Clinical Comparison of Caudal Postoperative Analgesic Effects after Different Doses of Ketamine Preemptively Injected in Children

Abstract:Objective To compare the efficacy and side-effects following different doses of ketamine(Ket) for caudal analgesia in children. Methods 120 children undergoing hernia repair or orchidopexy procedures under basic and caudal anesthesia,were randomly allocated to 6 groups to receive a caudal injection(n=20 in each). Group A:1% lidocaine,Group B:1% lidocaine+0.15% bupivacaine,Group C:1% lidocaine and 0.25 mg/kg Ket,Group D:1% lidocaine+0.5 mg/kg Ket,Group E:1% lidocaine+0.75 mg/kg Ket,Group F:1% lidocaine+1.0 mg/kg Ket. The injection volume was 1 ml/kg with adrenaline (1/400 000). Postoperative pain/sedation score and incidence of side effects were assessed in 48 h. Results Potent analgesic effect was achieved in groups C,D,E and F (P0.05).The analgesic effect in Groups C and D was similar to that in Group B,but was lower than in Groups E and F (P0.05).The sedation scores were higher in Group E and F, but the incidences of postoperative nausea and vomiting and the spirit side-effect of Ket were also higher than in other groups (P 0.05). Conclusions Ket (≤0.5 mg/kg) preemptive caudal injection may be safely given along with lidocaine basic anesthesia to achieve satisfactory postoperative analgesia in children.
