Relevant factors for hierarchical diagnosis of high risk group of acute pulmonary embolism

Abstract:Objective To explore the value of clinical features ,laboratory examinations and several scoring systems for hierarchical di-agnosis of high risk group of acute pulmonary embolism .Methods Forty-three consecutive subjects with pulmonary embolism were re-cruited from The First Affiliated Hospital Of Soochow University from June 2006 to March 2013 .Twenty-five male and eighteen female subjects were included in this study ,with average age 59.49 ±16.49.According to the relevant standards ,subjects were assigned into high-risk group,intermediate-risk group and low-risk group.Statistical differences of symptomatology ,routine laboratory examinations , the revised Geneva score,pulmonary embolism severity index (PESI),acute physiology and chronic health evaluation (APACHE Ⅱ) were analyzed among three groups .ROC curve was used to evaluate the diagnostic value of those factors for high risk subgroup of acute pulmonary embolism .Results Compared with intermediate-risk APE, high-risk APE had higher heart rate , PESI and HsCRP ( P<0.05).According to ROC curve,PESI cut-off value was 97.The diagnostic sensitivity and specific degrees of high risk subgroup of APE were 100%and 81.5%,respectively.Heart rate cut-off value was 94 bpm.The diagnostic sensitivity and specific degrees of high risk subgroup of APE were 91.7%and 55.6%,respectively.HsCRP cut-off value was 13.7 mg· L-1 .The diagnostic sensitivity and specif-ic degrees of high risk subgroup of APE were 88.3%and 77.8%,respectively.Conclusions PESI is a good diagnositic criteria for hi-erarchical diagnosis of high risk group of acute pulmonary embolism .
