Abstract:Background and purpose This study was to investigate the association of H-type hypertension with the odds of incident stroke and stroke death in Chinese adults. Methods 39,165 Chinese adults aged 35 years or older who had no history of stroke at baseline were prospectively followed to determine major cardiovascular events, with an average follow-up of 6.2 years. Using a nested case–control design, this report included 179 incident stroke cases (121 stroke deaths) and 179 controls without vascular events matched by age, sex, community, and length of plasma storage from the cohort. Baseline plasma total homocysteine (tHcy) level was measured for all the included subjects. Hypertensive patients, co-morbid with elevated plasma homocysteine levels, are defined as H-type hypertension. Logistic regression analysis was performed to investigate the associations of H-type hypertension with the odds of incident stroke and stroke death, with adjustment for important covariates. Results When analyzed individually, compared to subjects with tHcy<10 μmol/L, the OR (95% CI) of incident stroke for those with tHcy≥10 μmol/L but <20 μmol/L and tHcy≥20 μmol/L were 1.7 (0.8–3.7) and 2.9 (1.1–8.0), respectively. Hypertension was also independently associated with higher risk of incident stroke: 4.7 (2.8–7.9), compared to those without hypertension. When analyzed together, the highest risk was found among subjects with both hyperhomocysteinemia and hypertension (H-type hypertension): 13.5 (3.0–61.5) for incident stroke and 12.4 (2.7 to 58.1) for stroke death. Conclusion This study provided strong evidence that hyperhomocysteinemia and hypertension were two independent, modifiable risk factors, which acted at least additively to increase the risk of incident stroke and stroke death in Chinese population.
