Abstract:Gastrointestinal stromal tumors (GISTs) are the most commonly seen mesenchymal tumors of the gastrointestinal tract. The diagnosis of GISTs rests on histology and immunohistochemistry. Digestive endoscopy is one of the main methods for detecting GISTs, and endoscopic ultrasound may precisely determine its origin and depth of invasion. Surgical radical resection is the gold standard for treatment, while endoscopic resection is a newly developed therapeutic approach in recent years. The biological and clinical features as well as diagnosis and management of GISTs, especially the progress in endoscopic diagnosis and management were reviewed in this article.

Abstract:Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasms of the gastrointestinal tract. In recent years, detection of these subepithelial lesions has improved due to advances in endoscopic imaging techniques. Furthermore, developments in immunohistochemical technologies, allowing for reliable differentiation of GISTs from other subepithelial tumors, have improved the understanding of these lesions significantly. Alongside the emergence of these new technologies, clinical management of GISTs has progressed greatly in the last decade. However, major controversies still exist in various aspects of GIST management, such as diagnosis, treatment, and prognosis. This review article provides the current overview of the research status in the management of GISTs.

Abstract:The clinical symptoms of gastrointestinal stromal tumors ( GISTs) lack of specificity ,which results in difficult early diagnosis .GISTs is not sensitive to radiotherapy or chemotherapy ,and has easy recurrence and metastasis after surgical treatment.With the continuous study of endoscopic surgery ,gene testing and molecular targeted drug therapy ,the treatment pattern of GISTs has undergone a revolutionary change ,which has not only greatly improved the prognosis of patients ,but also offers more treatment options for the patients .At present,the treatment of GISTs has the following problems :the effec-tiveness and safety of endoscopic treatment need to be further confirmed ;the pathogenesis of GISTs is still under continuous exploration .Therefore, we need to develop novel molecular targeted agents to better improve the prognosis of the patients .

Abstract:Gastrointestinal stromal tumors(GISTs) are rarely one of gastrointestinal carcinomas that originate from mesenchymal tissue.GISTs are characterized by expression of CD117 receptor in cells and have variable biological phenotypes ranging from benign to highly malignant.The most common complaints of patients with GISTs are abdominal pain,peptic ulcer,bleeding or/and obstruction in digestive tract.GISTs have an extraordinarily high rate of recurrence after surgical resection and are highly resistant to radiation and standard chemotherapy.The imatinib mesylate(Gleevec) is a selective inhibitor of the tyrosine kinase receptor.The outcome for the patients with GISTs treated by Gleevec was promising in terms of immediate response and survival fraction.Recent research shows that a new targeting therapeutic agent,SU11248,can prolong the survival time of the patients with GISTs who were resistant to Gleevec therapy.

Abstract:Gastrointestinal stromal tumors( GIST),the most common mesenchymal tumor of the digestive tract,are diagnosed according to the detection results of histopathology,immunohistochemisty and gene.Laparoscopic resection is one of the hot topics of localized GIST treatment.Imatinib is a first-line drug for metastatic or recurrent GISTs,but most GISTs patients will develop resistance to it with the prolonging of treatment.Meanwhile other molecular targeted drugs have become second-line drugs for GIST patients with drug resistance and intolerant to imatinib.

Abstract:OBJECTIVE:To investigate the morphological features, immunohistochemical speciality of the gastrointestinal stromal tumors (GISTs), and its histogenesis as well.METHODS:The morphologic characteristics of GISTs were studied in 65 cases using light microscopy and 17 cases by electron microscopy. The expression of c-kit (CD117), CD34, vimentin, SMA, actin, desmin, S-100 and MBP were detected in all the cases with EnVision trade mark staining.RESULTS:Among 65 cases of GISTs, 46 were spindle cell type, 6 epithelioid cell type and 13 mixture type, equivalent to 85.5% (65 of 76) of all of the mesenchymal tumors of gastrointestinal tract admitted in the same period. The epithelioid cell type tumors composed of mainly the epithelioid cells, predominantly short spindle, oval or round in pattern, with an overall eosinophilic cytoplasm by hematoxylin-eosin stain. Focal cytoplasmic vacuolization was often seen. Sometimes signet-ring like cells and cells with a clear cytoplasm were seen in the epithelioid stromal tumor. The tumor cells arranged in interlacing fascicles forming whorls or sometimes cell clusters. Electronic microscopy showed the presence of interdigitating cell processes, in some areas, synapse-like structure and numerous desmosome-like junctions as well as a few gap junctions and small round neurosecretory granules. There were also abundant intermediate filaments and thin filaments of actin-type with longitudinal condensations (dense bodies). All of the 65 stromal tumors were strongly positive for vimentin (100%), 61 out of 65 tumors positive for CD117 (c-kit) (93.8%) and 51 out of 65 positive for CD34 (78.5%). Some cases also expressed SMA, actin, S-100 and MBP.CONCLUSIONS:GISTs were the most frequent mesenchymal tumor seen in the gastrointestinal tract. Under light microscope, the morphology of stromal tumors looks sometimes like a leiomyoma and Schwannoma. The application of immunohistochemical markers (particularly CD117 and CD34) and ultrastructural study are considered necessary for the differential diagnosis. GISTs may originate from the pluripotential precursor cells like the interstitial cells of Cajal.

