Abstract:Purpose: 10-Hydroxycamptothecin (HCPT) is an indole alkaloid isolated from a Chinese tree, Camptotheca acuminata, and has a wide spectrum of anticancer activity in vitro and in vivo mainly through inhibitory effects on topoisomerase I. HCPT has been shown to be more potent and less toxic than camptothecin and has recently undergone clinical trials. To determine how HCPT might be best used as an anticancer agent, preclinical studies of the pharmacokinetics, tissue distribution, metabolism and elimination of HCPT in rats were undertaken. Methods: HCPT was administered to rats by i.v. bolus injection at doses of 1, 3, and 10 mg/kg body weight. HCPT (lactone and carboxylate) and its metabolites in plasma, urine, feces, and various tissues were quantitated by reversed-phase HPLC. Pharmacokinetic parameters were then estimated. Results: Following i.v. administration at doses of 3 or 10 mg/kg, the plasma concentration-time profile for lactone HCPT could be best described by a three-compartment model, with terminal elimination half-lives of 140.4 and 428.6 min, respectively. A two-compartment model was used to fit the plasma concentration-time curve at 1 mg/kg, with a terminal elimination half-life of 30.5 min. Carboxylate HCPT had a longer half-life than the lactone form of HCPT. During the initial 6 h after dosing, urinary excretion was the major route of elimination, and fecal excretion became the major route of elimination thereafter. HCPT was widely distributed to various tissues including the enterohepatic system, kidney, and bone marrow. The lactone form of HCPT was detectable in various tissues examined up to 72 h after dosing at all the three test doses. HCPT glucuronides were present in plasma, urine, feces and various tissues. No significant toxicity was observed at doses of 1 or 3 mg/kg. Polyuria and hematuria were observed only during the initial 3 h after dosing at 10 mg/kg. Conclusions: Prolonged elimination of HCPT in vivo may have a significant impact on its therapeutic effects. HCPT is metabolized to its carboxylate form and glucuronides. Dose-dependent toxicity was observed with i.v. administration of HCPT. The results of this study should be useful in the design of future human trials with this anticancer drug.

It has been found that nanomedicine can significantly improve the stability and water solubility of HCPT. NanoMedicines with different diagnostic and therapeutic functions have been developed to significantly improve the anticancer effect of HCPT. In this paper, we collected reports on HCPT nanomedicines against tumors in the past decade. Based on current research advances, we dissected the current status and limitations of HCPT nanomedicines development and looked forward to future research directions. Keywords: 10-hydroxycamptothecin, antitumor, nanomedicine, nano-drug delivery systems A series of multiple processes, including the rapid spread and uncontrollable multiplication of abnormal cells, lead to the formation of malignant tumors with the potential for metastasis. 1 There are many different approaches to cancer treatment, such as conventional radiotherapy, chemotherapy, etc. However, some therapies are not as effective and some have side effects. 2 Natural compounds are a valuable resource for discovering and developing novel drugs, primarily for cancer treatment, that can serve as safe alternatives to the various synthetic drugs used in existing clinical therapies. 3,4 Evidence suggests that alkaloids are essential for the development of antitumor botanicals. Alkaloids are commonly found in plants, and new alkaloids have been found to have potent apoptotic and antitumor abilities against various cancer cells during the screening process of new drugs. 5,6 HCPT, a natural alkaloid, is the most potent anticancer compound among more than 20 monomers isolated from Camptotheca, with an anticancer activity equivalent to 30 times that of Camptothecin (CPT). 7 Numerous studies have shown that HCPT has the advantages of high efficiency, low toxicity and broad anticancer spectrum and is clinically used to treat colorectal cancer, liver cancer, gastric cancer, ovarian cancer, leukemia, head and neck tumors, etc. 8–11 However, its clinical application is limited by poor water solubility and low bioavailability. Current formulation research is focused on improving the solubility of lactone HCPT in water and improving the slow and controlled release of the drug to enhance its clinical application. 12 One promising approach to overcome these challenges is the incorporation of HCPT into different nano-sized delivery vehicles. 13 The use of nano-drug delivery systems for tumor-targeted therapy is an effective way to overcome the lack of specificity of conventional chemotherapeutic agents and the shortcomings of clinical treatment of tumors. 14,15 Nano-drug delivery systems can effectively enhance the in vivo distribution of chemotherapeutic drugs after systemic administration and improve the balance of efficacy and toxicity in systemic chemotherapeutic interventions. 16,17 Nanomedicines show potential therapeutic advantages over free drugs in vitro and in vivo. 18,19 However, although HCPT nanomedicines have been very much researched, there has not been significant progress in the clinical application of HCPT nanomedicines so far. 20 Studies have shown that inefficient accumulation and penetration in human tumors is the main reason for the poor clinical translation of most nanomedicines. 21 For this reason, many strategies, including enhancement of vascular permeability and optimization of nanoparticles, have been investigated. Preparation and Antitumor Activities of Micronized Hydroxycamptothecin

