Abstract:The first case of Hurthle cell tumor of the thyroid gland(HCT) was described in the literature a century ago. Since then, researches of HCT have been progressing from cellular to molecular level. We reviewed the literatures about HCT researching, with emphasis on differentiated diagnosis of benign and malignant HCT.

Abstract:A 55-year-old female patient underwent surgery for her thyroid lumps 10 years ago and pathological examination confirmed the diagnosis of primary papillary carcinoma of the thyroid. The metastasis of neck lymph nodes was conformed 8 years after operation, and the patient received metastatic lymph nodes resection. During the 3-year follow-up the patient had recurrent tumor. The pathological results showed that the lesion changed from papillary carcinoma to papillary carcinoma accompanied with squamous differentiation and transformed to squamous cell carcinoma of the thyroid at last. Finally the patient died from deterioration of the disease rapidly. From the literature review, it has been shown that the squamous cell carcinoma of the thyroid might originate from adenocarcinoma or squamous metaplasia of papillary carcinoma. Once the transformation occurs, the progress of the disease may be accelerated. This transformation is rare and usually confers a poor prognosis.
Unraveling the Enigma: A Five-Year Comprehensive Analysis of Hurthle Cell Tumors in South India's Tertiary Care Center

Abstract:Background: Hurthle cell tumors of the thyroid gland constitute a rare and enigmatic group of neoplasms, characterized by the presence of Hurthle cells exhibiting abundant eosinophilic cytoplasm and numerous mitochondria. Despite their low incidence, they pose diagnostic challenges and display diverse clinical outcomes. This study aims to provide a comprehensive analysis of the clinicopathological profile of Hurthle cell tumors within a tertiary care center in South India. Methods: Through a retrospective approach, we analyzed cases of Hurthle cell tumors diagnosed and treated at a tertiary care center over a five-year period. Clinical, radiological, and histopathological data were meticulously collected and scrutinized. The study focused on examining demographic details, presenting symptoms, imaging features, cytological findings, surgical management, and postoperative outcomes of the patients. Results: A total of 32 cases of Hurthle cell tumors were identified during the study period. The majority of patients were female (84%), with a mean age of 49.6 years for Hurthle cell carcinoma. Thyroid enlargement and neck mass were the most common presenting complaints. Fine-needle aspiration cytology showed characteristic features suggestive of Hurthle cell tumors in 33% of cases. Total thyroidectomy remains the mainstay surgical approach. Histopathological evaluation confirmed 62.5% of cases as benign adenomas and 37.5% as malignant carcinomas. Among malignant cases, 67% showed capsular invasion and 33% demonstrated vascular invasion. Of the patients, 33.3% received adjuvant radiotherapy. The overall survival rate was 100%. In our study, we found that thyroid nodules larger than 3 cm demonstrated a higher propensity for Hurthle cell carcinoma. Conclusion: Our findings support the multidisciplinary approach in managing Hurthle cell tumors, with a focus on tailored treatment plans for each patient based on individual characteristics. By recognizing the female predominance, assessing nodule size, and employing a combination of thyroidectomy and ablative therapy, clinicians can optimize patient care and contribute to better long-term prognosis and quality of life for those affected by Hurthle cell tumors. Continued research and collaborative efforts are necessary to advance our understanding and refine treatment strategies, paving the way for improved outcomes and enhanced patient management in the future.
Granular cell tumor:a clinicopathologic analysis of 15 cases

Abstract:Purpose To investigate the histogenesis and clinicopathologic features of granular cell tumor(GCT).Methods The clinical and pathologic profiles of 15 GCT cases were evaluated. Immunohistochemical study was performed on all cases.Results Our study included 13 cases of benign granular cell tumor(BGCT)and 2 malignant granular cell tumors(MGCT). There were 5 male patients and 10 female. Ages ranged from 19 to 69 years (mean, 41.6 years), and two patients with MGCT were 67 and 69 years old in this group. The tumor size of BGCT ranged from 0.4 to 5.3 cm (mean 2.3 cm), and the tumor sizes of 2 MGCTs were 6 cm and 14 cm respectively. All cases were mainly manifested as a solitary painless nodule located in the dermal, subcutaneous and submucosal tissues. Three cases of tumors occurred in the waist, two in the subaxillary region, two in the chest wall, one each in the breast, upper arm, uterus, perianal region, vocal cords, esophagus, colon and tongue. Microscopically, the tumor cells of BGCT usually possessed abundant eosinophilic granular cytoplasm, small and dark nuclei. The tumor cells of MGCT usually exhibited the following features: vesicular nuclei with prominent nucleoli, nuclear pleomorphism and spindling of some tumor cells. Immunohistochemical study showed strongly positive staining for NSE, S-100 protein and CD68 in all cases, but negative for SMA and MG. Conclusions GCT is a rare tumor showing an origin from Schwann cell. The majority of MGCT occur in the elderly, and volume of the tumor usually is relatively large.

