Abstract:Non-steroidal anti-inflammatory drugs (NSAIDs) is such kind of common used drugs that have a long history of argument on the controversy about its defined efficacy and adverse reaction. Much effort has been done in this field to reduce the adverse reactions as much as possible without at the expense of improving high efficacy. In the past decade, the development of drug adverse reaction research, pharmacoecnomics, pharmacogenetics and evidence-based medicine have put deeper insight in the research of the etiology, prevention development, and outcome of the adverse reaction caused by NSAIDs,which in turn will provide more scientific clue to reasonable selection of NSAIDs in clinical practice.
Analysis of Adverse Drug Reaction of NSAIDs Reported in Domestic Medical Journals Within Past 43 Years

Abstract:Purpose To explore the rules and characteristics of adverse drug reaction(ADR) of non-steroidal anti-inflammatory drugs(NSAIDs). Methods A metrological analysis was conducted on data of ADR of analgesics from Chinese medical journal reports in past 43 years. All reports were evaluated according to the ADR guidance of state food and drug administration bureau (SFDA). Results The amounts of ADR were not statistically different among groups of different sex and age strata. The ADRs commonly appeared within 3 months after taken medicine. Gastrointestinal reactions was the most frequent injured. Followed were skin injury, respiratory system injury, nervous system injury and hepototoxicity. The outcome of ADR was well,98.3% patients could fully recover or improve, but also quite a number of patients left sequela or death. Conclusions NSAIDs are widely used in clinical therapy and their ADR is comprehensive. Some of them belong to over the counter (OTC) drugs, so it is important to enhance the clinical monitoring and improve educations in common population.

Abstract:Non-steroidal anti-inflammatory drugs (NSAIDs) are one of the most widely consumed drugs. These drugs often end up in our water systems because human bodies cannot fully metabolize them, and the residual amounts are excreted through urine or feces. Their ubiquitous presence in wastewater poses various degrees of risk to non-targeted species. Cognizant of the situation, this review encompasses and probes into the occurrence, sources, removal strategies, and legislations pertaining to NSAIDs in aqueous environments. A comprehensive and state-of-the-art overview of the utilization of multifunctional nanomaterials such as nanoadsorbent, metal–organic frameworks, semiconductor-based nanocatalysts, and biologically modified nanoparticles for the efficient removal of NSAIDs in water has been discussed. Furthermore, the review is an output of a conscientious effort to bring attention to the current global guidelines, policies, and directives. The review also entails recommendations for future studies toward managing pharmaceutical contaminants in the environment. We believe that the present study is useful for environmentalists, legislators and policymakers, scientists, industrialists, and organizations working toward sustainable development goals.

Abstract:Cyclooxygenases play a vital role in inflammation and are responsible for the production of prostaglandins. Two cyclooxygenases are described, the constitutive cyclooxygenase-1 and the inducible cyclooxygenase-2, for which the target inhibitors are the non-steroidal anti-inflammatory drugs (NSAIDs). Prostaglandins are a class of lipid compounds that mediate acute and chronic inflammation. NSAIDs are the most frequent choices for treatment of inflammation. Nevertheless, currently used anti-inflammatory drugs have become associated with a variety of adverse effects which lead to diminished output even market withdrawal. Recently, more studies have been carried out on searching novel selective COX-2 inhibitors with safety profiles. In this review, we highlight the various structural classes of organic and natural scaffolds with efficient COX-2 inhibitory activity reported during 2011-2021. It will be valuable for pharmaceutical scientists to read up on the current chemicals to pave the way for subsequent research.

Abstract:Quality by design(QbD)concept is an advanced pharmaceutical quality control concept.The application of QbD concept in the research and development of pharmaceutical processes of traditional Chinese medicines(TCM)mainly contains five parts,including the definition of critical processes and their evaluation criteria,the determination of critical process parameters and critical material attributes,the establishment of quantitative models,the development of design space,as well as the application and continuous improvement of control strategy.In this work,recent research advances in QbD concept implementation methods in the secondary development of Chinese patent medicines were reviewed,and five promising fields of the implementation of QbD concept were pointed out,including the research and development of TCM new drugs and Chinese medicine granules for formulation,modeling of pharmaceutical processes,development of control strategy based on industrial big data,strengthening the research of process amplification rules,and the development of new pharmaceutical equipment.

Abstract:It has been widely accepted in modern pharmaceutical industry that quality cannot be tested into products , instead quality should be built-in by design . However , the implementation of Quality by Design ( QbD ) in development , manufacture and supervision of traditional Chinese medicine ( TCM ) still remains to be a challenge . The program of safety re-evaluation of TCM injections provides an opportunity to practice QbD approach in TCM industry . This paper discussed some potential ways that improve the quality of TCM injec-tions base on QbD related tools , including identification of critical quality attributes , critical process parameters and critical material attributes, development of design space and control strategy.

