Abstract:The causes of acute kidney injury (AKI) can be divided into prerenal, intrinsic and postrenal according to the pathological regions. We reported the diagnosis and treatment of one case of AKI associated with overdose of the oral anticoagulant warfarin. Furthermore, we discussed warfarin-related nephropathy (WRN) based on the review of relevant articles, with a view to improving clinicians' understanding and identification capability of WRN caused by warfarin treatment and broadening their relevant knowledge, by which the possibilities of missed diagnosis and misdiagnosis of this disease could be reduced considerably.

Abstract:Warfarin,one of the coumarins,is widely used as an oral anticoagulant in clinic because of the effects of both anticoagulation and thrombolysis.Since there are markedly interindividual and interethnic differences in plasma concentration and therapeutic efficacy of warfarin,how to use it rationally and safely by doctors has been becoming a key problem in clinical practice.In this review,we summarized the reasons for resulting in the interindividual difference in drug response under the maintenance dosage of warfarin from pharmacogenomic point of view.We also reviewed the influences of genetic polymorphisms of gene differences,such as CYP2C9 and VKORC1,in drug response and therapeutic efficacy of warfarin.Then,we introduced the latest clinical progress in the model of warfarin maintenance dose,which is based on pharmacogenomics research result and clinical data,to provide a new view for the individualized treatment.
Clinical study of cerebral infarction prevention with warfarin in nonvalvular atrial fibrillation

Abstract:Objective To investigate the efficacy and safety of warfarin in the prevention of cerebral infarction in nonvalvular atrial fibrillation (NVAF). Methods One hundred and thirty-six NVAF patients were randomized into warfarin group [receiving adjusted-dose warfarin,international normalized ratio(INR)was 2.0 - 3.0], aspirin group( receiving aspirin 100 mg/d) and control group (treated without anticoagulants )by random digits table. Followed up 18 months, and the main end point events and adverse effect of the three groups were compared. Results In 136 cases,4 cases lost,and 77 cases(58.3%) were male. The mean dose of warfarin was(2.5 ± 1.0) mg. During the follow-up period, main end point events occurred in 12 cases,with 1 case (2.50%, 1/40) in warfarin group, 4 cases(9.52%, 4/42 ) in aspirin group and 7 cases ( 14.00%, 7/50)in control group. There was no significant difference in main end point events among the three groups ( x2 =2.084,P =0.353). But in the patients with 3 or above risk factors,there was significant difference in the survival curve among the three groups ( x2 = 6.404, P = 0.041 ). The incidence rate of bleeding was higher in warfarin group than that in aspirin group,but there was no significant difference [5.00%(2/40) vs. 2.38%(1/42),P ＞ 0.05]. Conclusions Warfarin can improve survival rate especially in the patients with 3 or above risk factors,and the complication of bleeding occurs mostly when INR ＞ 3.0.Under closed monitoring (INR 2.0-3.0),adjusted-dose warfarin is safety and efficacy.

Abstract:The patients after coronary stent implantation requires regular dual antiplatelet therapy with aspirin and clopidogrel.When these patients are complicated with indications for anticoagulation therapy,such as atrial fibrillation,left ventricular thrombosis and so on,the triple antithrombotic therapy combined with aspirin,clopidogrel and warfarin is usually employed.However,because of significant bleeding risk of this treatment,clinicians are puzzled with its safety,effectiveness and necessity,and researchers currently debate this as a hot spot.In this paper,we have reviewed the recent literatures about the triple antithrombotic therapy,and discussed the application of this treatment in patients with coronary heart disease.

Abstract:Warfarin-related nephropathy (WRN) is defined as acute kidney injury subsequent to excessive anticoagulation with warfarin. Patients with mechanical prosthetic valves require long-term anticoagulant therapy. Nonetheless, warfarin remains the sole available option for anticoagulant therapy. Consequently, patients with mechanical prosthetic valves constitute a special group among the entire anticoagulant population. The present study recorded two cases of patients who had undergone mechanical prosthetic valve surgery and were receiving warfarin therapy. They presented to the hospital with gross hematuria and progressive creatinine levels. Notably, their international normalized ratio (INR) did not exceed three. Subsequent renal biopsies confirmed WRN with IgA nephropathy. The two patients continued to receive warfarin as anticoagulation therapy and were prescribed oral corticosteroids and cyclophosphamide, which resulted in improved renal function during the follow-up. Based on a review of all relevant literature and the present study, we proposed a new challenge: must elevated INR levels be one of the criteria for clinical diagnosis of WRN? Perhaps some inspiration can be drawn from the present article.