Abstract:OBJECTIVE:This study aims to evaluate the diagnostic value of ultrasound in thyroid Hürthle cell tumors.METHODS:A retrospective analysis was carried out on 27 patients with thyroid Hürthle cell tumors, in terms of the size, shape, boundary, echo, aspect ratio, cystic degeneration, calcification, peripheral halo sign and blood supply of the tumor, through surgical pathological validation. Then, these were compared with postoperative pathological results.RESULTS:The maximum diameter of the thyroid Hürthle cell tumors is between 0.6 cm and 4.6 cm. It had an oval nodule with clear boundaries, an aspect ratio > 1, and peripheral low-echo halos. Furthermore, 29.6% of tumors have even low-echo nodules without cystic changes, 48.1% and 22.1% have even medium or medium-low mixed echo nodules, and 44.4% have cystic changes in varying degrees. One nodule appeared with "micro-calcification", but pathological results confirmed that it was local collagen. Color Doppler blood flow imaging revealed that 88.8% of the nodules were surrounded with blood flow, filled with rich blood inside, and only 12.2% of the nodules had a little blood inside.CONCLUSION:Thyroid Hürthle cell tumors have nodules with even or uneven echoes on the background of the normal echoes of the thyroid, with an aspect ratio of > 1, clear boundaries and peripheral acoustic halos. Cystic changes, colloid crystallization and fibrosis can be seen inside in varying degrees. Ultrasonography has no significant value for the differential diagnosis of benign and malignant Hürthle cell tumors.

Abstract:Objective To improve the clinic,CT and pathologic knowledge of pleomorphic hyanilizing angiectatic tumor of soft parts(PHAT).Methods Clinical,CT and pathologic characters of PHAT in one case at the right nuchal region were detailedly analysed with literature review.Results The clinical symptoms of PHAT were slight and unspecified.The lesion appeared as a orbicular-ovate tumour with sublobe and clear edge,the density of tumor was uneven on plain scan and uneven enhanced after injected contrast agent,and a part of the tumour was enhanced close to the CT value of blood vessel.The characters of pathology were the presence of clusters of dilated thin-walled vessels,with prominent hyalinization of vessel wall,perivascular and intercellular hyalinization,and pantomorphia neoplastic cell distributed between these dilated vessels.Conclusion PHAT is a low potential malignant soft tissue tumor,it has some specific CT and pathologic characters.

Abstract:Purpose To investigate the clinicopatholoic characteristics of hyalinizing trabecular tumours(HTT) in the thyroid.Methods The clinical and pathologic data of a young man patient were reviewed.The specimen of the tumour was examined by microscopy and immunohistochemical staining for TTF1,Ki-67,Ki-67(MIB-1),E-cadherin,calcitonin,CgA,TFE3,Syn,AFP,EMA,CKpan and P53,with review of the literature.Results The patient presented with a tumour located in the left leaf of the thyroid.Grossly,the tumour showed a solitary round shape with white colour.Microscopically,it showed a trabecular-alveolar growth pattern of polygonal and elongated cells with a clear perinucleolus halo.The nuclei of the tumour featured acidophilic cytoplasmic inclusions.The tumour cells showed intense diffuse cytoplasmic MIB-1 positivity by immunohistochemical analysis,especially along the cell membranes.The cytoplasm packed with bundles of intermediate filaments was found by electron microscopy.Conclusion Tumour cells have a trabecular growth pattern in HTT.The immunohistochemical MIB-1 staining on the tumour cells show a membranous pattern.Sometimes,it is difficult to make differential diagnosis from solid variant of papillary carcinoma and medullary thyroid carcinoma.HTT is a tumour with low malignant potential.
A large series of hyalinizing trabecular tumors: Cytomorphology and ancillary techniques on fine needle aspiration

