Abstract:OBJECTIVE:To investigate the prevalence of aminoglycoside resistance and genotyping of acetyltransferase in Escherichia coli.METHODS:Resistance phenotypes to 12 antibiotics of 44 Escherichia coli isolates were analyzed using agar dilution method and 3 aminoglycoside resistance genes aac(3)-I, II and aac(6')-I were determined by PCR method.RESULTS:In 44 clinical isolates, the occurrence of ESBLs was 45.45%, resistance rates were discrepant for amikacin (18.18%), gentamicin (56.82%) and tobramycin (61.36%), the prevalence of phenotype TG (tobramycin and gentamicin) indicative of aac(3)-II production and TGA (tobramycin, gentamicin and amikacin) indicative of aac(6')-I production were 36.36% and 18.18%, respectively. The most common aminoglycoside resistance genotype of acetyltransferase was aac(3)-II (52.27%) and aac(6')-I was lower (29.55%), but no aac(3)-I was detected.CONCLUSION:At least 2 acetyltransferase genes exist in this area i.e. aac(3)-II and aac(6')-I.

Abstract:Aminoglycosides are important broad-spectrum antibiotics used in the therapy of many microbial infections. As the bacterial resistance to antibiotic therapy is appearing as an increasingly significant threat to public health, the development of aminoglycoside antibiotics with extended antibacterial spectrum and potency, devoid of nephro- and ototoxicity, and evading the resistance process returns to the focus of researchers. In this review, various developments brought to the aminoglycoside family of antibiotics effective against resistant bacteria have been described, focused on chemical modifications, drug-modifying enzyme inhibitors, and conformationally constrained analogs, as well as related antibacterial compounds, with the hope to provide information useful in rational design of novel antibiotics addressing bacterial resistance, and paving the way for new perspectives in antimicrobial therapy. The Resistance and Aminoglycoside-Modifying Enzyme Genotypes of High-Level Amino-Glycoside Resistant Enterococcus

Abstract:Objective To determine the antibiotic resistance and aminoglycoside-modifying enzyme genotypes of high-level aminoglycoside resistant Enterococcus strains isolated from clinical specimens. Methods The high-level aminoglycoside resistant (HLAR), high-level gentamicin resistant (HLGR) and high-level streptomycin resistant (HLSR) isolates were screened by the agar screening method. The resistance of these strains to 12 antimicrobial agents was determined by K-B method. The aminoglycoside-modifying enzyme genes were detected by polymerase chain reaction (PCR). Results The prevalence of HLAR was 73.6% in Enterococcus isolates. None of these HLAR isolates was found resistant to linezolid, vancomycin or teicoplanin. The prevalence of resistance to β-lactam antibiotics or quinolones was higher in Enterococcus faecium than in Enterococcus faecalis. The positive rate of aac(6')-Ie-aph(2)-Ia was 92.6% among HLGR isolates. Three HLGR isolates have the resistance gene mostly similar to aph(2)-Id. The rates of 6'-ant, 3-ant and str were all lower in HLSR strains. Conclusions HLAR Enterococcus spp. has become one of the most important antibiotic resistant pathogens of nosocomial infections. The resistance of HLGR is mainly conferred by aminoglycoside-modifying enzymes encoded by aac(6')-Ie-aph(2)-Ia.

Abstract:Aminoglycosides are valuable and broad spectrum of bactericidal antibiotics.However,their therapeutic effectiveness has been severely reduced in recent decades due to the emergence of bacterial strains that are insensitive to aminoglycoside action.The most common mode of bacterial resistance to aminoglycoside antibiotics is the enzyme-catalysed chemical modification on the special groups of the drug.Aminoglycoside-modifying enzymes are widely distributed among bacterial pathogens and include O-phosphoryltransferases(kinases),N-acetyltransferases,and O-adenyltransferases.These enzymes can use several aminoglycosides as substrates regardless of size and structural differences among them.Conversely,each aminoglycoside can be a substrate for many different AGMEs.In this review,the authors describe the thermodynamic characterization of aminoglycoside modified enzyme interacted with antibiotics in an effort to define shared aspects of enzyme-aminoglycoside complexes,which provides the necessary tools and understanding to design new drugs to combat antibiotic resistance.

