Spiral C T Arthrography: Multiplanar Reconstruction for Evaluating Internal Derangements of the Knee

Abstract:Objective To evaluate the value of spiral CT arthrography multiplanar reconstruction technique (MPR) for diagnosing internal derangements of the knee. Methods 10 bovine knees were used for the experiment. The menisci, anterior and posterior cruciate ligaments and cartilage of the bovine knees were injured artificially either with a hook (5 knees) or after arthrotomy (5 knees). After the incision was carefully sutured, 100 ml of air was injected into the joint, which was then scanned with a PQ6000 spiral computed tomography (CT) scanner. The data obtained were transformed to the work station, and MPR was used to illustrate lesions in the knee. The results of CT diagnosis were compared with what were inspected in the specimens. Clinically, the 10 knees in 9 patients diagnosed as internal derangement of the knee, were evaluated by the same method after 50-70 ml of air was injected into each of the joints. The MPR results were compared to the intraoperative findings. Results Experimentally, the sensitivity and specificity were 88.9% and 93.9% respectively for detection of meniscal abnormalities, 85.7% and 100% for detection of cruciate ligament lesions, and 72.7% and 100% for that of cartilage damage. Clinically, the sensitivity and specificity were 90.0% and 95.0% respectively for detection of meniscal lesion, and were 85.7% and 100% respectively for the ligaments. Conclosion Spiral CT arthrogrophy of the knee combined with MPR offers fine images of internal structures of the knee, with not only clear borders but also internal details of each structure. It is an accurate method for detecting meniscal, cruciate and collateral ligaments and cartilaginous lesions that cause internal derangement of the knee.
