Abstract:Two types of ischemic tolerance in the brain, rapid and delayed, have been reported in terms of the interval between the conditioning and test insults. Although many reports showed that delayed-phase neuroprotection evoked by preconditioning is evident after 1 week or longer, there have been a few investigations about rapidly induced tolerance, and the reported neuroprotective effects become ambiguous 7 days after the insults. The authors examined whether this rapid ischemic tolerance exists after 7 days of reperfusion in a rat focal ischemic model, and investigated modulating effects of the adenosine A 1 receptor antagonist DPCPX (8-cyclopentyl-1,3-dipropylxanthine). Preconditioning with 30 minutes of middle cerebral artery occlusion reduced infarct volume 7 days after 180 minutes of subsequent focal ischemia given after 1-hour reperfusion. The rapid preconditioning also improved neurologic outcome. These beneficial effects were attenuated by pretreatment of 0.1 mg/kg DPCPX, which did not influence the infarct volume after conditioning (30 minutes) or test (180 minutes) ischemia when given alone. The results show that preconditioning with a brief focal ischemia induces rapid tolerance to a subsequent severe ischemic insult, the effect of which is still present after 7 days of reperfusion, and that the rapid ischemic tolerance is possibly mediated through an adenosine A 1 receptor-related mechanism.

Abstract:AIM: To investigate the effects of adenosine A1 receptor antagonist on the cerebral protective action of desflurane short pretreatment. METHODS: The experiment was done in the laboratory of Department of Anesthesiology and Room of Nuclear Magnetic Resonance, Department of Radiology, Xijing Hospital, Fourth Military Medical University of Chinese PLA from January to March 2003. Thirty-six Sprague-Dawley rats were randomly assigned into six groups (n=6): In saline + desflurane group, the rats were treated with 2.5 mL/kg saline through intraperitoneal injection at 30 minutes before the desflurane pretreatment (one minimum alveolar concentration, 60 minutes) In dimethyl sulfoxide + desflurane group, the rats were treated with 2.5 mL/kg dimethyl sulfoxide through intraperitoneal injection at 30 minutes before the desflurane pretreatment. In adenosine A1 receptor antagonist + desflurane group, the rats were treated with 1 mg/kg adenosine A1 receptor antagonist through intraperitoneal injection at 30 minutes before the desflurane pretreatment. In saline + O2 group, the rats were treated with 2.5 mL/kg saline through intraperitoneal injection at 30 minutes before the sucking oxygen pretreatment. In dimethyl sulfoxide + O2 group, the rats were treated with 2.5 mL/kg dimethyl sulfoxide through intraperitoneal injection at 30 minutes before the sucking oxygen pretreatment. In adenosine A1 receptor antagonist + O2 group, the rats were treated with 1 mg/kg adenosine A1 receptor antagonist through intraperitoneal injection at 30 minutes before the sucking oxygen pretreatment. One hour after the ending of pretreatment, all the animals were induced focal cerebral ischemia for 120 minutes with thread occlusion on middle cerebral artery of right-side carotid artery. The alteration of neural system and range of cerebral infarction at 24 hours after reperfusion in animals were observed. RESULTS: Thirty-six rats were living at 24 hours after cerebral ischemia/reperfusion, and were all involved in the result analysis. ① Physiological parameter comparison among every group during pretreatment phase: There was insignificant difference of rectal temperature, partial pressure of oxygen of arterial blood (PaO2), partial pressure of arterial carbon dioxide, blood pressure and blood sugar during pretreatment in every group. ② Alteration of neuroethology: The neuroethology score was (3.00±0.89) points at 24 hours after reperfusion in the adenosine A1 receptor antagonist + desflurane group, which was significantly higher than that in the saline + desflurane group and dimethyl sulfoxide + desflurane group, and had insignificant difference as compared with every sucking oxygen pretreatment group. ③ Range of cerebral infarction: It was (247.9±38.24) mm3 detected with diffusion-weighed imaging It was (301.70±41.47) mm3 detected with TTC staining. They were all significantly higher than those in the saline + desflurane group and dimethyl sulfoxide + desflurane group, and had insignificant difference in every sucking oxygen pretreatment group. There was insignificant difference in the three sucking oxygen pretreatment groups. CONCLUSION: The adenosine A1 receptor antagonist, 8-cyclopentyl-1, 3-dipropylxanthine , can eliminate the cerebral protective effects induced by brief desflurane pretreatment, which indicates that its function may has relation with adenosine A1 receptor.
Preconditioning with Isoflurane Produces Dose-Dependent Neuroprotection Via Activation of Adenosine Triphosphate-Regulated Potassium Channels after Focal Cerebral Ischemia in Rats

