Identification and Functional Analysis of a Novel Germ-Line Mutation in Hmsh2 from a Chinese Hereditary Non-Polyposis Colorectal Cancer Family

Abstract:Objective:To identify a novel germ-line mutation in hMSH2 gene in a Chinese hereditary non-polyposis colorectal cancer (HNPCC) family and to analyze its function. Methods: Genomic DNA of the proband in a Chinese HNPCC family fulfilling Amsterdam Criteria was isolated. Nineteen exons of hMLH1 gene and 16 exons of hMSH2 gene were amplified by PCR. Mutational analysis was performed by denaturing high performance liquid chromatography, which was followed by direct sequencing of PCR fragments for abnormal chromatograms. When missense mutation was found in the proband, blood samples were taken from 50 sporadic colorectal cancer patients and 100 healthy individuals to determine whether it was single nucleotide polymorphisms(SNP) or mutation. Expression of hMLH1 and hMSH2 in proband’s tumor tissue was analyzed by immunohistochemical method. Analysis for microsatellite instability was performed on 5 paired samples of lymphocyte DNA and paraffin embedded or fresh-frozen tumor tissue. The pathological significance of the mutation was predicted by homology modeling. Results: A C-A missense mutation was found in the exon 13 of hMSH2 in the proband, which caused substitution of Ser703 by Tyr. Two patients in this family showed high-frequency microsatellite instability. Immunohistochemistry showed that the expression of hMLH1 was normal and hMSH2 was negative. The mutation site, differing from all other reported mutations, located in the region Ⅳ of hMSH2 and resulted in an abnormal fold of the protein. Conclusion: The germ-line mutation of C.2108CA (p.Ser703Tyr)is a novel mutation firstly identified in a Chinese HNPCC family.
