Abstract:Atrial fibrillation (AF) is one of the most common arrhythmias, associated with high morbidity, mortality, and healthcare costs, and it places a significant burden on both individuals and society. Anti-arrhythmic drugs are the most commonly used strategy for treating AF. However, drug therapy faces challenges because of its limited efficacy and potential side effects. Catheter ablation is widely used as an alternative treatment for AF. Nevertheless, because the mechanism of AF is not fully understood, the recurrence rate after ablation remains high. In addition, the outcomes of ablation can vary significantly between medical institutions and patients, especially for persistent AF. Therefore, the issue of which ablation strategy is optimal is still far from settled. Computational modeling has the advantages of repeatable operation, low cost, freedom from risk, and complete control, and is a useful tool for not only predicting the results of different ablation strategies on the same model but also finding optimal personalized ablation targets for clinical reference and even guidance. This review summarizes three-dimensional computational modeling simulations of catheter ablation for AF, from the early-stage attempts such as Maze III or circumferential pulmonary vein isolation to the latest advances based on personalized substrate-guided ablation. Finally, we summarize current developments and challenges and provide our perspectives and suggestions for future directions.
Potential Maintenance Mechanism in Atrial Fibrillation Patients Subject to Different Treatment Procedures: A Preliminary Study Based on a Human Atrial Model

Abstract:Two clinical ablation protocols, 2C3L and stepwise, have been routinely used in our group to treat atrial fibrillation (AF), but with a less than 60% long-term arrhythmia-free outcome achieved in patients. The goal of this study was to examine the underlying mechanism of low success in clinical outcome. MRI images from one patient were used to reconstruct a human atrial anatomical model, and fibrotic tissue was manually added to represent the arrhythmia substrate. AF was induced with standard protocols used in clinical practice. 2C3L and stepwise were then used to test the efficacy of arrhythmia termination in our model. The results showed that re-entries induced in our model could not be terminated by using either 2C3L or the stepwise protocol. Although some of the induced re-entries were terminated, others emerged in new areas. Ablation using only the 2C3L or stepwise method was not sufficient to terminate all re-entries in our model, which may partially explain the poor long-term arrhythmiafree outcomes in clinical practice. Our findings also suggest that computational heart modelling is an important tool to assist in the establishment of optimal ablation strategies.
Treatment of Long-Standing Persistent Atrial Fibrillation by Linear Ablation

Abstract:Objective To explore the feasibility and efficacy of linear ablation in treatment of long-standing persistent atrial fibrillation.Methods Thirty five patients with long-standing persistent atrial fibrillation were undergone catheter ablation,of which circumferential pulmonary vein ablation,roof line ablation,mitral isthmus ablation and tricuspid isthmus ablation were completed.If sinus rhythm was not restored,electrical cardioversion was performed and electrical isolation of pulmonary vein to atrium bilaterically and bidirectional conduction block was examined.To observe the acute ablation efficacy and the recurrence of atrial fibrillation during follow up.Results Two patients achieve sinus rhythm during ablation,while others restored sinus rhythm through cadioversion.All patients achieved electrical isolation of pulmonary vein to atrium bilaterically.The percentage of conduction block for roof line,mitral isthmus and tricuspid isthmus were 100%,88.6% and 91.4% respectively.Six(17.1%) patients had recurrence episodes during follow up.Conclusions Linear ablation and circumferential pulmonary vein ablation for long-standing persistent atrial fibrillation is feasibility with less operation time.

Abstract:Catheter ablation of atrial fibrillation is an evolving field,especially for chronic atrial fibrillation.Several approaches have been performed in clinic practice,which include circumferential pulmonary vein ablation/isolation,left atrial linear ablation,CFAE ablation,pulmonary vein denervation or stepwise ablation.However,a standard approach has not been established.The total successful rates vary from 21% to 95%.The investigations in the mechanism of chronic atrial fibrillation and methods of three-dimensional mapping,and the usage of new source and technique of ablation will increase the successful rate,decrease the recurrence rate and procedure time.The long-term result of ablation of chronic atrial fibrillation and its impact on the structure and function of heart need further investigations.