Abstract:In this study, we determined whether repeated brief isoflurane (Iso) anesthesia induces ischemic tolerance to focal cerebral ischemia in a dose-response manner and whether the effect is dependent on adenosine triphosphate-regulated potassium channels. In Experiment 1, 40 rats were randomly assigned to 4 groups: control animals received 100% oxygen 1 h/d for 5 days, whereas the isoflurane (Iso)1, Iso2, and Iso3 groups received 0.75%, 1.5%, or 2.25% Iso in oxygen 1 h/d for 5 days. In Experiment 2, 36 rats were randomly assigned to 4 groups: controls received 100% oxygen 1 h/d for 5 days; animals in the Iso and I+G (Iso+glibenclamide) groups received 2% Iso in oxygen 1 h/d for 5 days, and the I+G group received glibenclamide (GLB) (5 mg/kg intraperitoneally) before each Iso pretreatment. Animals in the GLB group received GLB (5 mg/kg intraperitoneally) once a day for 5 days. Twenty-four hours after the last pretreatment, the right middle cerebral artery was occluded for 120 min. Neurologic deficit scores (NDS) and brain infarct volumes were evaluated at 24 h. The NDS and infarct volumes of Iso2 and Iso3 were less than those of the controls (P < 0.05). The infarct volume in Iso3 was smaller than in Iso2 (P < 0.05). The NDS and infarct volume in the Iso group were less than in the control and I+G groups (P < 0.05). There was no statistical difference among the control, I+G, and GLB groups. The study demonstrated that repeated Iso anesthesia induces ischemic tolerance in rats in a dose-response manner. GLB, an adenosine triphosphate-regulated potassium channel blocker, abolished the tolerance induced by Iso.

Abstract:The aim of the present study was to investigate the first protective window of preconditioning with electroacupuncture (EA) against focal cerebral ischemia, and to explore whether adenosine is involved in the rapid tolerance phenomenon. Sixty-four male Sprague–Dawley rats were randomly assigned to eight groups (n=8 in each). Animals in the control group received no treatment, and animals in EA1–EA4 groups received EA at 0.5, 1, 2 and 3h before induction of focal cerebral ischemia, respectively. Rats in DPCPX group were intraperitoneally injected with 1mgkg−1 8-cyclopentyl-1,3-dipropylxanthine (DPCPX), 3h before induction of focal cerebral ischemia. Animals in vehicle group and EA+DPCPX group were pretreated with 1mlkg−1 dimethyl sulfoxide (DMSO, the solvent of DPCPX) and 1mgkg−1 DPCPX 30min before preconditioning with EA, respectively. All rats were anesthetized with 40mgkg−1 pentobarbital sodium intraperitoneally. Animals that required EA preconditioning, received EA with intensity of 1mA and frequency of 15Hz at the Baihui acupoint (GV 20) for 30min. The focal cerebral ischemia was produced by the right middle cerebral artery occlusion (MCAO) for 120min. The neurologic deficit scores (NDS) and brain infarct volumes were evaluated at 24h after reperfusion. All rats survived until 24h after reperfusion. Preconditioning with EA at 2h before induction of focal cerebral ischemia improved neurologic outcome (P<0.05 versus control) and reduced the infarct volume (P<0.01 versus control) at 24h after reperfusion. These beneficial effects were reversed by pretreatment with 1mgkg−1 DPCPX, whereas this agent itself did not affect the NDS and volume in drug-ischemic controls after ischemia. The results show that preconditioning with single EA session induces rapid tolerance to focal cerebral ischemia. The rapid ischemic tolerance appears at 2h (but not at 0.5, 1, or 3h) after preconditioning, and is possibly mediated through an adenosine A1 receptor-related mechanism.