Abstract:Objective To investigate the clinical application of warfarin in nephropathy patients of our hospital.Methods The data of inpatients using warfarin in the department of nephronia of Peking university first hospital from July 1 to December 31 in 2014 were collected,the indication,usage,target range of international normalized ratio (INR),the INR level during hospitalization,and adverse drug reactions were analyzed.Results A total of 150 inpatients who received warfarin were enrolled.The use of warfarin involoved 6 indications which was mainly used for prophylactic anticoagulation therapy of nephrotic syndrome patients whose serum albumin decreased significantly (130,86.67％).127 never used warfarin before,and their initial dose were 3 mg (65 cases),1.5 mg (52 cases),or 2.25 mg (10 cases).The maintenance dose was (2.46 ± 1.26)mg.There were some differences in the INR target range.75 patients (50.00％) with INR ＜ 1.5,103 patients (68.67％)with INR ＜ 2.0;6 bleeding events happened during therapy,the incidence was 4.00％.Conclusion The warfarin dosage and the INRs of kidney disease patients were lower.It needs further evidence and more clinical trials to decide whether pharmaceutical intervention is needed.

Abstract:Background: Warfarin is a commonly used oral anticoagulant. It has a narrow therapeutic window and wide variation in individualized dosing, and is used clinically for the treatment of thromboembolic diseases. Due to the widespread use of traditional Chinese medicine (TCM) in China and the complex composition and diverse mechanisms of action of TCM, the combination of TCM and warfarin in patients has led to fluctuations in the international normalized ratio of warfarin or bleeding. To ensure rational clinical use, we summarize the TCMs with which warfarin interacts and the possible mechanisms, with a view to providing a clinical reference. Aim of the study: To summarize the mechanisms by which Chinese herbal medicines affect the enhancement or weakening of the anticoagulant effect of warfarin, to provide theoretical references for clinicians and pharmacists to use warfarin safely and rationally, and to avoid the adverse effects associated with the combination of Chinese herbal medicines and warfarin. Methods: A computerized literature search of electronic databases, including PubMed, MEDLINE, Cochrane Library, Web of Science (WOS), China National Knowledge Infrastructure (CNKI) and WANFANG Data was performed. Key words used in the literature search were “warfarin”, “Chinese medicine”, “traditional Chinese medicine”, “Chinese patent medicine” etc. and their combinations in a time limit from January 1, 1990 to May 1, 2021. A total of 64 articles were obtained following the selection process, including clinical reports, pharmacological experiments and in vitro experiments which were reviewed to determine the mechanism of the anticoagulant effect of herbal medicine on warfarin. Results: The mechanisms affecting the anticoagulant effect of warfarin are complex, and herbal medicines may enhance and diminish the anticoagulant effect of warfarin through a variety of mechanisms; thus, clinical use needs to be cautious. Some herbal medicines have shown inconsistent results in both in vivo and ex vivo experiments, pharmacology and clinical studies, and should be the focus of future research. Conclusion: With the widespread use of TCM, the combination of warfarin and TCM is more common. This article will promote clinicians’ knowledge and understanding of the TCMs which interact with warfarin, in order to avoid the occurrence of adverse clinical treatment processes, and improve the efficacy and safety.

Critical Appraisal on the Role of Warfarin in the Current Era

Abstract:Warfarin has been the most extensively used oral anticoagulant (OAC) in medical settings for over 60 years. Its uses, potential adverse effects, and methods for reversing its effects have been firmly established, rendering it a routine medication in medical settings where most professionals feel at ease employing it. Compared to other vitamin K antagonists (VKAs), such as acenocoumarol, warfarin offers benefits like diminished prothrombin time (PT) assays leading to enhanced oral anticoagulation. Observations over the past few years have seen the inclusion of novel/direct OACs (NOACs/DOACs) in the anticoagulant armamentarium. Although DOACs have several advantages, warfarin still has an important role in subsets of patients where DOACs are contraindicated, not well-tolerated, or cannot afford DOACs due to higher costs. Moreover, there are patient profiles where warfarin is still considered a superior choice compared to DOACs, such as age group of >75 years, kidney failure with creatinine clearance (CrCl) below 30 mL/minute, and prosthetic mechanical valve replacement. Precise management of the international normalized ratio (INR) is crucial for the effectiveness of warfarin treatment. INR monitoring is the major concern in the Indian context due to the lack of laboratories for standardized measurement. Adopting strategies such as point-of-care INR monitoring devices and anticoagulation clinics can help to improve clinical outcomes with warfarin therapy. The present review provides a critical overview of the role of warfarin therapy in the current OAC arsenal and strategies for improving therapeutic control and patient adherence. Analysis of characteristics of warfarin therapy in different clinical departments