Abstract:OBJECTIVE To prepare micronized hydroxycamptothecin(nHCPT),and to investigate the antitumor activities and toxicity of nHCPT.METHODS Micronized hydroxycamptothecin was prepared using Supercritical Anti-Solvent Processis method(SAS).The morphology and the particle size of nHCPT were detected by SEM and AFM,respectively.The chemical properties of nHCPT were confirmed by FTIR and HPLC.The content of the residual DMSO was determined by GC analysis.The cytotoxicity on nHCPT on PC-3,MCF-7 and HCT-8 was evaluated by MTT method using HCPT and TPT as positive control,and the nHCPT uptakes by cultured HCT-8 cells were investigated by Micromanipulator System.The inhibition effect against tumors and the toxicity on organs of nHCPT were determined in sarcoma-180(S-180) bearing mice.RESULTS The particle size of nHCPT was(150±40) nm,and the morphology was irregularly spherical.nHCPT showed amorphous form,and the chemical properties was not changed.The residual DMSO content in nHCPT was less than the ICH limit.The samples were stable during storage at 4 ℃ and drying condition for a month in the dark.The inhibition rate of nHCPT was higher than HCPT and TPT on three kinds of tumor cells,and the ability of penetration into cell membrane was improved obviously compared with HCPT.nHCPT also exhibited superior antitumor activities compared with HCPT,which was similar to TPT.However,nHCPT had less toxicity than TPT and HCPT.CONCLUSION nHCPT had potential to become the drug material of new drug formulations due to it′s special characterization.

Abstract:Hydroxycamptothecin (HCPT) has been shown to have activity against a broad spectrum of cancers. In order to enhance its tissue-specific delivery and anticancer activity, we prepared HCPT-loaded nanoparticles made from poly(ethylene glycol)-poly(γ-benzyl-L-glutamate) (PEG-PBLG), and then studied their release characteristics, pharmacokinetic characteristics, and anticancer effects. PEG-PBLG nanoparticles incorporating HCPT were prepared by a dialysis method. Scanning electron microscopy (SEM) was used to observe the shape and diameter of the nanoparticles. The HCPT release characteristics in vitro were evaluated by ultraviolet spectrophotometry. A high-performance liquid chromatography (HPLC) detection method for determining HCPT in rabbit plasma was established. The pharmacokinetic parameters of HCPT/PEG-PBLG nanoparticles were compared with those of HCPT.

Abstract:The review provides a detailed discussion of recent advances in the medicinal chemistry of camptothecin, a potent antitumor agent that targets topoisomerase I. Thousands of CPT derivatives have been synthesized. Two of them, Topotecan and Irinotecan, are commercially approved for use in clinic as antitumor agents while more are still in clinic trials. This review summarizes the current status of the modern synthetic approaches to CPT, the mechanism of action of CPT, the structure-activity relationship(SAR), a number of novel CPT analogs and their biologic activity. There is a systematic evaluation of A-, B- and E-ring- modified camptothecins reported recently.
Antiangiogenic potential of 10-hydroxycamptothecin

Abstract:To investigate the antiangiogenic potential of 10-hydroxycamptothecin (HCPT), the proliferation of human microvascular endothelial cells (HMEC) and seven human tumor cell lines were detected by SRB assay, and the endothelial cell migration and tube formation were assessed using two in vitro model systems. Also, inhibition of angiogenesis was determined with a modification of the chick embryo chorioallantoic membrane (CAM) assay in vivo. Morphological assessment of apoptosis was performed by fluorescence microscope. HCPT 0.313-5 micromol x L(-1) treatment resulted in a dose-dependent inhibition of proliferation, migration and tube formation in HMEC cells, and HCPT 6.25-25 nmol x egg(-1) inhibited angiogenesis in CAM assay. HCPT 1.25-5 micromol x L(-1) elicited typical morphological changes of apoptosis including condensed chromatin, nuclear fragmentation, and reduction in volume in HMEC cells. HCPT significantly inhibited angiogenesis both in vitro and in vivo at relatively low concentrations, and this effect was related with induction of apoptosis in HMEC cells. These results taken collectively suggest that HCPT may be a potent antiangiogenetic and cytotoxic drug and further investigation is warranted.