Abstract:Recent advances in techniques, technologies and proven superiority over anti-arrhythmic drugs have made catheter ablation the cornerstone of management for atrial fibrillation (AF), which has shown a steady increase in prevalence in the ageing population worldwide. The aim of therapeutic interventions is to achieve stable sinus rhythm that would improve the quality of life and reduce the risk of AF-associated complications. Pulmonary veins (PVs) were first described as the source of initiation of ectopic triggers driving AF, which led to the establishment of PV isolation (PVI) as the most widely practiced procedure to treat AF. Antral PVI is still recognized as the stand-alone ablation strategy for newly diagnosed paroxysmal AF (PAF). However, in non-PAF patients, PVI seems to be inadequate and several adjunctive strategies, including ablation of left atrial posterior wall and non-PV triggers, AF mapping and ablation of rotors and drivers, ethanol infusion of vein of Marshall and renal denervation, etc. have been reported with mixed results. Recent trials have also documented the benefits of early rhythm control in preventing cardiovascular events in addition to slowing the progression of PAF to more persistent forms. Similarly, very late relapse of the arrhythmia after successful PVI has drawn attention to the critical role of non-PV triggers and highlighted their relevance as potential ablation targets during repeat procedures. Ablation technology is also under constant evolution with the introduction of non-thermal energy sources and new tools to create durable lesions. This review summarizes the indications, advancements, and future perspective of AF ablation. Summarizes the different facets of atrial fibrillation (AF) ablation including history, indications, current ablation strategies, quality of life, cognition and progression of AF, and the future perspectives.

Abstract:Multiple wavelet hypothesis and fibrillatory conduction are believed to be atrial fibrillation’s pathogenesis. Radio frequency ablation(RFA) technique, a therapy for atrial fibrillation(AF), applies radio frequency(RF) energy to targeted tissue to make it transmural. Research on AF ablation has already been conducted in China. Currently, there are single-electrode and dual-electrode ablation electrodes. It is discovered that the latter can reduce the treatment time and maintain the ablation shape of the tissue. Clinical application has shown that it has become the first-line treatment option for part of indications patients with AF.

Abstract:Objective To analyze the status of catheter ablation of atrial fibrillation (AF) in 2008 in China. Methods The data in regard of catheter ablation of AF in 2008 provided by AF registration research platform was analyzed retrospectively. Results A total of 2808 cases from 53 hospitals were registered, male 1946 cases,female 862 cases. Average age was (57.4 ± 11.4) years. The proportions of paroxysmal,persistent and permanent AF were 71.5％ ,22. 8％ and 5.7％ ,respectively. 47.2％ patients had organic heartdiseases. The left atrium diameter was(40. 6±1 1.7) mm,the left ventricular end-diastolic dimension was (48.4±5.4) mam,and left ventricular ejection fraction was 0. 63±0. 08. The main procedures were circumferential pulmonary vein (PV) isolation and circumferential PV isolation plus necessary atrial auxiliary line. The energy used for catheter ablation was radiofrequency. Success rate was 82. 1％ ,and recurrence rate was 17.9％. Factors impacting success and recurrence rates included left atrium diameter, type of AF, and procedures. Overall rate of complications was 1.7％, severe complications, such as pericardial tamponade and pulmonary vein stenosis account for only 0. 74％ ,esophagus atrial fistula was not observed. Conclusion It is proposed that catheter ablation could be the first-line therapy for patients with paroxysmal AF and without organic heartdiseases in qualified hospitals.

Abstract:In recent years, the prevalence of and mortality associated with cardiovascular diseases have been rising in most countries and regions. AF is the most common arrhythmic condition, and there are several treatment options for AF. Pulmonary vein isolation is an effective treatment for AF and is the cornerstone of current ablation techniques, which have one major limitation: even when diagnosed and treated at a facility that specializes in ablation, patients have a greater chance of recurrence. Therefore, there is a need to develop better ablation techniques for the treatment of AF. This article first compares the current cryoablation (CBA) and radiofrequency ablation (RFA) techniques for the treatment of AF and discusses the utility and advantages of the development of pulsed-field ablation (PFA) technology. The current research on PFA is summarized from three perspectives, namely, simulation experiments, animal experiments, and clinical studies. The results of different stages of experiments are summarized, especially during animal studies, where pulmonary vein isolation was carried out effectively without causing injury to the phrenic nerve, esophagus, and pulmonary veins, with higher safety and shorter incision times. This paper focuses on a review of various a priori and clinical studies of this new technique for the treatment of AF.

Abstract:Nonpharmacologic atrial fibrillation therapies include surgery and catheter ablation.Surgical ablation is minimally invasive surgery,but the application is still limited.Catheter ablation,however,has developed very rapidly.The in-depth study of the pathogenesis of atrial fibrillation,the start of circumferential pulmonary vein ablation,and complex fractionated atrial electrograms ablation,the success rate of catheter ablation has increased while the complications have decreased.The use of catheter ablation is expected to become the primary treatment of atrial fibrillation.