Abstract:Objective To analysis the characteristic of warfarin in differ-ent clinical departments and different indications.Methods Data of in-patients using warfarin in clinical department of Peking university first hospital from June 2013 to July 2014 were collected and analyzed retro-spectively, including warfarin initial dose, discharged dose, low molecu-lar weight heparin/heparin overlap time, international normalized ratio ( INR) monitoring time after first use, INR monitoring frequency, INR target and treatment of non-therapeutic INR target or bleeding.Results A total of 219 cases of patients with 12 indications in 11 clinical de-partments were enrolled, 115 male cases and 104 female cases aged from 16 to 88.Characteristics of warfarin therapy in the continued-use group presented no significant difference overall, while differences were out-standing in the first use group.Conclusion When providing medication guide of warfarin, clinical pharmacists should select the appropriate ini-tial dose according to the basic characteristics of patient at each clinical departments and the indications.They need to monitor INR regularly, choose appropriate anticoagulation intensity, and pay attention to strengthening patient′s medication education, in order to prevent throm-bosis and reduce bleeding events.

Abstract:Objective The aim of this study was to compare the efficacy and safety for particular regimen and dosage in venous thromboembolism (VTE) patients with renal insufficiency. Methods English language searches of PubMed, Embase, and Web of Science (inception to May 2021). RCTs evaluating anticoagulants for VTE treatment at acute phase, extension phase, and VTE prophylaxis in patients with renal insufficiency and reporting efficacy (death, recurrence, or occurrence of VTE) and safety (bleeding) outcomes were selected. The methodological quality of each study included was assessed at the outcome level using the risk-of-bias assessment tool developed by the Cochrane Bias Methods Group. Results Twenty-one trials that involved 76,574 participants and 8,972 (11.7%) patients with renal insufficiency were enrolled, including 10 trials on VTE treatment in acute phase (3–12 months), four trials on VTE treatment in extension phase (6–36 months), and seven trials for VTE prophylaxis. For acute VTE treatment, compared with dabigatran etexilate, apixaban (RR 5.90, 95%CI 1.00–34.60) and rivaroxaban (RR 6.18, 95%CI 1.17–32.75) were significantly associated with increased risk of death or recurrence. For extension treatment of VTE, aspirin had the highest probability of the most effective and safest treatment, followed by apixaban. For VTE prophylaxis, compared with enoxaparin, desirudin was associated with lower risk of VTE occurrence (RR 0.56, 95% CI 0.34–0.91), but had higher risk of bleeding than dabigatran etexilate. Conclusion The network meta-analysis informs the optimal choice of anticoagulants and their particular dosage for treatment and prophylaxis of VTE patients comorbid renal insufficiency. Abstract:Warfarin is a widely prescribed anticoagulant for thromboembolic disorders and exhibits wide inter-individual differences in its pharmacodynamic effects. Warfarin exerts its anticoagulant effect by inhibiting the enzymatic activity of vitamin K 2,3-epoxide reductase complex, subunit 1 (VKORC1) which regenerates reduced vitamin K as an essential cofactor for the post-translational γ-carboxylation of glutamic acid residues on coagulation factors II, VII, IX and X, and the anticoagulant proteins C, S and Z. Recent studies have shown polymorphisms in genes involved in the uptake of vitamin K (apolipoprotein E [ApoE]), reduction of vitamin K 2,3-epoxide (VKORC1), metabolism of warfarin (cytochrome P450 2C9 [CYP2C9]), and gamma carboxylation (γ-glutamyl carboxylase [GGCX]) to influence the pharmacokinetics and pharmacodynamics of warfarin in patients from different ethnic backgrounds, resulting in variable warfarin dose requirements. Understanding the causal relationship of these polygenic influences on warfarin dose requirements in patients of different ethnicity may be vital in reducing inter-patient variability and optimising anticoagulant therapy.