[Preliminary Study of Lyophilized 10-Hydroxycamptothecin in Advanced or Recurrent Malignancies].

Abstract:BACKGROUND & OBJECTIVE:10-Hydroxycamptothecin (HCPT) is the inhibitor of topoisomerase I with anti-cancer effectiveness on several solid tumors. TUOXI (lyophilized HCPT) has higher purity and stability in comparison with solution for injection HCPT. The purpose of this study was to investigate the efficacy, toxicity, and proper dosage of TUOXI as single agent in treatment of advanced and recurrent solid tumors.METHODS:Sixty patients with the median age of 53 (range from 17 to 73 years) were enrolled into this multicenter phase II clinical trial. Among them, 18 patients were chemonaive and 42 were recurrent from chemotherapy; 22 patients with NSCLC, 12 nasopharyngeal carcinoma, 9 primary liver cancer, 9 colorectal carcinoma, 2 pancreatic carcinoma, and 6 miscellaneous malignancies. HCPT was given at the dosage of 6-8 mg/m(2) x d for 5-10 consecutive days based on the toxicity.RESULTS:Fifty-one patients were valuable for effectiveness. The objective response rate for the whole group was 15.7%. The partial remission (PR) rates were 16% for 6 mg/m(2) group and 15.4% for 8 mg/m(2) group, respectively. The PR rates were 13.7% (3/22) for NSCLC, 33.3% (3/9) for colorectal carcinoma, and 16.6% (2/12) for advanced nasopharyngeal carcinoma, respectively. The PR rate for 60 intent-to-treat patients was 13.3% (8/60). Myelosuppression was the dose-limiting toxicity and other adverse reactions included nausea/vomiting, diarrhea, and skin rash. The incidence of grade III+IV adverse events were 32%, 8%, 8%, 6%, and 4% for leucopenia, skin rash, thrombocytopenia, nausea/vomiting, and diarrhea, respectively. No renal, pulmonary, and cardiac toxicity were found.CONCLUSION:TUOXI (HCPT lyophilized powder) had relatively broad- spectrum anti-cancer efficacy and was effective on advanced or recurrent NCSLC, colorectal carcinoma, and NPC. And the recommended dosage is 6-8 mg/m(2) as 4 hours infusion for 5-10 consecutive days every 3 weeks. Further clinical investigation on large number of solid tumors cases are warranted.
AMPK-mTOR-ULK1 axis activation-dependent autophagy promotes hydroxycamptothecin-induced apoptosis in human bladder cancer cells

Abstract:10-hydroxycamptothecin (HCPT), a natural plant extract, exerts anticancer capacity. HCPT has been reported to induce apoptosis and autophagy in human cancer cells. The interaction between autophagy and apoptosis induced by HCPT and the molecular mechanism in bladder cancer cells were investigated in this study. Our results confirmed that HCPT suppressed cell viability and migration and caused cell-cycle arrest in T24 and 5637. Then, we used Z-VAD(OMe)-FMK to clarify that apoptosis induced by HCPT was mediated by caspase. Moreover, HCPT boosted autophagy through activating the AMPK/mTOR/ULK1 pathway. Blocking autophagy by 3-methyladenine, the adenosine monophosphate-activated protein kinase (AMPK) inhibitor dorsomorphin and siATG7 reversed HCPT-induced cytotoxicity. Conversely, rapamycin and the AMPK activator AICAR enhanced growth inhibition and cell apoptosis, suggesting that autophagy played a proapoptosis role. Taken together, our findings showed that HCPT-induced autophagy mediated by the AMPK pathway in T24 and 5637 cell lines, which reinforced the apoptosis, indicating that HCPT together with autophagy activator would be a novel strategy for clinical treatment in bladder cancer.