Abstract:BackgroundCatheter-based ablation to perform pulmonary vein isolation (PVI) has established itself as a mainstay in the rhythm control strategy of atrial fibrillation. This review article aims to provide an overview of recent advances in atrial fibrillation ablation technology.MethodsWe reviewed the available literature and clinical trials of innovations in atrial fibrillation ablation technologies including ablation catheter designs, alternative energy sources, esophageal protection methods, electroanatomical mapping, and novel ablation targets.ResultsInnovative radiofrequency (RF) catheter designs maximize energy delivery while avoiding overheating associated with conventional catheters. Single-shot balloon catheters in the form of cryoballoons, radiofrequency, and laser balloons have proven effective at producing pulmonary vein isolation and improving procedural efficiency and reproducibility. Pulsed field ablation (PFA) is a highly anticipated novel nonthermal energy source under development, which demonstrates selective ablation of the myocardium, producing durable lesions while also minimizing collateral damage. Innovative devices for esophageal protection including esophageal deviation and cooling devices have been developed to reduce esophageal complications. Improved electroanatomical mapping systems are being developed to help identify additional non-pulmonary triggers, which may benefit from ablation, especially with persistent atrial fibrillation. Lastly, the vein of Marshall alcohol ablation has been recently studied as an adjunct therapy for improving outcomes with catheter ablation for persistent atrial fibrillation.ConclusionsNumerous advances have been made in the field of atrial fibrillation ablation in the past decade. While further long-term data is still needed for these novel technologies, they show potential to improve procedural efficacy and safety.

Abstract:Introduction To evaluate the global research results of the catheter ablation and surgical treatment of atrial fibrillation in the past 40 years by bibliometrics, and to explore the hotspots and prospects for future development. Methods Relevant literatures were selected from the Web of Science Core Collection. VOSviewer 1.6.17, SciMAT 1.1.04, and CiteSpace 5.8.R1 were used to analyze the data objectively, deeply and comprehensively. Results As of July 14, 2021, 11,437 studies for the catheter ablation and surgical treatment of atrial fibrillation have been identified from 1980 to 2021. The Journal of Cardiovascular Electrophysiology and Circulation respectively ranked first in terms of the number of publications and the number of co-citations. A total of 6,631 institutions from 90 countries participated in the study, with USA leading the way with 3,789 documents. Cryoablation, atrial fibrosis, substrate modification, minimally invasive and access surgery will still be the research focus and frontier in the next few years. Conclusions The publication information for the catheter ablation and surgical treatment of atrial fibrillation were reviewed, including country, institution, author, journal publications, and so on. Developed countries had the advantage in this research areas, and cooperation with low-income countries should be improved. The former research hotspots in the field of catheter ablation and surgical treatment of atrial fibrillation were analyzed, and the future research direction was predicted.

Abstract:Atrial fibrillation (AF) is the most common arrhythmia in the adult population and catheter ablation has emerged as an important rhythm-control strategy and is the most common cardiac ablation procedure performed worldwide. The antiarrhythmic drugs have demonstrated moderate efficacy in long-term maintenance of sinus rhythm; moreover, they are often not tolerated and are associated with adverse events. Catheter ablation has proven to be effective in treating AF, although long-term outcomes have been significantly less favorable in persistent AF than in paroxysmal. The current guidelines recommend catheter ablation as class I indication for patients whom antiarrhythmic drugs have failed or are not tolerated, and as first-line rhythm-control therapy in selected patients with symptomatic AF. Advances in technology and innovative ablation protocols resulted in a remarkable improvement of the efficacy outcomes after pulmonary vein isolation. This review seeks to provide an updated report of the current practices and approaches, and to describe the latest advances in technology that aim to improve procedural safety, efficacy and to reduce procedural requirements in terms of duration and fluoroscopy exposure.