Abstract:Résumé Objectif Explorer le rôle des récepteurs de ľadénosine A 1 dans la tolérance rapide à ľischémie cérébrale induite par le préconditionnement à ľisoflurane. Méthode Nous avons réparti 75 rats en cinq groupes de 15: Témoin, 8-cyclopentyl-1,3-dipropulxanthine (DPCPX), Isoflurane, DPCPX+Isoflurane et Véhicule+Isoflurane. Tous les rats ont subi une occlusion de deux heures de ľartère cérébrale moyenne droite (OACM). Le préconditionnement avec ľisoflurane a été fait une heure avant ľOACM dans les groupes Isoflurane, DPCPX+Isoflurane et Véhicule+Isoflurane par une exposition à un mélange de 1,5 % ďisoflurane et de 98 % ďoxygène pendant une heure. Dans les groupes Témoin et DPCPX, les rats ont été exposés à 98 % ďoxygène une heure avant ľOACM qui a duré une heure. Un antagoniste sélectif des récepteurs de ľadénosine A 1 , DPCPX, a été administré (0,1 mg·kg -1 ) 15 min avant ľexposition à ľisoflurane/oxygène dans les groupes DPCPX et DPCPX+Isoflurane pour évaluer ľeffet de ľantagoniste des récepteurs de ľadénosine A 1 sur le préconditionnement à ľisoflurane. Le sulfoxyde de diméthyle, le solvant de DPCPX, a été administré (1 mL·kg -1 ) 15 min avant ľexposition à ľisoflurane dans le groupe Véhicule+Isoflurane. Les scores de déficit neurologique et les volumes ďinfarctus cérébral ont été évalués 24 h après la reperfusion. Résultats Dans les groupes Isoflurane et Véhicule+Isoflurane, les scores de déficit neurologique étaient plus bas et les infarctus cérébraux de moindre volume que dans le groupe Témoin (P < 0,01). Dans le groupe DPCPX+Isoflurane, les scores de déficit neurologique étaient plus élevés et les infarctus cérébraux de plus grand volume que dans les groupes Isoflurane et Véhicule+Isoflurane (P < 0,01). Conclusion Le préconditionnement avec de ľisoflurane réduit la lésion ischémique cérébrale focale chez les rats et ľantagoniste (DPCPX) des récepteurs de ľadénosine A 1 atténue la neuroprotection induite par le préconditionnement à ľisoflurane.
Effect of adenosine A1 receptor in the rapid ischemic tolerance induced by pretreatment with single electroacupuncture in rats

Abstract:Objective: To investigate whether adenosine is involved in the rapid tolerance induced by pretreatment with single electroacupuncture in rats. Methods: Forty male SD rats (280-320 g) were assigned into five groups(n=8 in each group): control group, 8-cyclopentyl-1,3-dipropylxanthine (DPCPX)- ischemic(DI) group, electroacupuncture-pretreated (EA) group, dimethy 1 sulfoxide(DMSO, solvent of DPCPX) plus electroacupuncture-pretreated group(DME), and DPCPX plus electroacupuncture-pretreated(DPE) group. The rats were injected with 40 mg/kg pentobarbital sodium plus 1 ml/kg saline and 40 mg/kg pentobarbital sodium plus 1 mg/kg DPCPX 3 hours before middle cerebral artery occlusion(MCAO) in control and DI group respectively. The animals were injected with 1 ml/kg saline, 1 ml/kg DMSO and 1 mg/kg DPCPX 30 minutes before pretreatment with electroacupuncture at the Baihui acupoint(百会穴). The animals were treated with electroacupuncture at Baihui acupoint under anesthesia with 40 mg/kg pentobarbital sodium in EA, DME and DPE group respectively. The middle cerebral artery was occluded with 3-0 nylon monofilament 120 minutes after pretreatments, the changes of neurological deficit scores(NDS) was evaluated at 24 hours after reperfusion. The infarct volume was then assessed with 2% 2,3,5-triphenyltetrazolium chloride(TTC) staining after the neurological outcome evaluation. Results: All animals survived. The NDS in EA and DME groups were lower than those in controls (P0.01 and P0.05 respectively), and the infarct volume in EA and DME groups were significantly smaller than that in controls at 24 hours after reperfusion( all P0.05). There was no difference among DI, DPE and controls. DPCPX partially reversed the amelioration of NDS and the reduction of infarct volume that was induced by pretreatment with single electroacupuncture after focal ischemia, whereas the agent itself did not affect the NDS and volume after ischemia. Conclusion: The present study indicates that pretreatment with single electroacupuncture induces acute ischemic tolerance possibly via a partial mediation of the central adenosine receptors in the brain.
Comparison of isoflurane-induced and sevoflurane-induced preconditioning for rapid brain ischemic tolerance in rats

