliptin in the Treatment of Type 2 Diabetes Mellitus: Rapid Assessment and Economic Analysis based in China

Abstract:Objective:To elucidate the effectiveness,safety and economy of linagliptin in the treatment of type 2 diabetes mellitus,and provide evidence for clinicians and policymakers.Methods:HTA reports and systematic reviews/Meta-analyses were retrieved and screened following defined inclusion and exclusion criteria.Results of included studies were comprehensively analysed after quality evaluation;a Microsoft Excel based cost analysis between linagliptin and glimepiride was conducted based on clinical data from a multicenter,international randomised controlled trial,using medical expenses and drug costs in China,to compare linagliptin with glimepiride.Results:Three HTA reports and 6 systematic reviews/meta-analyses were finally included in the rapid assessment.Compared with placebo,linagliptin could significantly reduce HbA1c (-0.62%--0.76%) and FPG (-1.01--1.12 mmol · L-1) in patients with baseline mean HbA1c 7.70%-8.71% and FPG 7.57-11.03 mmol · L-1,when used both as monotherapy and combination therapy.Linagliptin could also raise the control rate of HbA1c level with favorable safety.Efficacy was generally similar between linagliptin and other oral antidiabetic drugs.Linagliptin was superior to glimepiride on weight,risk of hypoglycemia and adverse cardiovascular events.From health care system perspective,one-year incremental cost was ?-1 757.32 per patient among linagliptin group relative to glimepiride group;from social perspective,incremental cost was ?-1 977.70,indicating more expenses could be saved.Conclusion:Linagliptin was generally effective and safe in the treatment of type 2 diabetes mellitus.Compared with glimepiride,linagliptin significantly reduced risk of hypoglycemia and weight gain,and presented a relatively lower risk of adverse cardiovascular events,being likely a cost-saving option in China.