Abstract:Aminoglycosides are potent bactericidal antibiotics targeting the bacterial ribosome, where they bind to the A-site and disrupt protein synthesis. They are particularly active against aerobic, Gram-negative bacteria and act synergistically against certain Gram-positive organisms. Aminoglycosides are used in the treatment of severe infections of the abdomen and urinary tract, bacteremia, and endocarditis. They are also used for prophylaxis, especially against endocarditis. Bacterial resistance to aminoglycosides continues to escalate and is widely recognized as a serious health threat. This might be the reason for the interest in understanding the mechanisms of resistance. It is now clear that the resistance occurs by different mechanisms such as prevention of drug entry, active extrusion of drugs, alteration of the drug target (mutational modification of 16S rRNA and mutational modification of ribosomal proteins), and enzymatic inactivation through the expression of enzymes, which covalently modify these antibiotics. Enzymatic inactivation is normally due to acetyltransferases, nucleotidyltransferases, and phosphotransferases. In this review, we focus on the recent concept of molecular understanding of aminoglycoside action and resistance.

Abstract:Choosing the appropriate antibiotics to treat bacterial infections has grown more challenging as a result of the emergence of antibiotic-resistant bacteria. Aminoglycosides, as broad-spectrum antibiotics, are increasingly being used clinically; however, for most effective employment of aminoglycosides, a comprehensive understanding of aminoglycoside resistance genes' prevalence and dissemination is required. Therefore, to better understand the global resistance status of aminoglycoside antibiotics and the prevalence of antibioticresistance genes (ARGs) in various bacterial species, this systematic review gathered relevant data from multiple studies. Two primary resistance mechanisms-aminoglycoside enzymatic modification and 16S rRNA methylation-were assessed, and the prevalence of the corresponding ARGs was described. The coexistence of aminoglycoside ARGs with other ARGs was also demonstrated, as was the relationship between aminoglycoside ARGs and resistant phenotypes. The lack of effective therapeutic agents to combat resistant pathogens presents a real threat to public health. Genotypes of Aminoglycoside-Modifying Enzyme and Clinical Study of High-Level Gentamycin Resistant Enterococcus

Abstract:OBJECTIVE:To determine the antibiotics resistance, aminoglycoside-modifying enzymes and homology of high-level gentamycin resistant enterococcus in clinical specimens.METHODS:The high-level gentamicin resistant (HLGR) isolates were screened by the agar method and the resistance of 14 antimicrobial agents was determined by K-B method. The aminoglycoside-modifying enzyme genes were detected by polymerase chain reaction (PCR). Pulsed-field gel electrophoresis (PFGE) was used to analyze the homology of HLGR isolates.RESULTS:The ratio of HLGR was 64.2% (68/106). Among the HLGR,there were no isolates resistant to linezolid, vancomycin and tecoplanin, and Enterococcus faecium was more resistant to beta-lactam antibiotics and quinolone than Enterococcus faecalis. The positive rate of aac(6')-Ie-aph(2')-Ia was 92.6% and 3 isolates had the resistance gene mostly similar to aph(2')-Id. And among 51 HLGR isolates from the hospitalized patients, PFGE grouped 17 E. faecalis isolates into 4 clusters (A-D), and 33 E. faecium isolates into 8 clusters (A-H) with A cluster as predominant.CONCLUSION:HLGR has become the important antibiotic resistance bacteria which results in nosocomial infection; and aac(6')-Ie-aph(2')-Ia is the main aminoglycoside-modifying enzyme gene which causes HLGR.

Abstract:Objective: To investigate the antibiotics resistance of Enterococcus, the aminoglycoside-modifying enzymes (AME) and homology of high-level gentamicin resistant (HLGR) Enterococcus in clinical specimens for the implementation of effective infection control measures.Methods: The resistance of 13 antimicrobial agents was determined by Kirby-Bauer (K-B) or agar dilution method. And the HLGR and high-level streptomycin resistant (HLSR) isolates were screened by agar screen. Production of beta-lactamases was tested by the nitrocefin disc method. The aminogtycoside-modifying enzyme genes were detected by polymerase chain reaction (PCR). Pulsed-field get electrophoresis (PFGE) was used to analyze the homology of HLGR isolates from in-patients.Results: No isolates resistant to linezolid, vancomycin and teicoplanin were found. Ampicillin-resistant isolates did not produce beta-lactamases and 68 HLGR isolates were screened at the rate of 64.2%. The positive rate of aac(6')-Ie-aph(2")-1a was 86.8% and 3 isolates had the new AME gene designated aph(2")-le mostly similar to aph(2")Id. Among 51 HLGR isolates from in-patients, PFGE grouped 17 Enterococcus faecalis (E. faecalis) isolates into 4 clusters (A-D), and 33 Enterococcus faecium (E. faecium) isolates into 8 clusters (A-H), of which the A cluster is the main.Conclusions: HLGR has become the important antibiotic resistance pathogen causing nocosomial infection. A novel aminoglycoside resistance determinant:plasmid-mediated 16S rRNA methylase