Abstract:AIM:To investigate whether preconditioning with isoflurane(Iso) and sevoflurane(Sevo) could induce rapid tolerance to focal cerebral ischemia in rats and to compare their neuroprotective effects against cerebral ischemic injury.METHODS: Forty-two male SD rats weighing 280-320 g were randomly assigned into 3 groups(n=14,each group).Animals in Control,Iso and Sevo groups received preconditioning with 970 mL/L O2,1 minimum alveolar concentration(MAC,15 mL/L) isoflurane in 970 mL/L O2 and 1 MAC(24 mL/L) sevoflurane in 970 mL/L O2 for 1 h respectively.Four rats of each group were randomly assigned to mean artery pressure(MAP) monitoring and artery blood gas analysis at the onset and the end of preconditioning.At 1h after preconditioning,the other rats were subjected to the right middle cerebral artery occlusion(MCAO)for 2 h,followed by reperfusion for 72 h.The neurological behavior scores(NBS) were assessed at 24,48,and 72 h after reperfusion.The percentage of infarct volume was determined at 72 h after reperfusion.RESULTS: The NBS of Iso group and Sevo group at 24,48 and 72 h after reperfusion were significantly higher than those of Control group(P<0.05).The percentage of infarct volume in Iso group and Sevo group was(31.9±3.7)% and(38.5±2.3)%,significantly lower than that in Control group [(48.6±2.2)%,P=0.001 and P=0.032,respectively].No significant difference was found in NBS and infarct volume between Iso group and Sevo group.CONCLUSION: Preconditioning with 1 MAC isoflurane or 1 MAC sevoflurane induces rapid brain ischemic tolerance in rats subjected to focal cerebral ischemia,with similar neuroprotective effects.

Abstract:Objective To determine if isoflurane has protective effect on brain against focal ischemia-reperfusion injury in rats. Methods Thirty-two male SD rats weighing 280-320 g were anesthetized with intraperitoneal peritobarbital 40 mg·kg-1 . Middle cerebral artery occlusion (MCAO) was performed by surgical exposure of common, internal and external carotid arteries through a longitudinal incision in the neck. A nylon filament (0.25-0.30 mm in diameter) with rounded end was inserted into internal carotid artery and threaded cranially until resistance was felt. The depth of insertion was about 20-22 mm. The animals were randomly divided into four equal groups: sham operation group in which carotid arteries were exposed but no MCAO was performed; ischemia group:the animals inhaled 100% O2 for 30 min before the beginning of MCAO which was maintained for 2 h; isoflurane groups: isoflurane 0.9% (group 3 ) or 1.5 % ( group 4) was inhaled for 30 min before the 2 h MCAO. Tympanic membrane temperature was monitored during ischemia-reperfusion. After MCAO and emergence from anesthesia, neurologic deficit of the animals was assessed by a 0-4 grade scoring system (0 = no deficit, 4 = unable to walk, unconscious). After 2 h MCAO 22 h or 70 h reperfusion the animals were decapitated and brain was immediately removed for determination of infarct size and light and electron microscopic examination. Results The tympanic membrane temperature on the ischemic side was significantly lower than that on the non-ischemic side. The maximal difference in the temperature between the two sides was 0.78℃±0.35℃. Isoflurane inhalation significantly reduced the infarct size at the end of 22 h and 70 h reperfusion. 1.5% isoflurane inhalation was more effective than 0.9% in reducing the infarct size. Light and electron microscopic examination showed that isoflurane significantly reduced neuronal, mitochondrial and endothelial cell damage induced by ischemia-reperfusion injury.Conclusion Inhalation of 0.9% or 1.5% isoflurane can protect brain from focal ischemia reperfusion injury to some extent in rats.