[Technical principles of ablation therapy]

Abstract:Catheter ablation is a standard procedure in modern cardiology. It can significantly improve the quality of life and life expectancy of cardiac arrhythmia patients. Besides cardiac mapping, ablation itself is a fundamental step to successfully treat cardiac arrhythmias. There are various ablation technologies at hand: In traditional radiofrequency (RF) ablation, electrical current flow generates coagulation necrosis. When understanding the biophysical principles of RF ablation, the investigator is capable to adapt lesion geometry and size to the requirements of the procedure and vary them individually. In addition, lesion metric indices evaluate and integrate important parameters such as power, duration, impedance and contact force to standardize and control RF lesions. Cryoablation induces ice crystals within myocardial tissue, which lead to destruction and electrical scarring of the treated tissue. Histologically, cryolesions are well-delineated with preserved tissue architecture and intact endocardium. Pulsed field ablation (PFA) is a novel rising technology, particularly used for pulmonary vein isolation. In contrast to classic thermal technologies (RF and cryoablation), PFA uses pulsed electrical fields to electroporate cardiac tissue and thereby creates damage on a cellular level only.

Abstract:Objective To summarize the results and trend of registry study on atrial fibrillation(AF) catheter ablation in China.Methods From 1998 to 2007,the data were collected by questionnaires from multicenters of consecutive patients who were underwent radiofrequency catheter ablation for AF,and retrospective analysis was performed.Results From 1998 to 2007,patients submitted to catheter ablation were increased from 11 to 2620.Among them.the patients with persistent and permanent AF and average age were increased steadily.The patients with structural heart disease,or with left atrial diameter(LAD)were increasing year by year.Circumferential pulmonary vein ablation and segmental pulmonary vein ablation were the main strategies during this registry period.Radiofrequency is the major source of energy and Carlo is the mainly used eleetroanatomic mapping system for ablation.During follow-up period.the total success rate was 77.1%and the recurrenee rate Was 22.9%.Age,gender,with structural heart disease,LAD,left ventrieular end diastolic diameter (LVEDD),type of AF,ablation procedure were the main factors which affect success rate and recurrence rate.Application of antiarrhythmic drug dechned while anticoagulant agent enhanced.Nevertheless,complications occurrod in 5.3%of patients:the most common complications were ecchymoma and cardiac tamponade.Only 1 had esophagus atrail fistula.Conclusions Curative AF ablation can be recommended as first-line therapy for symptomatic paroxysmal AF in qualified center.

Abstract:PURPOSE: Catheter ablation of atrial fibrillation (AF) is now one of the most frequently performed ablation procedures, but there are currently 2 important challenges: achieving permanent/durable rather than transient pulmonary vein isolation (PVI) and improving the results of ablation for the wider patient population with persistent AF.METHODS: Recent technical advances in the technique of ablation and the results of clinical trials aimed at achieving more permanent and durable PVI are reviewed. We also summarize recent advances in identifying atrial fibrosis and in understanding the pathophysiology of AF relevant to selecting patients for ablation of persistent AF.FINDINGS: The use of contact force-sensing technology, adenosine testing after ablation, and pace capture-guided ablation all have the potential for achieving more durable ablation. Selection of patients suitable for ablation of persistent AF may be improved by assessing the extent of atrial fibrosis with delayed enhancement imaging with cardiac magnetic resonance or by assessing the pattern of atrial electrical activity with the use of complex atrial electrograms. Advances in treatment are likely to result from the recognition of localized rotors and focal sources as primary sustaining mechanisms for all types of human AF and in the use of noninvasive mapping for their identification. Linear ablation to supplement PVI may improve the results of AF ablation.IMPLICATIONS: Rapidly unfolding advances in the techniques of AF ablation and the understanding of mechanisms of AF hold promise for improving the durability of PVI and for extending the technique to carefully selected patients with persistent AF.

Abstract:Atrial Fibrillation(AF) and its complications are serious threat to human health and the radio frequency ablation (RFA) becomes one of the main therapies of AF. Conventionally, the RFA is performed by unipolar ablation mode. Because the unipolar ablation mode is point-to-point and incomplete linear lesion formation, the success rates of treatment on AF decline and the procedures are time consuming. In order to solve these shortcomings, the bipolar ablation mode and the multichannel frequency ablation method that facilitate the easy creation of linear lesion are presented, especially kinds of multichannel frequency ablation technique and applications are introduced in this paper.
Catheter Radiofrequency Ablation for the Treatment of Paroxymal Atrial Fibrillation

Abstract:【Objective】To evaluate the effectiveness and safety of the treatment of atrial fibrillation by catheter radiofrequency ablation.【Methods】Catheter radiofrequency ablation was applied in 60 patients with drug refractory paroxysmal atrial fibrillation.Three different methods(segmental ablation of pulmonary vein,isolation of pulmonary vein guided by CARTO and EnSite 3000 system) were performed to disconnect the electrical activity of pulmonary vein.【Results】All 60 patients were followed up for 3~48 months.Forty two(70%) patients were free of atrial fibrillation,10 patients gained partial success,and the ablation failed in 8(13.3%) patients.Ablation guided by CARTO and EnSite 3000 system could reduce X-ray exposure,there was not severe complications.【Conclusion】Catheter radiofrequency ablation is a performable method for the treatment of paroxysmal atrial fibrillation and has effectiveness for most patients with atrial fibrillation.

