Incidence and related factors of intraocular pressure elevation at early stage after pars plana vitrectomy for retinal detachment

Abstract:Objective: To investigate the incidence and risk factors of intraocular pressure (IOP) elevation at early stage after pars plana vitrectomy (PPV) for retinal detachment.Methods: To retrospectively analyze 228 eyes (227 patients) with retinal detachment who underwent PPV from May 2016 to April2018. Thirty-six eyes (36 patients) with macular epiretinal membrane were included as control group.Gender, intraoperative procedures, intraocular tamponade and high myopia were involved to analyze the risk factors of IOP elevation at early stage after PPV for retinal detachment.Results: A total of 65 eyes with retinal detachment occurred IOP elevation (28. 5%). The incidence of postoperative IOP el-evation was 27. 9%, 10.5%, 34. 4% and 2.8% in patients with photocoagulation, cryotherapy, pho-tocoagulation plus cryotherapy and in control group, respectively (P<0.05). The incidence of IOP el-evation with retinal detachment in silicone oil tamponade and air tamponade was 32.2% and 13.3%, respectively, with significant difference (P<0.05). The incidence of IOP elevation was 48. 6% in pa-tients with high myopia and 19. 8% in patients without high myopia, with significant difference (P<0.01). The risk was significantly increased in patients with photocoagulation plus cryotherapy thera-py, silicone oil tamponade, and high myopia.Conclusion: Intraocular tamponade and high myopia are closely related to the occurrence of ocular hypertension at early stage after PPV for retinal detach-ment. High myopia can increase the risk of intraocular pressure elevation after PPV in patients with retinal detachment. Long-term follow -up should be conducted to avoid visual impairment caused by short-term elevated and long-term chronic elevated intraocular pressure.