Abstract:Non-steroidal Anti-inflammatory Drugs (NSAIDs) are extensively utilized pharmaceuticals worldwide. However, owing to the improper discharge and disposal practices, they have emerged as significant contaminants that are widely distributed in water, soils, and sewage sediments. This ubiquity poses a substantial threat to the ecosystem and human health. Consequently, it is imperative to develop rapid, cost-effective, efficient and reliable approaches for containing these substance in order to mitigate the deleterious impact of NSAIDs. This research provides a comprehensive review of the occurrence, fate, and hazards associated with NSAIDs in the general environment. Additionally, various removal technologies, including advanced oxidation processes, biodegradation, and adsorption, were systematically summarized. The study also presents a comparative analysis of the benefits and drawbacks of different removal technologies while interpreting challenges related to NSAIDs' removal and proposing strategies for future development.

Abstract:Objective: To analyze the situation and trend of using NSAIDs. Methods: According to the varieties, amount, purchase cost of NSAIDs in outpatient department of Huashan Hospital affiliated to Fudan University, DDDs, DDDc and purchase cost were analyzed retrospectively. Results: The total purchase costs were 800-1 200 thousand Yuan every year during 2000-2004, and DDDc has been increasing year by year. Meloxicam, diclofenac and ibuprofen sustained-release preparation ranked top 3 on the DDDs list. A new kind of NSAIDs, named selective cyclooxygenase-2 inhibitors has been introduced to the clinical use since 2000. Conclusion: There are always some hazards to use NSAIDs for a long time. So the aim of research is to improve the formulation of NSAID products and get the new and safe NSAIDs.

Abstract:OBJECTIVE:To analyze the development of new drug introduction management,and to provide reference for the management of new drug introduction in medical institutions.METHODS:Related literatures in which title and keywords included "new drug introduction"were retrieved from VIP database during Jan.1998-Dec.2009,and statistical analysis was conducted during four periods.RESULTS & CONCLUSIONS:A total of 36 related literatures have been retrieved,the number of literatures during 2004-2006 is much higher than other three periods.It may be associated with the implementation of Prescription Management Method(trial).Review and comments was in a decreasing tendency(37.5%,50%,21.4%,12.5%).Researchers reached unanimous agreement on new drug introduction,and other aspects should be strengthened.Management case literatures always take up the high proportion(62.5%,50%,46.8%,50%).Research method literatures have appeared since 2004(0,0,35.7%,37.5%),it indicates that focus of researchers have been shifted to new drug introduction management,which become the breakthrough points of its development.

Abstract:Diclofenac is a nonsteroidal anti-inflammatory drug (NSAID) of the phenylacetic acid class with anti-inflammatory, analgesic, and antipyretic properties. Contrary to the action of many traditional NSAIDs, diclofenac inhibits cyclooxygenase (COX)-2 enzyme with greater potency than it does COX-1. Similar to other NSAIDs, diclofenac is associated with serious dose-dependent gastrointestinal, cardiovascular, and renal adverse effects. Since its introduction in 1973, a number of different diclofenac-containing drug products have been developed with the goal of improving efficacy, tolerability, and patient convenience. Delayed- and extended-release forms of diclofenac sodium were initially developed with the goal of improving the safety profile of diclofenac and providing convenient, once-daily dosing for the treatment of patients with chronic pain. New drug products consisting of diclofenac potassium salt were associated with faster absorption and rapid onset of pain relief. These include diclofenac potassium immediate-release tablets, diclofenac potassium liquid-filled soft gel capsules, and diclofenac potassium powder for oral solution. The advent of topical formulations of diclofenac enabled local treatment of pain and inflammation while minimizing systemic absorption of diclofenac. SoluMatrix diclofenac, consisting of submicron particles of diclofenac free acid and a proprietary combination of excipients, was developed to provide analgesic efficacy at reduced doses associated with lower systemic absorption. This review illustrates how pharmaceutical technology has been used to modify the pharmacokinetic properties of diclofenac, leading to the creation of novel drug products with improved clinical utility.