Abstract:INTRODUCTION　The “ischemic tolerance in brain” fou nded by Kitagawa et al[1] is an ideal model to investigate the mechanisms of cerebral ischemic damage and neuroprotection. But exposing patients to brief ischemic events before an antici pated cerebral ischemia occurs seems difficult and unacceptable in clinical practice. Therefore, some investigators studied the possibility of chemical preconditionin g which mimics ischemic preconditioning, and demonstrated that some chemicals coul d induce the ischemic tolerance in brain [2]. However, the clinical applic ation of these chemical substances is yet to be elucidated because of their toxicity or side ef fects.　　Recently, isoflurane and sevoflurane have been proved to be able to mimic ischemic preconditioning in myocardial ischemia[3-6]. Whether they could also induce the same effects in brain has not been reported yet. Therefore, the present stud y was undertaken to testify if isoflurane could induce ischemic tolerance in brain on middle cerebral artery occlusion (MCAO) rat model.
Effects of isoflurane pretreatment on changes of intracellular calcium in cerebral ischemic/reperfusion injury in rats

Abstract:AIM: To investigate the changes of intracellular calcium concentration in vital brain slices after pretreatment with isoflurane in cerebral ischemic/reperfusion rats. METHODS: Focal cerebral ischemia was induced by inserting a suture into the middle cerebral artery of rats. Reperfusion was accomplished by withdrawing the suture after 2 hours' ischemia. The changes of intracellular calcium in vital brain slices were determined by fluorescent Ca~2+-indicator fluo-3/AM. The effects of isoflurane pretreatment were evaluated before cerebral ischemia on the intracellular calcium of cortex and striatum. RESULTS: Intracellular calcium concentration increased significantly both in the cortex and the striatum of the brain after cerebral ischemia/reperfusion. Isoflurane pretreatment given before cerebral ischemia inhibited the elevation of Ca~2+ in the cortex but not in the striatum induced by ischemia/reperfusion injury. CONCLUSION: Isoflurane can inhibit Ca~2+ overloading in cortex induced by cerebral ischemia/ reperfusion, which may be one of the mechanisms of isoflurane neuroprotection.
The Protective Effect of Isoflurane Pretreatment Against Global Cerebral Ischemia-Reperfusion Injury in Rats

Abstract:Objective To investigate if isoflurane pretreatment can protect brain from bilateral hemispheric ischemia-reperfusion injury. Methods Seventy-eight male Wistar rats weighing 250-300 g were randomly divided into 4 groups : (A) sham operation group ( n = 15 ); (B) ischemia-reperfusion group (I/R, n = 21); (C) ischemia-preconditioning group (IP, n = 21) and (D) isoflurane pretreatment group (ISO, n = 21) Group B, C and D were further divided into 3 subgroups according to the duration of reperfusion 6 h, 24 h, 72 h. Global cerebral ischemia was produced by 4-artery occlusion technique. In sham operation group (A) bilateral vertebral and common carotid arteries were exposed but not occluded. In ischemia-reperfusion group (I/R) bilateral vertebral arteries were occluded by cauterization and bilateral common carotid arteries were exposed and clamped for 20 min, then undamped for reperfusion of different duration (6 h, 24 h and 72 h) . In ischemia-preconditioning group (C) ischemia-reperfusion was preceded by 3 min global ischemia. In isoflurane-pretreatment group (D) the animals inhaled 1,5% isoflurane for 2 h before I/R. The animals were sacrificed right after reperfusion and brain was removed immediately for microscopic examination of hippocampal CA1. The number of living neurons (HE staining) and apoptotic neurons (TUNEL) was counted. Results (1) In group B (I/R) the number of living neurons in CA1 was decreasing with duration of reperfusion from 90.2 ± 2.4 (after 6 h reperfusion) to 45.8 ?±4.9 (72 h of reperfusion) ( P 0.01) ; while in group C (ischemia preconditioning) and D (isoflurane-pretreatment) the number of living neurons in CA1 after 72 h reperfusion was 82.1 ±4.1 and 79.9 ± 5.4 respectively. (2) In group B (I/R) the number of apoptotic neurons in CA1 increased with duration of reperfusion. The percentage of apoptotic neurons after 72 h of reperfusion was significantly lower in group C and D than that in group B. Conclusion Isoflurane pretreatment can protect brain by suppressing ischemia-induced neuronal apoptosis.