Abstract:The gastrointestinal stromal tumor(GIST) is the most common mesenchymal neoplasm of the gastrointestinal tract,and was different from leiomyoma,neurogenic tumor,which occurs mainly with the Kit gene and platelet-derived growth factor receptor alpha gene mutation.Targeting KIT has led to a new treatment paradigm in GISTs.Imatinib and sunitinib,both tyrosine kinase inhibitors directed to KIT,were approved for first-and second-line treatment of metastatic and unresectable GISTs.In this article,we will review the molecular classification and targeted drug therapy of GISTs.

Abstract:Mesenchymal gastrointestinal cancers are represented by the gastrointestinal stromal tumors (GISTs) which occur throughout the whole gastrointestinal tract, and affect human health and economy globally. Curative surgical resections and tyrosine kinase inhibitors (TKIs) are the main managements for localized GISTs and recurrent/metastatic GISTs, respectively. Despite multi-lines of TKIs treatments prolonged the survival time of recurrent/metastatic GISTs by delaying the relapse and metastasis of the tumor, drug resistance developed quickly and inevitably, and became the huge obstacle for stopping disease progression. Immunotherapy, which is typically represented by immune checkpoint inhibitors (ICIs), has achieved great success in several solid tumors by reactivating the host immune system, and been proposed as an alternative choice for GIST treatment. Substantial efforts have been devoted to the research of immunology and immunotherapy for GIST, and great achievements have been made. Generally, the intratumoral immune cell level and the immune-related gene expressions are influenced by metastasis status, anatomical locations, driver gene mutations of the tumor, and modulated by imatinib therapy. Systemic inflammatory biomarkers are regarded as prognostic indicators of GIST and closely associated with its clinicopathological features. The efficacy of immunotherapy strategies for GIST has been widely explored in pre-clinical cell and mouse models and clinical experiments in human, and some patients did benefit from ICIs. This review comprehensively summarizes the up-to-date advancements of immunology, immunotherapy and research models for GIST, and provides new insights and perspectives for future studies.

Abstract:Gastrointestinal stromal tumor(GIST) is a gastrointestinal tumor originated from the mesenchymal tissue. There are many factors affecting the prognosis of this disease. With the development of pathological technology and the updated evidence-based medical clues, more elements influencing the prognosis of GIST have been detected. Therefore, it is necessary to summarize the effects of various factors on the prognosis of GIST, such as tumor location, tumor size, nuclear division number, whether the tumor ruptured, surgical method, Ki67, SDH, c-kit mutation, PDGFRA mutation expression and so on. Abstract:Gastrointestinal stromal tumors (GIST) constitute 1-3% of all gastrointestinal malignancies and is the most common mesenchymal tumor of the gastrointestinal tract. Although GIST were first described in the literature in the year 1941, important advances of kit mutation and tyrosine kinase inhibitors were not made to understand and manage GIST until the last decade. Here current advances in research of possible cellular origin, diagnostic biomarkers and prognostic factors of primary GIST are reviewed, and the management of primary duodenal GIST is focused on due to its specific location. It is possible that personalized assessment and therapy will turn out to be another milestone for primary GIST.

Abstract:Gastrointestinal stromal tumor (GIST) is the most common mesenchymal tumor of the gastrointestinal tract. Its pathogenesis is defined by mutations within the KIT and PDGFRα gene. Surgical resection is the only radical treatment at present, but recurrence is common. In recent years, targeted therapy with imatinib mesylate, which inhibits KIT kinase activity, represents the other cornerstone for the treatment of GIST. For resectable GIST, operation combined with neoadjuvant and adjuvant therapy with imatinib mesylate or other tyrosine kinase inhibitors can improve the prognosis of high-risk patients before or after complete resection. For unresectable GIST, targeted therapy with imatinib mesylate can effectively inhibit and ameliorate the progression of GIST.

Abstract:Gastrointestinal stromal tumor (GIST) is the most common mesenchymal tumor of the gastrointestinal tract, which is thought to be insensitive to radiotherapy, so radiotherapy is rarely used in GIST.At present, studies have shown that GIST is not generally resistant to radiotherapy and radiotherapy related side effects of grade 3 or above occur at a low incidence, so application of the appropriate dose of radiation to a large extent can relieve symptoms and improve local control rate.As the first choice for non-surgical treatment towards advanced GIST, tyrosine kinase inhibitor can enhance the radiotherapy sensitivity by inhibiting ERK signal transduction pathway and excessive expression of RAD51 protein, attenuating DNA damage repair.The combined use of radiotherapy and tyrosine kinase inhibitor may increase clinical benefit.In this article, the current application of radiotherapy in gastrointestinal stromal tumors and research progress were reviewed.