Abstract:Objective To assess the therapeutic effect and safety of radiofrequency catheter ablation of atrial fibrillation.Methods We analyzed the clinical data of 47 patients with atrial fibrillation who underwent radiofrequency catheter ablation between March 2013 and January 2008,in the First Affiliated Hospital of Xi'an Jiaotong University.In the average 32 months’follow-up,Holter monitering and echocardiography were reviewed for the left atrial diameter.Results The immediate success rate of catheter ablation for atrial fibrillation was 82.6%.The long-term success rate was 65%,the rate of paroxysmal atrial fibrillation was 69.7%,and the rate of longstanding persistent atrial fibrillation was 42.9%.After ablation,the left atrial diameter were markedly reduced compared with that before treatment [(36.3 ± 4.3 )mm vs .(38.1 ± 5.9 )mm)(P < 0.001 ).The patients with recurrent atrial fibrillation were older than those without recurrence,their left atrial diameter was bigger,and the prevalence rate of hypertension was higher (P <0.05).The average atrial fibrillation load was 14.9% after ablation compared with 46.1% before (P <0.05).Conclusion Radiofrequency catheter ablation is an effective and safe treatment of atrial fibrillation,especially for paroxysmal atrial fibrillation.The left atrial diameter was significantly decreased after radiofrequency catheter ablation compared with that before the ablation.

Abstract:Catheter ablation of atrial fibrillation (AF) has evolved over the past 20 years from being a novel, unproven procedure to a commonly performed procedure. Triggers are important for the initiation of AF and a suitable substrate is important for perpetuation of AF. Remodeling, including electrical and structural remodeling, is common in patients with persistent AF. Therefore, targeting the remodeled atrium is a critical issue during persistent AF ablation. However, ablation outcomes remain suboptimal despite aggressive substrate modification. Empirical linear ablation is not recommended because of the difficulty in achieving complete linear block and it is recommended only if macroreentry tachycardia develops during the procedure. Complex fractionated atrial electrogram (CFAE) ablation is recommended in the Heart Rhythm Society Consensus Document but efficacy has been limited in long-term follow-up studies. Rotor ablation is controversial. A combined approach using CFAE, similarity and phase mappings with rotor identification may be helpful in searching for AF sources and subsequent substrate ablation. Nevertheless, more prospective randomized studies are required to validate efficacy and safety. (Circ J 2016; 80: 306–313)
The registration study of catheter ablation in patients with atrial fibrillation in China

Abstract:Objective To analyze the status and trend of the development of catheter ablation of atria] fibrillation ( AF) in China up to 2005. Methods In March 2006, a registry form was dispatched to all hospitals performing catheter ablation of AF. After items in regard of catheter ablation of AF during 1998 to 2005 had been filled by each hospital, the forms were collected for analysis. Results A total of 3 1% cases from 40 hospitals were registered , malein 2 193 cases, femalein 1 003 cases. Average age was (54.77±5.98) years. The proportions of paroxysmal, persistent and permanent AF were 85.67% , 11.51% and 2.82% , respectively. 45.93% patients had one or more than one basic disease, left atrium thrombus accounted for 0.9%. Diameter of left atrium was(37.02±3.98) mm, the left ventricular end-diastolic dimension was (46.81±4.05) mm, and left ventricular ejection fraction was0. 59±0.06. The main procedures included focal ablation, segmental PV ablation, circumferential ablation of PV ostia, left atrial substrate modification and PV antrum modification. The energy used for catheter ablation included radiofrequency (95.96%), ultrasound (3.00%) and cryoablation (1.22%). The end points of five procedures were different from each other. Factors impacting success and recurrence rales included gender, age, basic diseases , cardiac function and structure, experience of the operators, type of AF, course of AF, procedures and energy. After catheter ablation, the applications of antiarrhythmia drug decreased markedly, and the anticoagulation therapy strengthened. Overall rate of complication was 7.48% , Severe complications, such as pericardial tamponade and pulmonary vein stenosis only account for 3. 19%. Conclusions It is proposed that catheter ablation could be the first-line therapy for patients with paroxysmal AF and without basic diseases in qualified hospitals.