Abstract:AIM:Aquaporins (AQPs) are water-channels that play important roles in brain water homeostasis and cerebral edema induced by brain injury. This study aimed to investigate the relationship between AQP4, bone morphogenetic protein 4 (BMP4)/Smad1/5/8 signaling pathway and isoflurane post-conditiong, which has effects on brain edema in rats with cerebral ischemia/reperfusion (I/R) injury. METHODS:Cerebral I/R injury was induced in rats by using the middle cerebral artery occlusion (MCAO) model for 90min, followed by 24h of reperfusion. Isoflurane post-conditioning (ISO) group received 90min ischemia and underwent 1.5% isoflurane post-conditioning for 60min after initiating reperfusion. Neurobehavior, brain water content, thionine staining and 2, 3, 5-triphenyl tetrazolium chloride staining were evaluated to measure levels of brain edema and damage. Expressions of AQP4, BMP4, Smad1/5/8 and phosphorylated Smad1/5/8 were detected by using Western blot, quantitative real-time polymerase chain reaction (qRT-PCR), and immunofluorescence (IF) staining. RESULTS:Compared with the Sham group, neurological behavior score, brain infarct volume and water content of MCAO model rats increased with reperfusion injury. However, in the ISO group, cell edema and damage of brain was significantly ameliorated (P<0.01). qRT-PCR showed less AQP4 mRNA expression in the hippocampal tissue of the ISO group than in the I/R group (P<0.01). Western blot and immunofluorescence results showed similar changes in protein levels of both groups. Related protein expressions showed expressions of BMP4 and Smad1/5/8 increased in the ISO group (P<0.01), whereas total Smad1/5/8 expression didn't change in all groups. When BMP4 inhibitor (LDN193189) was injected, expression levels of AQP4 increased and neuronal density decreased (P<0.05). By contrast, expression levels of BMP4 did not change significantly after pre-injection of AQP4 inhibitor (TGN020) (P>0.05), but neuronal density increased (P<0.05). CONCLUSION:Isoflurane post-conditioning may inhibit occurrence of brain edema and reduce cerebral I/R injury through down-regulating expression of AQP4, This process may be related to the activation of BMP4/Smad1/5/8 signaling pathway.
Sevoflurane Postconditioning Reduces Myocardial Reperfusion Injury in Rat Isolated Hearts Via Activation of PI3K/Akt Signaling and Modulation of Bcl-2 Family Proteins.

Abstract:Sevoflurane postconditioning reduces myocardial infarct size. The objective of this study was to examine the role of the phosphatidylinositol-3-kinase (PI3K)/Akt pathway in anesthetic postconditioning and to determine whether PI3K/Akt signaling modulates the expression of pro- and antiapoptotic proteins in sevoflurane postconditioning. Isolated and perfused rat hearts were prepared first, and then randomly assigned to the following groups: Sham-operation (Sham), ischemia/reperfusion (Con), sevoflurane postconditioning (SPC), Sham plus 100 nmol/L wortmannin (Sham+Wort), Con+Wort, SPC+Wort, and Con+dimethylsulphoxide (DMSO). Sevoflurane postconditioning was induced by administration of sevoflurane (2.5%, v/v) for 10 min from the onset of reperfusion. Left ventricular developed pressure (LVDP), left ventricular end-diastolic pressure (LVEDP), maximum increase in rate of LVDP (+d P /d t ), maximum decrease in rate of LVDP (−d P /d t ), heart rate (HR), and coronary flow (CF) were measured at baseline, R30 min (30 min of reperfusion), R60 min, R90 min, and R120 min. Creatine kinase (CK) and lactate dehydrogenase (LDH) were measured after 5 min and 10 min reperfusion. Infarct size was determined by triphenyltetrazolium chloride staining at the end of reperfusion. Total Akt and phosphorylated Akt (phospho-Akt), Bax, Bcl-2, Bad, and phospho-Bad were determined by Western blot analysis. Analysis of variance (ANOVA) and Student-Newman-Keuls’ test were used to investigate the significance of differences between groups. The LVDP, ±d P /d t , and CF were higher and LVEDP was lower in the SPC group than in the Con group at all points of reperfusion ( P <0.05). The SPC group had significantly reduced CK and LDH release and decreased infarct size compared with the Con group [(22.9±8)% vs. (42.4±9.4)%, respectively; P <0.05]. The SPC group also had increased the expression of phospho-Akt, Bcl-2, and phospho-Bad, and decreased the expression of Bax. Wortmannin abolished the cardioprotection of sevoflurane postconditioning. Sevoflurane postconditioning may protect the isolated rat heart. Activation of PI3K and modulation of the expression of pro- and antiapoptotic proteins may play an important role in sevoflurane-induced myocardial protection.

