Abstract:Objective To compare and analyze the clinical results of arthroscopic suture-bridge technique combined with or without preserving the articular tissue for treatment of Grade Ⅲ bursal-side partial-thickness rotator cuff tears .Methods From March 2010 to October 2012, the data of 39 patients of Grade Ⅲ bursal-side partial-thickness rotator cuff tears were retrospectively analyzed .All the patients were treated by arthroscopic repair using a suture-bridge technique .According to the quality and tension of the articular tissue during the operation , the patients were divided into two groups:group A of 21 patients and group B of 18 patients.Group A underwent the arthroscopic suture-bridge technique to repair the rotator cuff directly with the normal articular tissue preserved .Group B adopted the arthroscopic conversion to full-thickness tear combining suture-bridge technique to repair the rotataor cuff .Before the operation and at the last follow-up, the visual analog scale ( VAS) was used to assess the pain; American shoulder and elbow surgeons scores ( ASES ) , the Constant shoulder score ( CSS ) , University of California at Los Angeles scores (UCLA) and simple shoulder test ( SST) were used to evaluate and analyze the overall outcomes.The results of the two groups were compared .Results All the 39 patients were followed up for 32.4months (range, 24-52 months) in average.VAS scores were improved from (6.1 ±2.1) in group A and (6.5 ±1.5) in group B before the operation to (0.8 ±0.4) and (1.2 ±1.2) respectively at the last follow-up;ASES increased from (45.8 ±13.7) in group A and (41.4 ±12.3) in group B preoperatively to (93.3 ±3.0) and (90.5 ±7.3) respectively at the last follow-up;CSS increased from (43.1 ±13.8) in group A and (39.1 ±8.6) in group B preoperatively to (81.6 ±5.0) and (81.5 ±7.4) respectively at the last follow-up; UCLA improved from ( 15.0 ±4.0 ) in group A and ( 14.2 ±3.6 ) in group B preoperatively to (32.2 ±2.4) and (31.6 ±2.1) respectively at the last follow-up; SST increased from (4.8 ±1.4) in group A and (4.7 ±1.0) in group B preoperatively to (9.1 ±0.9) and (9.1 ±1.3) respectively at the last follow-up.The differences between the pre-operation data and the data of the last follow-up within the same group were significant ( P<0.05 ) , but the differences in the data of the last follow-up were not significant between the two groups ( P>0.05 ) .According to the patients′satisfaction and the UCLA grading evaluation of curative effects , in group A, six cases were excellent , 13 cases were good and two cases were fair; the excellent and good rate was 90.5%.In group B , four cases were excellent, 13 cases were good and one case was fair; the excellent and good rate was 94.4%.No complication was observed .Conclusions Arthroscopic suture-bridge technique combined with or without preserving the articular tissue in the treatment of Grade Ⅲ bursal-side partial-thickness rotator cuff tears can achieve satisfactory results in selected patients , and the excellent and good rate is over 90%.The clinical outcomes of the patients who adopt directly repair with arthroscopic suture -bridge technique are not statistically different from those who use the method of conversion to full-thickness tear combining suture-bridge technique in this study .
