Risk factor analysis of the oxygen desaturation in extubation early stage of general anesthesia patients

Abstract:Objective To analyze the independent risk factors of oxygen desaturation in the extubation in the early stage of general anesthesia patients,to provide a guidance for clinical management.Methods The 2 027 clinical data of patients,who were transferred into postanesthesia care unit after general anesthesia from May 22 to August 22,2017 in Changhai hospital of naval military medical university,were collected to analyze the risk factors by logistic regression.Results The incidence of oxygen desaturation was 7.15%.The single factor analysis showed that the oxygen desaturation after extubation was related to multiple factors such as the age ≥60 years,body mass index (BMI) ≥25 kg/m2,preoperative SPO2<95% in room air,preoperative hypertension,surgery time >3 h and snoring,breathing rate≤8 times per minute,pain score> 3 points,sedation score≤4 points and cough strength≤2 points,which were statistically significant (P<0.05 or P<0.01).Six risk factors got into the logistic regression equation,which were BMI≥25 kg/m2,preoperative hypertension,the preoperative SpO2<95% in room air,and the breathing rate≤8 times per minute,sedation score≤4 points,and cough strength<2 points after extubation.Conclusion The incidence of oxygen desaturation patients in the early stage after extubation is closely related with patient's BMI≥25 kg/m2,preoperative hypertension,preoperative SpO2<95% in room air and breathing rate≤ 8 times per minute,sedation score≤4 points and cough strength≤2 points independently.It is necessary to give high risk patients with active intervention to ensure patients' safety during their recovery period.