Abstract:Objective To investigate if isoflurane preconditioning induces ischemic tolerance against neuronal damage produced by middle cerebral artery occlusion (MCAO) in rats Methods Thirty male SD rats weighing 350 400g were randomly divided into three groups: control group (no pretreatment,n=10),isoflurane preconditioning group (ISO group) (inhalation of 2% isoflurane and 98% O 2 1h per day lasting 5d,n=10), oxygen preconditioning group (O 2 group) (inhalation of 98% O 2 1h per day lasting 5d ,n=10) Right MCAO was induced by a 3 0 nylon thread with round tip inserted cranially into right internal carotid artery under isoflurane anesthesia and maintained for 120 min The neurologic deficit score (NDS) was evaluated 1, 3, 6, 12, 16 and 24h after reperfusion and the infarct volume was calculated at 24th following reperfusion Results The NDS of ISO group was lower than that of other two groups at each time interval (P0 01) The infarct volume of control, O 2 and ISO group was (147 1±35 7) mm 3, (155 4±32 8) mm 3 and (17 4±20 7) mm 3 respectively The difference between ISO and control groups was significant (P0 001), but there was no significant difference between O 2 and control groups (P0 05) Conclusions Isoflurane pretreatment can induce ischemic tolerance against neuronal damage produced by transient MCAO in rats

Abstract:Objective To investigate the effect of different durations of low-concentration isoflurane anesthesia on the expression of 2B subunits-containing N-methyl-D-aspartate receptors (NR2B) in the hippocampus of mice.Methods Forty C57BL/6 mice, aged 10 weeks, weighing 20-24 g, were randomly divided into 5 groups (n=8 each) using a random number table: control group (group C);0.7％-1.0％ isoflurane inhaled for 30 min group (group I1);0.7％-1.0％ isoflurane inhaled for 2 h group (group I2);0.7％-1.0％ isoflurane inhaled for 4 h group (group I3);0.7％-1.0％ isoflurane inhaled for 6 h group (group I4).Morris water maze test was carried out at 48 h after the end of anesthesia, and the escape latency and frequency of crossing the original platform were recorded.The mice were sacrificed after the end of the test, and the hippocampi were removed for determination of the expression of activated caspase-3 and NR2B (using Western blot analysis).Results Compared with group C, the escape latency was significantly shortened, and the frequency of crossing the original platform was increased in I1 and I2 groups, no significant changes were found in the activated caspase-3 expression, and the expression of NR2B was up-regulated in I1, I2 and I3 groups, and the escape latency was prolonged, the frequency of crossing the original platform was decreased, the activated caspase-3 expression was up-regulated, and the expression of NR2B was down-regulated in group I4.Conclusion The mechanism underlying the effect produced by different durations of low-concentration isoflurane anesthesia on the cognitive function is related to the expression of NR2B in the hippocampus of mice.

Abstract:Aim: To investigate the antinociceptive effect of adenosine agonist R-phenylisopropyl-adenosine (R-PIA) given to conscious rats by intracerebroventricular (ICV) and intrathecal (IT), and identify the effect of R-PIA on minimum alveolar concentration (MAC) of halothane with pretreatment of A 1 receptor antagonist 8-cyclopentyl-1,3-dipropylxanthine (DPCPX) or K + channel blocker 4-aminopyridine (4-AP). Methods: Sprague-Dawley rats were implanted with 24-gauge stainless steel guide cannula using stereotaxic apparatus and ICV method, and an IT catheter (PE-10, 8.5 cm) was inserted into the lumbar subarachnoid space, while the rats were under pentobarbital anesthesia. After one week of recovery from surgery, rats were randomly assigned to one of the following protocols: MAC of halothane, or tail-flick latency. All measurements were performed after R-PIA (0.8–2.0 μg) microinjection into ICV and IT with or without pretreatment of DPCPX or 4-AP. Results: Microinjection of adenosine agonist RPIA in doses of 0.8–2.0 μg into ICV and IT produced a significant dose- and time-dependent antinociceptive action as reflected by increasing latency times and ICV administration of adenosine agonist R-PIA (0.8 μg) reducing halothane anesthetic requirements (by 29%). The antinociception and reducing halothane requirements effected by adenosine agonist R-PIA was abolished by DPCPX and 4-AP. Conclusion: ICV and IT administration of adenosine agonist R-PIA produced an antinociceptive effect in a dose-dependent manner and decreased halothane MAC with painful stimulation through activation of A 1 receptor subtype, and the underlying mechanism involves K + channel activation.

