Abstract:Objective:To promote the rational use of antiplatelet drugs by analyzing the characteristics of platelet aggregation function through the standardized detection technology, and determining the association between platelet aggregation function and age in apparently healthy elderly Chinese.Methods:In this study, 5 658 apparently healthy elderly participants aged ≥60 years were enrolled from 6 hospitals between January 1, 2018 and August 31, 2019. The platelet aggregation rate induced by arachidonic acid (AA) and adenosine diphosphate (ADP) was detected by a standardized automatic platelet aggregation instrument, and the platelet aggregation rate induced by AA and ADP among various age groups (60-69, 70-79 and 80-89 years) was analyzed. Reference intervals were defined according to CLSI document C28-A3 using the nonparametric method ( P2.5 and P97.5 were used to determine the 95% confidence interval). Results:The platelet aggregation rates induced by AA and ADP was similar between hospitals by standardized platelet detection techniques (all P>0.05). Among 5 658 patients, 2 633 (46.54%) received AA-induced platelet aggregation test, and the AA-induced platelet aggregation rate was significantly higher in the 70-79 and 80-90 years groups than that in the 60-69 years group ([72.35±11.67]% and [72.67±11.45]% vs [70.65±12.57]%, all P<0.001), while which was similar between the 70-79 years and 80-90 years groups ( P=0.840). ADP-induced platelet aggregation rate was detected in 3 025 participants (53.46%), and there was no statistically significant difference among all age groups ( P=0.654). The recommended reference interval for AA-induced platelet aggregation was 37.12%-88.47% for people aged 60-69 years, and 37.45%-88.97% for people aged 70-90 years. The recommended reference interval for ADP-induced platelet aggregation rate is 47.95%-92.11%. Conclusions:The results of platelet aggregation are consistent across laboratories using standardized testing systems through standardized testing procedures. It is necessary to establish a reference range for the platelet aggregation rate induced by AA in the elderly people aged 60-69 and older than 70 years, while the platelet aggregation rate induced by ADP does not require a separate reference range for the platelet aggregation rate in the elderly.