Abstract:The DNA topoisomerase I inhibitors, 10-hydroxycamptothecin (HCPT) and camptothecin (CPT), are indole alkaloids isolated from the Chinese tree, Camptotheca acuminata. They have been shown to have a wide spectrum of anticancer activity both in vitro and in vivo. However, their use has been limited due to their water-insolubility. The purpose of the present study was 2-fold, to determine the in vitro and in vivo activity of HCPT and CPT against human breast cancer and to determine the pharmacokinetics of the two drugs to better understand how they can best be used therapeutically. The bl vitro inhibitory effect on tumor growth was observed with breast cancer cell line MDA-MB-468. The in vivo antitumor effects were then determined using severe combined immunodeficient (SCID) mice bearing MDA-MB-468 xenografts. The tumor-bearing mice were orally administered HCPT (1, 3, 6, 9 mg/kg/day, 5 days per week) or CPT (1, 3, 6 mg/kg/day, 5 days per week) for 3 weeks. Growth of the MDA-MB-468 cells was inhibited by HCPT and CPT in vitro and in vivo in a dose-dependent manner. Complete regression of the tumor xenografts, determined by tumor measurement and microscopic examination, occurred in the groups of animals treated with doses of HCPT or CPT of 3 mg/kg/day or more. In general, HCPT was more effective and less toxic than CPT. To determine the potential mechanisms for the pharmacologic differences, the comparative pharmacokinetics of HCPT and CPT were determined in tumor-bearing SCID mice following i.v. or oral administration of H-3-HCPT or H-3-CPT. Parent drugs and their metabolites in plasma, urine, feces, and various tissues were quantified by a recently developed reversed-phase HPLC method. Significant absorption of both HCPT and CPT was observed after oral administration, with CPT having a higher bioavailability. HCPT and CPT were distributed widely into various tissues including the tumor, enterohepatic system, kidneys, and bone marrow. These studies indicate that HCPT and CPT are of potential use in treatment of breast cancer, providing the basis for the design of future human trials with these anticancer drugs.

Abstract:Camptothecin (CPT), an alkaloid isolated from the Camptotheca tree, has demonstrated significant anticancer properties in a range of malignancies. However, its therapeutic efficacy is limited by its hydrophobicity, poor bioavailability, and systemic toxicity. Derivatives, analogues, and nanoformulations of CPT have been synthesized to overcome these limitations. The aim of this review is to comprehensively analyze existing studies to evaluate the therapeutic efficacy, mechanistic aspects, and clinical potential of CPT and its modified forms, including derivatives, analogues, and nanoformulations, in cancer treatment. A comprehensive literature review was performed using PubMed/Medline, Scopus, and Web of Science databases; articles were selected based on specific inclusion criteria, and data were extracted on the pharmacological profile, clinical studies, and therapeutic efficacy of CPT and its different forms. Current evidence suggests that derivatives and analogues of CPT have improved water solubility, bioavailability, and reduced systemic toxicity compared to CPT. Nanoformulations further enhance targeted delivery and reduce off-target effects. Clinical trials indicate promising outcomes with enhanced survival rates and lower side effects. CPT and its modified forms hold significant promise as potent anticancer agents. Ongoing research and clinical trials are essential for establishing their long-term efficacy and safety; the evidence overwhelmingly supports further development and clinical testing of these compounds.

Abstract:Nanotechnology associated with a crystal engineering approach was proposed for improving the solubility and efficacy of hydrophobic drugs in this study. 10-hydroxycamptothecin polymorphic nanoparticle dispersions (HCPT-PNDs) were prepared using the supercritical anti-solvent technique coupled with the high-pressure homogenization method. Shape- and polymorph-dependent tumor suppression was observed in both in vitro and in vivo models, where needle-shaped HCPT-PND exhibited dramatic improvement of antitumor efficacy. A benefit of controllable size and a large surface-to-volume ratio of needle-shaped nanoparticles is the improvement of dissolution properties, which facilitates enhancing pharmacokinetic and pharmaco-dynamic properties. The needle-shaped HCPT-PND, which with longer blood retention time and more effective cellular uptake, makes it possible to accumulate drug in tumor tissues and exhibit higher cytotoxicity. No severe systemic toxicity was observed due to sustained-dissolution and the low dose of drug in normal tissues. The results suggest that the needle-shaped HCPT-PND is an interesting nano-formulation of HCPT.