Abstract:BackgroundExposure to pharmacological concentration of inhaled anesthetics such as isoflurane can cause short- or long-term cognitive impairments in preclinical studies. The selective antagonists of the histamine H3 receptors are considered as a promising group of novel therapeutic agents for the treatment of cognitive disorders. In this study, we investigated whether ciproxifan, a nonimidazole antagonist of H3 histamine receptors, could overcome the functional and electrophysiological sequela associated with isoflurane anesthesia. MethodsAdult male Sprague Dawley rats were exposed to 1.4% isoflurane or vehicle gas for 2 h. The memory tests (novel object recognition and passive avoidance) as well as in vivo hippocampal excitatory synaptic potentials were recorded 24 h postanesthesia. Locomotor activity, anxiety, and nociception 24 h after isoflurane were also examined. The drugs (ciproxifan 3 mg/kg or saline) were intraperitoneally injected 30 min prior to the behavioral tests or long-term potentiation induction. ResultsAnimals that were previously (24 h) exposed to 1.4% isoflurane for 2 h displayed no preference for novel objects and had impaired retention of a passive avoidance response at 1 h after sample phase. Treating isoflurane-exposed rats with ciproxifan significantly improved the memory performance, as evidenced by an increased discrimination ratio in objects recognition and prolonged retention time in passive avoidance test. Accordingly, hippocampus long-term potentiation was reduced in animals that received isoflurane, while administration of ciproxifan completely abolished the effect of isoflurane exposure on synaptic plasticity. Neither isoflurane nor ciproxifan altered motor performance, anxiety, and nociceptive responses. ConclusionThese results suggest that H3R in the CNS, probably in the hippocampus, may serve as therapeutic target for improvement of anesthesia-associated cognitive deficits.
Protective Effect of Enflurane, Isoflurane and Sevoflurane on Brain Ischemia in Rat

Abstract:Objective To investigate the protective effects and its mechanisms of enflurane, isoflurane and sevoflurane on rat brain ischemia induced by oxygen-glucose deprivition (OGD) on rat cerebral cortical slices. Methods Slices of the rat cerebral cortex were made and divided into four groups: control group; injury group; volatile anesthetics+OGD injury group; picrotoxin+volatile anesthetics+OGD injury group. The histologic changes of brain slices were assessed by hematoxylin and eosin staining, the degree of injury and the effects of volatile anesthe- tics were quantitatively evaluated by A value of the absorbance at 490 nm of slices, which were co-incubated with TTC for 30 min. Results Compared to the slices of OGD injury groups, the histologic changes of the degenerated neurons (characterized by cytoplasmic swelling and pyknotic nuclei) were ameliorated in the slices preconditioned by volatile anesthetics. Compared to the control groups, OGD injury for 10 min decreased the A value of slices (P 0.01). 1.0 and 2.0 MAC of enflurane, isoflurane and sevoflurane inhibited the decrease of A values induced by OGD injury (P0.01). 50 μmol/L of picrotoxin abolished the increase of A value of slices preconditioned with 2.0 MAC of enflurane, partially decreased the increase A value of slices preconditioned with 2.0 MAC of isoflurane, and had no effect on the increase of A value of slices preconditioned with 2.0 MAC of sevoflurane. Conclusion The preconditioning of enflurane, isoflurane, and sevoflurane for 30 min can protect the rat cerebral cortical slices against 10 min OGD injury. Activation of the GABA A receptor may be contributed to the neuroprotective effects of enflurane and isoflurane.