Abstract:Abstract: In this manuscript, the resource distribution, pharmacological activity, pharma-cokinetics of sinomenine and the structure, synthesis, biological activity and mechanism of sinomenine derivatives reported from 2000 to December 2023 were reviewed. The lit-erature was retrieved through Web of Science, PubMed, Science Direct, SciFiner Scholar and other websites. Sinomenine belongs to isoquinoline alkaloids and was extracted from the Chinese herb Sinomenium acutum root. In Asian countries such as China and Japan, it is commonly prescribed as a treatment for rheumatoid arthritis. In addition, sinomenine also has sedative, analgesic, anti-inflammatory, immunosuppressive, neuroprotective, an-ti-drug dependence, anti-tumor and other biological activities. Sinomenine limited its ap-plication prospects because of its large dosage, poor epidermal permeability and short half-life. To overcome these defects, new sinomenine derivatives have been synthesized. Based on the comprehensive analysis of relevant literature at home and abroad, this paper reviews the recent progress in the study of sinomenine's pharmacological effects and structural modifications. Future research on sinomenine will focus on improving its thera-peutic effect, and developing new drug preparations and structural modifications. It is hoped that this review will help to better understand the research progress of sinomenine and provide constructive suggestions for further research of sinomenine.

Abstract:Acetylsalicylic acid (ASA) is a clinical drug with multiple effects, including prevention of cardiovascular adverse events and anti-cancer effects. However, gastrointestinal side effects, such as gastrointestinal ulcers and bleeding, limit the use of ASA and reduce patient compliance. Various studies have investigated the mechanisms of ASA-induced gastrointestinal injury, and many medicines have been reported to be effective in preventing and treating the adverse gastrointestinal effects of ASA. New formulations of ASA have demonstrated milder gastrointestinal injury than ASA alone. In this article, we summarized the mechanisms of ASA-induced gastrointestinal injury, drugs that resist gastrointestinal side effects of ASA, and progress in research on formulation improvement of ASA to help resolve the clinical dilemma of ASA usage.

Literature analysis of non-steroidal anti-inflammatory drugs induced severe drug eruptions

Abstract:OBJECHVE To analyze the clinical characteristics and relevance of severe drug eruptions induced by non-steroidal anti-inflammatory drugs (NSAIDs),and to supply information for clinical application.METHODS Relevant literatures reported in domestic and foreign journals from 1970 to 2017 were collected and statistically analyzed.Data were extracted inclu ding age,sex,drugs possibly inducing adverse drug reactions (ADRs),types of severe drug eruptions and so on.RESULTS Relevant literatures were retrieved and 49 patients were involved.In these patients,27 ADRs were induced by single non-sster oidal anti-inflammatory drug,22 by combined medication.Of 49 NSAIDs induced severe drug eruptions,the top four drugs were acetaminophen,piroxicam,ibuprofen and etoricoxib.The average latency of severe drug eruption was 7.88 ± 10.42 d,and hypersensitivity syndrome induced by NSAIDs had the longest latency of 36.50 ± 14.20 d.Most of the cases were relieved after drug withdrawn except one patient died due to toxic epidermal necrolysis.CONCLUSION These findings suggest clinicians to monitor patient skin symptoms seriously,to reduce the occurrence of severe drug eruptions.

Abstract:In China, research in drug design began in the late 1970s and has flourished in the early 2000s through several generations of effort, which has placed the research level of China in this field at the global forefront. With the application of computer-aided drug design approaches, many small-molecule drug candidates have entered the clinical trial stage, and some have been approved for therapeutic use by the National Drug Administration of China. This paper reviews the development of drug design in China and summarizes the main achievements in this field over the past 40 years in terms of establishment, growth, maturation, and thriving stages.

Abstract:Nonsteroidal anti-inflammatory durgs(NSAIDs) are among the most widely prescribed medications with their uses limited by significant toxicity.The studies of the cyclooxygenase(COX) show that the side effects of classical NSAIDs come from the inhibition of COX-1,while the therapeutic effects come from the inhibition of COX-2.Selective COX-2 inhibitors were developed as NSAIDs which display the profiles of good potent activities and low side effects. In order to search for the new NSAIDs,using the marketed selective COX-2 inhibitor celecoxib as a leading compound and based on the result of molecular modeling study of COX-2 inhibitors,a series of novel diaryl-substituted-1,2,4-trizole derivatives were designed and synthesized.All the target compounds were confirmed by IR,MS,~1H NMR spectra and elemental analysis.The results of preliminary pharmacological tests in vivo show that some compounds have certain anti-inflammatory effect.

The regulatory system for new drug evaluation and registration in China was gradually developed in accordance with international standards. The approval and registration of TCM in China became as strict as those of chemical drugs and biological products. In this review, TCM-based new drug discovery and development are introduced according to the TCM classification of nine categories.

Abstract:Herein, we review the recent progress in the synthesis of representative nonsteroidal anti-inflammatory drugs (NSAIDs), ibuprofen and naproxen. Although these drugs were discovered over 50 years ago, novel practical and asymmetric approaches are still being developed for their synthesis. In addition, this endeavor has enabled access to more potent and selective derivatives from the key frameworks of ibuprofen and naproxen. The development of a synthetic route to ibuprofen and naproxen over the last 10 years is summarized, including developing methodologies, finding novel synthetic routes, and applying continuous-flow chemistry.
