Short-term efficacy of intracavitary radiotherapy combined with hyperthermia after concurrent chemoradiotherapy in the treatment of locally advanced cervical cancer

Abstract:Objective To compare the difference of the short-term efficacy of intracavitary radiotherapy combined with hyperthermia and intracavitary radiotherapy alone after concurrent chemoradiotherapy in locally advanced cervical cancer.Methods Fifty-four patients with locally advanced cervical cancer were treated from August 2006 to February 2017.After during the pelvic field radiotherapy (50 Gy/25 f) and intracavitary radiotherapy (10 ～ 12 Gy/2 f),the efficacy was evaluated with pelvic MRI.There were 44 patients who did not reach partial response,and these patients were randomly divided into two groups:the combined treatment group (hyperthermia + intracavitary radiotherapy group) and the intraperitoneal radiotherapy group alone.Twenty three patients were rolled in the combined treatment group and 21 patients in the intraperitoneal radiotherapy group.The combined treatment group was treated with intracavitary radiotherapy for 18 ～ 24 Gy/3 ～ 4 f,once a week.At the same time,local deep hyperthemia was performed on the day of intracavitary radiotherapy within 1 hour.Results The objective response rate was 91.3％ (21/23) in the combined treatment group and 90.5％ (19/21) in the intracavitary radiotherapy group.There was no significant difference between the two groups (x2 =0.18,P =0.67).The complete response rate was 82.6％ (19/23) in the combined treatment group and 42.9％ (9/21) in the intracavitary radiotherapy group,the difference was statistically significant (x2 =5.88,P =0.02).Conclusion Intracavitary radiotherapy combined with hyperthermia after concurrent chemoradiotherapy can improve the complete response rate of patients with locally advanced cervical cancer.