Abstract:Aminoglycosides have been used for the treatment of a broad range of life-threatening Gram-positive and Gram-negative bacterial infections.These agents bind to the A site of the 16S rRNA of the bacterial 30S ribosomal subunit and subsequently block growth through interference with protein synthesis.The mechanisms of resistance to aminoglycosides in pathogenic bacteria were previously believed to be restricted to production of aminoglycoside-modifying enzymes,a decrease in intracellular antibiotic accumulation,and the substitution of ribosomal proteins or mutation of rRNA.Plasmid-mediated 16S rRNA methylases,which confer a high level of resistance to various clinically important aminoglycosides,including 4,6-disubstituted deoxystreptamine aminoglycoside,were reported to be involved as part of a novel aminoglycoside resistance mechanism in pathogenic Gram-negative rods.At present,six types of plasmid-mediated 16S rRNA methyltransferase genes,armA,rmtA,rmtB,rmtC,rmtD,and npmA have been found in members of the family Gram-negative bacilli.Also,these genes are mediated by bacterium-specific recombination systems,such as transposons,and are easily translocated to other DNA target sites.The 16S rRNA methylases were supposed to have originated from a self-defense mechanism in aminoglycoside-producing actinomycetes,however,the different G+C content of the methylase genes between Gram-negative bacilli and actinomycetes challenged this theory.The true origination of these six types of methylases requires further study.The selection pressure exerted by a variety of antibiotics will therefore promote the dissemination of genetic elements encoding the methylases.Because of the clinical importance of these enzymes,the further global dissemination of 16S rRNA methylase genes among pathogenic bacilli will be a cause of great concern in the near future.This review is about the mechanism of resistance,classification,genetic environment,and epidemiology of 16S rRNA methylase.

Dozens of aminoglycoside-modifying enzymes are known at the gene sequence level, but few have been characterized in the details of their mechanism. This review summarizes the state of knowledge of the best studied of these enzymes, focusing on their catalytic mechanisms and inhibition.

Abstract:Antibiotic-resistant bacteria are considered one of the major global threats to human and animal health. The most harmful among the resistant bacteria are β-lactamase producing Gram-negative species (β-lactamases). β-lactamases constitute a paradigm shift in the evolution of antibiotic resistance. Therefore, it is imperative to present a comprehensive review of the mechanisms responsible for developing antimicrobial resistance. Resistance due to β-lactamases develops through a variety of mechanisms, and the number of resistant genes are involved that can be transferred between bacteria, mostly via plasmids. Over time, these new molecular-based resistance mechanisms have been progressively disclosed. The present review article provides information on the recent findings regarding the molecular mechanisms of resistance to β-lactams in Gram-negative bacteria, including CTX-M-type ESBLs with methylase activity, plasmids harbouring phages with β-lactam resistance genes, the co-presence of β-lactam resistant genes of unique combinations and the presence of β-lactam and non-β-lactam antibiotic-resistant genes in the same bacteria. Keeping in view, the molecular level resistance development, multifactorial and coordinated measures may be taken to counter the challenge of rapidly increasing β-lactam resistance.
Exciting Bacteria to a Hypersensitive State for Enhanced Aminoglycoside Therapy by a Rationally Constructed AIE Luminogen

Abstract:The diminishing effectiveness of existing aminoglycoside antibiotics (AGs) compels scientists to seek new approaches to enhance the sensitivity of current AGs. Despite ongoing efforts, currently available approaches remain restricted. Herein, a novel strategy involving the rational construction of an aggregation-induced-emission luminogen (AIEgen) is introduced to significantly enhance Gram-positive bacteria's susceptibility to AGs. The application of this approach involves the simple addition of AIEgens to bacteria followed by a 5 min light irradiation. Under light exposure, AIEgens efficiently generate reactive oxygen species (ROS), elevating intrabacterial ROS levels to a nonlethal threshold. Post treatment, the bacteria swiftly enter a hypersensitive state, resulting in a 21.9-fold, 15.5-fold, or 7.2-fold increase in susceptibility to three AGs: kanamycin, gentamycin, and neomycin, respectively. Remarkably, this approach is specific to AGs, and the induced hypersensitivity displays unparalleled longevity and heritability. Further in vivo studies confirm a 7.0-fold enhanced bactericidal ability of AGs against Gram-positive bacteria through this novel approach. This research not only broadens the potential applications of AIEgens but also introduces a novel avenue to bolster the effectiveness of AGs in combating bacterial infections. A novel approach, utilizing a rationally constructed aggregation-induced-emission luminogen (AIEgen), is reported to significantly enhance the susceptibility of Gram-positive bacteria to aminoglycoside antibiotics (AGs). Coexistence of Arma and Genes Encoding Aminoglycoside-Modifying Enzymes in Acinetobacter Baumannii