Abstract:Lung cancer, including lung metastatic cancer, remains one of the most difficult types of cancer to treat. Therefore, the search for new agents for its treatment is very important. 10-Hydroxycamptothecin (HCPT) was proved to have ideal anticancer activity in curing series cancer cells. In this study, the anticancer effect of HCPT on melanoma lung metastasis cancer was investigated by several administration routes, and whether the effect may be attributed to the induction of tumor cells apoptosis was determined. MTT assay results showed that HCPT exhibited selective cytotoxic activity against B16-F10 cells in a concentration- and time-dependent manner. Hoechst 33258 staining and transmission electron microscopy showed typical apoptotic morphology such as condensed chromatin, irregular nuclei, and apoptotic body formation. Flow cytometry analysis indicated a growth on apoptotic cells and a cell-cycle arrest in S phase after treatment with HCPT. In vivo melanoma pulmonary metastases were inhibited by treatment with HCPT. A more significant inhibition was observed if HCPT was administered by aerosol inhalation than that given by i.v. or i.p. administration. Thus, HCPT exhibited potential anticancer activity against B16-F10 cells in vitro and in vivo. However, the possible mechanisms involved still need to be investigated to explain this behavior.

Abstract:Camptothecin shows great promise as an anticancer drug,whose lactone E-ring will bond to the DNA topoisomeraseⅠ to change the topological structure of the DNA and induce the apoptosis of the cell consequently.CPT has a pH-depended equilibrium between the lactone form and the carboxylate form in aqueous solution,however in the physiological milieu the hydrolysis of the lactone E ring leads to the ring open and results in the predominance of inactive carboxylate form.Additionally,some factors,such as the poor aqueous solubility and the severe side effect including neutropenia,thromobocytopenia,hemorrhagic cystitis,and G.I.symptoms,also inhibit the clinical use of the CPT.Therefore,it is significant to improve the solubility and stability and to take advantage of the tumor physiology with lower systemic toxicity.The progress and perspective on CPT modification including the small molecular derivates or analogs from modification of the active sites like the ring A/B/E of the CPT and the delivery systems conjugated to the polymer or the natural supra-molecule or the micelles or hydrogels covalently or uncovalently were summarized.In addition,the progress of the modifications and the effort to lead more efficient CPT conjugates or derivatives to come to the clinic applications were also discussed.

Abstract:Camptothecin is known to possess a broad spectrum of anticancer activities, however due to its high toxicity and low solubility in water, camptothecin is severely limited in clinic. To optimize the effects of camptothecin in the treatment of malignancies, besides the structural modification and reconstruction of camptothecin, the establishment of new drug delivery systems becomes a hot topic in the camptothecin research. This paper aims to summarize the advanced progress in the delivery system and function of camptothecin at recent years. The advances in improving the poor solubility, lactone ring instability, controlled release systems and targeting delivery systems of camptothecin are also reviewed. As the related methods and technologies are improved and developed, camptothecin will be a group of prospective anticancer drugs.

Abstract:The current formulations of the natural anti-cancer product hydroxycamptothecine display poor water solubility and stability , short half-life, adverse effects ( or toxicity ) , and many other issues to limit its clinical use .Nano targeted drug delivery systems have emerged in recent years and shown promising prospects in targeted drug delivery , controlling drug release , improving the bioavailability of poorly soluble drugs , and reducing side effects of toxic drugs .Thus, nano tar-geting drug delivery system has attracted considerable attention as a novel and potentially effective and safe approach to deliver a variety of drugs , particularly natural anti-cancer medicines including hydroxycamptothecin .This article reviews the recent status of various nano targeted and other drug delivery systems for hydroxycamptothecin , which are currently under investigation and development , and discusses their characteristics and theoretical benefits for future clinical applications .

Abstract:OBJECTIVE To study the pharmacokinetic and tissue distribution of L-HCPT and C-HCPT in mice with lung cancer after aerosol inhalation of HCPT.METHODS HPLC-FLD was used to determine the L-HCPT and C-HCPT in plasma and tissues of mice.The regulation on lactone/carboxylate equilibrium of hydroxycamptothecin was compared,analyzed and estimated for pharmacokinetic parameters in plasma and tissues of mice after aerosol inhalation.RESULTS The recovery,the intra-day RSD and the inter-day RSD of L-HCPT met the requirement of detection.The concentrations of L-HCPT were improved after aerosol administration,especially in lung,which was the highest among that in plasma and tissues.CONCLUSION Aerosol inhalation possessed a certain degree of lung targeting,it could rise the concentration of L-HCPT in lung,which was good for the therapy of lung cancer.
