Thrombotic Thrombocytopenic Purpura in Patients with Systemic Lupus Erythematosus

Abstract:Objective:To recognize the clinical and pathological features of thrombotic thrombocytopenic purpura (TTP) in patients with systemic lupus erythematosus. Methods:Cases report and literature reviews were used in this study. Results: Two female patients with systemic lupus erythematosus (SLE) who had severe uremia, microangiopathic hemolytic anemia, thrombocytomia, and cerebral dysfunction due to TTP were investigated. In pathology, both of them showed the hyaline thrombus from afferent arteriole to capillary in the glomeruli, which induced shrinkage and ischemic glomeruli, and severe tubular epithelial lesions-only membranous configuration with the loomed tubular epithelial plasma, but the composition of epithelium cell disappeared in partial renal tissues. Conclusion:Detection of the fragmentation of peripheral red blood cells may help the early diagnosis of TTP. Clinically, TTP should differentiate from hemolytic uremic syndrome, and histologically, it ought to rule out other renal microangiopathic lesions such as malignant hypertension、antiphospholipid antibodys syndrome, scleroderma or mixed connective tissue disease. Our data suggests that intensive therapy including continuous blood purification,plasmapheresis (using fresh frozen plasma) combined with immunosuppressive sequential therapy may be life-saving in patients with lupus related TTP.