Abstract:A total number of 104 Acinetobacter baumannii isolates obtained from a teaching hospital were studied for the genetic basis of their resistance to aminoglycosides. The rate of aminoglycoside-resistant A. baumannii was 76.9% (80/104), and 66.25% (53/80) aminoglycoside-resistant isolates were high-level resistant to either aminoglycosides including amikacin, gentamicin and tobramycin (MIC >= 512 mu g/ml). Genes encoding aminoglycoside-modifying enzymes including ant(2")-Ia, aac(6')-Ib, aph(3')-Ia, aac(3)-Ia and aph(3')-VI, and 16S ribosomal RNA (rRNA) methylase armA, rmtA, rmtB, rmtC, rmtD, rmtE and npmA were detected by polymerase chain reaction (PCR), armA gene was present in 50% (52/104) of A. baumannii isolates with high-level resistance to aminoglycosides ( MICs >= 512 mu g/ml). No rmtA, rmtB, rmtC, rmtD, rmtE or npmA genes were found. The rates of ant(2")-Ia, aac(6')-Ib, aph(3')-Ia, aac(3)-Ia and aph(3')-VI were 68.25%,20%,70%,47.5% and 1.25%, respectively. armA, ant(2")-Ia and aph(3')-Ia genes coexisted in 62.5%(50/80) of aminoglycoside-resistant isolates, armA, ant(2")-Ia and aac(6')-Ib were commonly positive in same isolates. The presence of aminoglycoside-modifying enzymes was responsible for the moderate level resistance to aminoglycosides in A. baumannii, whereas armA was responsible for the high-level resistance to aminoglycosides. Aminoglycoside-resistant A. baumannii isolates were classified into genotypes by Random Amplified Polymorphic DNA PCR (RAPD-PCR), pattern A was almost throughout wards of the hospital. In summary, most A. baumannii is resistant to aminoglycosides, in the hospital coexistence of armA and genes encoding aminoglycoside-modifying enzymes are the main reasons.

Abstract:Abstract Infection caused by drug-resistant Staphylococcus aureus is a rapidly developing public health and veterinary concern. Lack of a comprehensive understanding of the mechanisms underlying the emergence of drug-resistant strains, it makes S. aureus one of the most intractable pathogenic bacteria. To identify mutations that confer resistance to anti-S.aureus drugs, we established a laboratory-based adaptive evolution system and performed 10 rounds of evolution experiments against 15 clinically used antibiotics. We discovered a panel of known and novel resistance-associated sites after performing whole-genome sequencing. Furthermore, we found that the resistance evolved at distinct rates. For example, streptomycin, rifampicin, fusidic acid and novobiocin all developed significant resistance quickly in the second round of evolution. Intriguingly, the cross-resistance experiment reveals that nearly all drug-resistant strains have varying degrees of increased sensitivity to fusidic acid, pointing to a novel approach to battle AMR. In addition, the in silico docking analysis shows that the evolved mutants affect the interaction of rifampcin-rpoB, as well as the novobiocin-gyrB. Moreover, for the genes we got in the laboratory evolution, mutant genes of clinical isolates of human had significant differences from the environmental isolates and animal isolates. In conclusion, we believe that the strategy and dataset in this research will be helpful for battling AMR issue of S.aureus, and adaptable to other pathogenic microbes.

Abstract:Bacterial drug resistance is rapidly developing as one of the greatest threats to human health. Bacteria will adopt corresponding strategies to crack the inhibitory effect of antibiotics according to the antibacterial mechanism of antibiotics, involving the mutation of drug target, secreting hydrolase, and discharging antibiotics out of cells through an efflux pump, etc. In recent years, bacteria are found to constantly evolve new resistance mechanisms to antibiotics, including target protective protein, changes in cell morphology, and so on, endowing them with multiple defense systems against antibiotics, leading to the emergence of multi-drug resistant (MDR) bacteria and the unavailability of drugs in clinics. Correspondingly, researchers attempt to uncover the mystery of bacterial resistance to develop more convenient and effective antibacterial strategies. Although traditional antibiotics still play a significant role in the treatment of diseases caused by sensitive pathogenic bacteria, they gradually lose efficacy in the MDR bacteria. Therefore, highly effective antibacterial compounds, such as phage therapy and CRISPER-Cas precision therapy, are gaining an increasing amount of attention, and are considered to be the treatments with the moist potential with regard to resistance against MDR in the future. In this review, nine identified drug resistance mechanisms are summarized, which enhance the retention rate of bacteria under the action of antibiotics and promote the distribution of drug-resistant bacteria (DRB) in the population. Afterwards, three kinds of potential antibacterial methods are introduced, in which new antibacterial compounds exhibit broad application prospects with different action mechanisms, the phage therapy has been successfully applied to infectious diseases caused by super bacteria, and the CRISPER-Cas precision therapy as a new technology can edit drug-resistant genes in pathogenic bacteria at the gene level, with high accuracy and flexibility. These antibacterial methods will provide more options for clinical treatment, and will greatly alleviate the current drug-resistant crisis.