Abstract:Background: Hyalinizing trabecular tumors (HTTs) are rare, essentially benign, follicular cell-derived thyroid neoplasms characterized by a trabecular growth pattern and nuclear pseudoinclusions. Their cytological findings are misleading, because these tumors are often misinterpreted on fine needle aspirate cytology as malignant lesions, such as papillary thyroid cancer and/or medullary thyroid cancer, leading to unnecessary total thyroidectomy. The aim of this study was to analyze the cytomorphological features and application of ancillary techniques in a series of HTTs. Methods: Of 26 histological cases of HTT collected from September 2001 to December 2018, 18 cases had concomitant cytopathology. Cytological cases were processed with liquid-based cytology (LBC). Immunocytochemistry for HBME-1 and galectine-3 as well as molecular testing for BRAFV600E mutation were performed on both LBC and histological specimens. Results: The 18 lesions with fine needle aspirate cytology ranged in size from 5 to 45 mm. Cytological diagnoses included: 1 benign lesion favoring goiter (5.5%), 4 atypia of undetermined significance (22.2%), 6 follicular neoplasms (33.3%), 5 suspicious for malignancy favoring papillary thyroid cancer (28%), and 2 malignant (11%). Hence, 89% HTT had a negative concordant immunopanel, and they were 100% wild-type BRAFV600E . Conclusion: The majority of our HTTs (83.3%) were diagnosed in the indeterminate Bethesda categories, suggesting that their cytomorphological features pose issues for reaching a conclusive cytological diagnosis. The ancillary test results in our series support the fact that HTT is a benign neoplasm.

Abstract:Background: Hyalinizing trabecular tumors (HTT) are rare follicular cell-derived tumors of the thyroid gland that are infrequently reported in otolaryngology literature. We present here an interesting case of HTT which provides the basis for review of this entity's clinical characteristics, criteria useful in making the diagnosis, and any currently available therapeutic modalities. Case presentation: A 70-year-old Caucasian female underwent a CT scan of her chest and was incidentally found to have a nodule within the right thyroid lobe. Gross examination of the excised thyroid lobe revealed a circumscribed and encapsulated lesion (tan / gritty in texture), confined to the gland. Histologic sections of the lesion revealed a circumscribed neoplasm with a trabecular and organoid architecture associated with abundant dystrophic calcification. Neoplastic cells showed a spindled morphology with clumped chromatin and ample eosinophilic cytoplasm. Conclusions: Histologically, HTT is a follicular cell-derived tumor composed of neoplastic cells arranged in a trabecular pattern with hyalinization and calcification of extracellular material. Distinguishing features of HTT include minimal cytologic atypia with a low nuclear: cytoplasmic ratio, cellular aggregates around hyalinized material, and nuclei with clumped chromatin and occasional grooves and/or pseudoinclusions. Though debated in the literature, the general consensus is that this tumor is a benign entity. It is our hope that additional clinical research will elicit awareness of these rare tumors.

