Early Determinants of Non-Reflow by the Evaluation of the Resolution of ST Segment Elevation in Patients with Acutemyocardial Infarction after Emergency Percutaneous Coronary Intervention

Abstract:Objective: To examine the prognostic implication of ST segment resolution immediately after primary percutaneous coronary intervention (PCI) for ST segment elevation in myocardial infarction (STEMI).Methods:A total of 180 patients with STEMI who underwent successful PCI within 12 hours after the symptom onset were studied. Single-photon emission computed temography ( SPECT ) was performed immediately after PCI and repeated 48 hours later.The myocardial perfusion defect score in SPECT above 4 (≥4) was defined as SPECT sign of non-reflow(NRF). Continuous 12-lead electrocardiogram (ECG) monitoring was put on directly before and 48 hours after PCI. ST segment deviation was measured. The reduction of ST segment elevation in the lead with maximal initial elevation after PCI below 30%(STI_1) or 30% to 70%(STI_2) was defined as ECG sign of NRF. Results: With SPECT method, only 27 patients were detected by NRF, while 36 patients by STI_1 and 42 patients by STI_2. There was no significant difference between STI_1 and STI_2(P0.05).Conclusion:The reduction of ST segment elevation below 30% or 30% to 70% indicates the early useful information about NRF in the individual patient with STEMI within 48 hours after PCI.