Abstract:Antibiotic resistance has become a major problem in the treatment of gram-positive bacterial infections. These organisms are able to escape antibiotic activity through several mechanisms including beta-lactamase production, altered penicillin-binding proteins, aminoglycoside-modifying enzymes, modification of the target site of the antibiotic, and active efflux. Resistant determinants may be carried on the chromosome, and are transmitted vertically by clonal dissemination, or on mobile elements such as plasmids, transposons or integrons capable of horizontal transfer both within and between species. Several resistant genes can be inserted in a given integron, resulting in multi-drug resistance. Antimicrobial resistance mechanisms can be caused or induced following the widespread use of antibiotics. The most important gram-positive resistant organisms include penicillin-resistant Streptococcus pneumoniae, methicillin-resistant Staphylococcus aureus and coagulase-negative staphylococci, methicillin-resistant Staphylococcus aureus with intermediate resistance to vancomycin, and enterococcal strains that express high-level resistance to aminoglycosides and/or resistance to vancomycin. In several instances, these strains show multi-drug resistance and cannot be treated with currently available agents. Therapeutic strategies include the use of a higher antibiotic dosage, the use of alternative, non-conventional drugs, alone or in combination, after demonstration of their efficacy in vitro and in experimental animal models, and the development of new drugs. New agents available include quinupristin/dalfopristin and line-zolid with activity against most resistant gram-positive bacteria. Promising new drugs that may reach the market in the near future include daptomycin and oritavancin. Careful use of antibiotics and adherence to infection control standards are crucial in preventing the development and the spread of resistant organisms.
RNA polymerase stalling-derived genome instability underlies ribosomal antibiotic efficacy and resistance evolution

Abstract:Bacteria often evolve antibiotic resistance through mutagenesis. However, the processes causing the mutagenesis have not been fully resolved. Here, we find that a broad range of ribosome-targeting antibiotics cause mutations through an underexplored pathway. Focusing on the clinically important aminoglycoside gentamicin, we find that the translation inhibitor causes genome-wide premature stalling of RNA polymerase (RNAP) in a loci-dependent manner. Further analysis shows that the stalling is caused by the disruption of transcription-translation coupling. Anti-intuitively, the stalled RNAPs subsequently induce lesions to the DNA via transcription-coupled repair. While most of the bacteria are killed by genotoxicity, a small subpopulation acquires mutations via SOS-induced mutagenesis. Given that these processes are triggered shortly after antibiotic addition, resistance rapidly emerges in the population. Our work reveals a mechanism of action of ribosomal antibiotics, illustrates the importance of dissecting the complex interplay between multiple molecular processes in understanding antibiotic efficacy, and suggests new strategies for countering the development of resistance.

Abstract:Acinetobacter spp., especially Acinetobacter baumannii, are important opportunistic pathogens responsible for a variety of nosocomial infections, including ventilator-associated pneumonia, bacteraemia, surgical site infections, secondary meningitis, and urinary tract infections. Abstract:The increasing antibiotic resistance is now threatening to take us back to a pre-antibiotic era. Bacteria have evolved diverse resistance mechanisms, on which in-depth research could help the development of new strategies to control antibiotic-resistant infections. Epigenetic alterations and protein post-translational modifications (PTMs) play important roles in multiple cellular processes such as metabolism, signal transduction, protein degradation, DNA replication regulation and stress response. Recent studies demonstrated that epigenetics and PTMs also play vital roles in bacterial antibiotic resistance. In this review, we summarize the regulatory roles of epigenetic factors including DNA methylation and regulatory RNAs as well as PTMs such as phosphorylation and succinylation in bacterial antibiotic resistance, which may provide innovative perspectives on selecting antibacterial targets and developing antibiotics.