Abstract:Objective: To investigate the clinical and pathological features of gastrointestinal stromal tumors (GIST). Methods: 52 cases of GIST were retrospectively reviewed in terms of clinical and pathological features. Results: The gastrointestinal bleeding(65%) was the most common clinical manifestation. Pathologically, our study showed that 35 cases (67%) were spindle cells type, 7 cases(13%) were epithelioid cells type and 10 cases(19%) were mixed cells type. In immunohistochemistry, positive rate for CD117, CD34, SMA, Desmin and S-100 were 94.23%, 78.84%, 48.07%, 44.23% and 9.61% respectively. The pathological diagnosis showed 9 cases (17%) were benign, 20 cases (38%) were borderline, 23 cases (44%) were malignant. Conclusions: GIST has a variety of clinic manifestations and it was misdiagnosed. The detection of CD117 can confirme diagnosis.

Abstract:Objective To review the pathological analysis, clinical diagnosis and surgical treatment of gastrointesti- nal stromal tumor(GIST). Methods A total of 87 cases were detected by the Envision immunohistochemical method, and were diagnosed as GIST. The pathological features, immunohistochemical phenotypes, clinical manifestations and imaging findings were analyzed. Results !Of all the GIST examined, 59 cases were located in the stomach (67.8 %), 23 in the small intestine (26.4 %), 1 in the colon (1.1 %) and 4 in other sites including the mesentery, omentum and retroperitonium (4.6 %). All of the 87 cases were positive for vimentin (87/87, 100 %), 87 cases for CD117 (87 /87, 100 %), 75 cases for CD34 (75/87, 86.2 %), 11 cases for SMA (11/87, 12.6 %), 8 cases for desmin (8/87, 9.2 %), 17 cases for S-100 (17/87, 19.5 %) and 13 cases for NSE (13/87, 14.9%). Eleven cases were diagnosed as benign (12.6%), 18 potentially malignant (20.7 %), 28 of low malignancy (32.2%) and 30 of high malignancy (34.5 %). Seventy six cases were successfully followed- up(87.4%), and 12 cases (13.8%) were confirmed to have recurrence or metastasis. All the patients were submitted to op- eration, and 3 who hed subsequently metastasis or recurrence were treated with imatinib mesylate. Conclusions The diag- nosis of GIST depends on pathological and immunohistochemical examinations. CD117 is the most important marker for the diagnosis of GIST. The size of the tumor and the mitotic frequency of the cells are crucial for the differentiation be- tween benign and malignant tumors. Surgical resection is the treatment of choice for GIST. Imatinib mesylate has shown high efficacy for the treatment of patients with unresectable or metastatic GIST. Long-term follow-up of the GIST patients should be carried out.

Abstract:Gastrointestinal stromal tumor (GIST) is the most common mesenchymal tumor of the gastrointestinal tract. Twenty years ago, tyrosine kinase inhibitors targeting KIT/platelet-derived growth factor receptor α were applied to GIST treatment and achieved great success. Therefore, this rare tumor has attracted great attention of clinicians and researchers. After 20 years of exploration and practice, great progress has been made in the clinical diagnosis and research level of GIST in China. In terms of clinical diagnosis and treatment, the pathological diagnosis system has been established and deepened, and the surgical treatment has gradually tended to be standardized, minimally invasive and integrated. More and more targeted drugs continue to challenge the survival limit of GIST patients, and the standardized diagnosis and treatment model has been initially formed and continuously developed. In terms of GIST related researches, drug clinical research from China has experienced the exploration from imitation to innovation, the clinical research related to surgical treatment is undergoing preliminary exploration. The research related to endoscopic treatment has formed a distinctive style with unique Chinese characteristics, and the translational research has also helped solving the clinical problems. In the future, the clinical diagnosis and treatment of GIST in China will be more standardized, minimally invasive, precise, comprehensive and whole process. The research direction in the field of GIST will be to carry out high-quality multi-center clinical research and targeted laboratory researches for clinical focus issues.

At present, the management of GIST has updated from surgical resection to individualized treatment strategy, which includes surgical resection, adjuvant therapy and neoadjuvant therapy. The purpose of this article is to review the recent advances in adjuvant and neoadjuvant therapy of GIST.

Abstract:Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumors of the digestive system which biological behavior is diverse. It is believed that GIST has malignant potential regardless of its size. Risk grade of GIST is the main criterion for evaluating whether or not targeted therapy is carried out before and after operation. Therefore, definite pathological diagnosis and risk grade before treatment have important clinical value for the treatment of patients with GIST. This paper reviewed the advances in imaging studies on risk grade of GIST.
