Clinical Retrospective Analysis and Comprehensive Evaluation of ABO-incompatible Liver Transplantation

Abstract:Objective To evaluate the curative effect of ABO incompatible liver transplant zation and the influence on prognosis.Method The clinical data of 4136 patients undergoing liver transplantation in our center were summarized during 1999.9-2013.12.According to the ABO blood group,the patients were divided into 3 groups:ABO-identical (ABO-Id,n=3892),ABO-compatible (ABO-C,n =170) and ABO-incompatible (ABO-In,n =74).The incidence of postoperative complications and rejection,and survival rate were retrospectively compared among 3 groups.Result The 3-month,6-month,1-year,3-year and 5-year survival rate in ABO-Id,ABO-C,and ABO-In groups was 90.9％,88.1％,82.1％,71.6％,68.3％; 88.3％,84.5％,79.1％,67.6％,64.3％,and 77.8％,72.4％,66.0％,53.1％,48.3％,respectively.The 3-month,6-month,1-year,3-year and 5-year survival rate in AB~In group was significantly lower than the other two groups (P＜0.05).The incidence of postoperative infection in ABO-ld,ABO-C,and ABO-In groups was 20.5％,22.4％,39.2％,respectively,and that in ABO-In group was significantly higher than the other two groups (P ＜ 0.05).There was no significant difference in the incidence of postoperative biliary complications and vascular complications,acute and chronic rejection among 3 groups (P＞ 0.05).Conclusion With the mature operation technology and more advanced immune inhibitors,we have already effectively decreased the occurrence of postoperative acute and chronic rejection.But due to a higher incidence of postoperative infection,using ABO incompatible liver? still? needs to be cautious.