Abstract:Warfarin and heparin have formed the mainstay in the prophylaxis of deep vein thrombosis (DVT), stroke prevention in atrial fibrillation, and treatment of thromboembolic disease (TED). However, these choices are hampered by difficult administration, interactions with other medications, side effect profile, and limited indications for treatment. Anti-factor Xa (anti-Xa) inhibitors have already entered the drug market with the drug Fondaparinux being the first anti-Xa inhibitor to be approved for use in the U.S. by the Food and Drug Administration (FDA), and other drugs such as idraparinux being currently in development. A new class of medications, known as direct thrombin inhibitors (DTI), includes the parental agents lepirudin, argatroban and bivalirudin which have been approved by the FDA and the oral agents ximelagatran, melagatran and dabigatran. The latter three drugs which are oral DTIs may soon replace warfarin and heparin as the preferred medications for DVT prophylaxis and for reducing the relative risk of stroke. These drugs do not rely on blocking serine proteases nor do they require a co-factor (antithrombin III) like unfractionated heparin (UFH) or low molecular weight heparin (LMWH). DTIs are rapid in onset, easy to administer, do not interact with other medications or foods, have limited side effects, and can be administered in a fixed dose. The DTI ximelagatran has already been approved in several European and Asian countries, and over a dozen randomized clinical trials have been conducted demonstrating its performance to be on par with warfarin. However, approval by the FDA in the U.S. remains pending in view of reported incidences of elevations in hepatic enzymes that are currently under evaluation. This review examines the role of DTIs in the prevention and treatment of TED and the recent patents reported in the literature.
Hemorrhgic and thromboembolic complications during warfarin anticoagulation therapy

Abstract:Objective To study the incidence of and identity risk factors for hemorrhage and thromboembolism during long-term warfarin therapy.Methods All patients were studied in the People's Hospital of Peking University Anticoagulant Clinic from 2001-04 to 2003-11,in whom a course of warfarin therapy intended to last for more than 4 weeks.All bleeding and thromboembolism events were classified as minor or serious or fatal and life-threatening.Incidences of adverse events were analyzed statistically.Results A total of 128 patients were enrolled in the investigation,the median age of the study group was 67 years(ranged 25 to 83).There were 41 bleeding events occurring in 34 patients,and 5 events of thromboembolism.Age and hepatocirrhosis were significantly associated with bleeding complications(P=0.040 and P=0.014).Conclusion Hemorrhage is the major side effect of warfarin;age and hepatocirrhosis are significantly associated with bleeding complications.
Clinical study of warfarin anticoagulation effect in elderly patients based on vitamin K epoxide reductase complex 1 and cytochrome P450 2C9 genetic polymorphisms

Abstract:Objective To investigate the efficacy and safety of warfarin anticoagulation in Chinese elderly patients based on vitamin K epoxide reductase complex 1 (VKORC1) and cytochrome P450 2C9 (CYP2C9) genetic polymorphisms.Methods Clinical data of 41 elderly patients with initial anticoagulation therapy in our emergency department and respiratory department were collected.Patients were divided into observation group (n=20,patients treated with warfarin based on genetic polymorphisms) and control group (n =21,patients treated based on clinical experience).The international normalized ratio (INR),the time of INR stabilized within target range (2.0-3.0) and the incidence of bleeding episodes in 6-month follow up were compared between groups.Results INR within target range at day 3,4,5 and 7 were 0.0％,42.1％,52.6％,68.4％ in observation group and 0.0％,10.0％,25.0％,35.0％ in control group,respectively.There were significant differences in INR within target range at day 4,7 between the two groups (both P＜0.05),while no significant difference was found in INR within target range at day 5 (P＞0.05).The time of INR stabilized within target range was shorter in observation group than in control group [(9.5±2.4) d vs.(12.3± 4.8) d,P＜0.05].Bleeding complication occurred in 3 patients in observation group and 5 patients in control group,and there was no significant difference between the two groups.Conclusions Warfarin therapy based on VKORC1 and CYP2C9 gene polymorphisms may shorten the time of first INR reaching the target value and INR within target range in elderly patients.However,the risk of bleeding complications should be alerted.

Abstract:Venous thromboembolism (VTE) is of high incidence and prevalence worldwide. Renal insufficiency has high disease burden with insidious development and is accompanied with disorder of coagulation system. A higher prevalence of VTE has been observed among patients with renal insufficiency whereas VTE patient with renal insufficiency had higher rates of adverse outcomes. Recent evidence indicated that renal insufficiency was an important risk factor for both short and long-term prognosis for VTE. Renal function also affects the choice of anticoagulation therapy and dosage adjustment of drugs. We conducted a comprehensive review of the pathogenesis, mechanism, prognosis and treatment strategy for VTE patients who comorbid renal insufficiency by searching the latest and most advanced national and international articles, to provide integrated information for the prevention and treatment for VTE patients.
Warfarin-induced gastrointestinal bleeding with acute liver failure: A case report

Abstract:Warfarin is a coumarin anticoagulant, the only used drug for long-term anticoagulant therapy after mechanical mitral valve replacement. The most common adverse reaction of warfarin is hemorrhage, and thus, the international normalized ratio (INR) should be monitored and the dose should be adjusted timely in patients taking warfarin. Other minor adverse reactions of warfarin include nausea, diarrheal, and skin rash. Nevertheless, warfarin-induced acute liver injury has rarely been reported.