Abstract:Objective: To explore the clinicopathological features, diagnosis and differential diagnosis of hyalinizing trabecular tumor (HTT) of the thyroid. Methods: The four HTT specimens were collected including demographics, clinical information, relevant images, the extent of thyroidectomy, the follow-up and representative pathological data of tumors were available for analysis. In addition, the immumohistochemical staining related to the tumor as well as the BRAF and N-ras mutation analysis were analysed. Results: The mean age of four patients was 47 years old and the mean size of the tumor was 2.8 cm. Most of the patients were asymptomatic, while detecting incidentally by using neck ultrasound test. Ultrasound imaging of all cases showed demarcated substantial hypoechoic nodules in ipsilateral thyroid lobe. Computed Tomography (CT) showed a clear low density shadow in the affected thyroid lobe. Tumors of three cases were located at the left, but the other one was located at the right thyroid gland with a complete fibrous capsule. The cytological features resembled papillary thyroid carcinoma (PTC). The histological test indicated that the tumors had characteristic of trabecular growth pattern with hyalinizing material. The tumor cells were in shape of polygonal, oval or high columnar with an acidophilic or clear cytoplasm. The nuclei were oval with inconspicuous small nucleoli, prominent grooves and pseudoinclusion body in cell nucleus. Mitosis and psammoma bodies were rare to be observed. Cytoplasmic "yellow bodies" were frequently observed. The hyaline material was prominent, with positive periodic acid-Schiff (PAS) and negative Congo red staining. Immunohistochemically, tumor cells were positive for thyroglobulin (Tg), thyroid transcription factor-1 (TTF-1), CD56 and negative for calcitonin, cytokeratin 19 (CK19), HBME-1, S-100 and synaptophysin (SyN). Chromogranin A (CgA) and galectin-3 were expressed weakly in some cases. Staining with the MIB-1 antibody showed membranous/cytoplasmic immunoreactivity. Whereas, another clone of Ki-67 (SP6) showed a common nuclear pattern with an index of <1%. None of the four cases exhibited the BRAF V600E protein reactivity. Gene mutation analysis demonstrated no BRAF and N-ras mutation. There was no evidence of local recurrence or metastasis after 6 to 36 months of follow-up. Conclusions: HTT is an uncommon thyroid tumor with very low malignant potential. It has no particular clinical features, so it's often misdiagnosed in fine needle aspiration cytology (FNAC)/Ultrasonography-guided fine needle aspiration cytology (US-FNAC) and frozen section (FS). Its final diagnosis mainly relies on typical histopathological features and characteristic expression pattern of MIB-1 immunohistochemical staining.
Cytomorphological traits of fine-needle aspirates of hyalinizing trabecular tumor of the thyroid gland: A brief report

Abstract:Background: The incidence of thyroid tumor is increasing, and preoperative diagnosis of hyalinizing trabecular tumor (HTT) is difficult. Aim: To investigate the cytological features of HTT of the thyroid gland. Settings and design: A retrospective observational study. Materials and methods: Ultrasonography, preoperative needle aspiration cytology, postoperative histopathology, immunohistochemistry, and BRAF V600E gene test were performed in five patients with HTT to analyze the pathological characteristics of the patients and review the relevant literature. Results: Four female and one male patients with HTT were recruited. Fine-needle aspiration cytology (FNAC) showed bloodstained background tumor cells with multiple morphologies. The tumor cells exhibited ovoid nuclei, abundant cytoplasm, fine chromatin, nuclear crowding and overlapping, and small nucleoli. Focal nuclear pseudoinclusions and grooves were present. No papillary structures or psammoma bodies were observed. In all cases, tumor cells were radially distributed around the eosinophilic extracellular matrix. In 40% (2 in 5) of cases, trabecular patterns of elongated tumor cells were present, with their nuclei staggered along the longitudinal axis of tumor cells in the trabeculae. FNAC suggested two cases of HTT and three cases of papillary thyroid cancer. Post-operational biopsy indicated they were HTT cases. Conclusion: HTT is a rare thyroid tumor with non-specific clinical manifestations. It can be misinterpreted as papillary thyroid carcinoma by FNAC. However, its cytomorphological traits are helpful in the diagnosis. In combination with FNAC, immunohistochemistry, and molecular testing, HTT can be accurately diagnosed.
Hyalinizing trabecular tumour of the thyroid: fine-needle aspiration cytological diagnosis and correlation with histology

Abstract:Hyalinizing trabecular tumour (HTT) is a rare thyroid neoplasm of follicular cell origin characterised by a trabecular growth pattern and prominent intratrabecular and intertrabecular hyalinisation. These peculiar histological features allow the prompt recognition of this neoplasm in surgical specimens. However, cytological diagnosis of HTT remains elusive and misleading because of overlapping characteristics with other thyroid tumours, particularly papillary thyroid carcinoma (PTC), medullary thyroid carcinoma (MTC) and the newly described non-invasive follicular thyroid neoplasm with papillary-like nuclear features (NIFTP). Nevertheless, the proper recognition of this neoplasm on preoperative cytological preparations is important to avoid unnecessary overtreatment of this indolent lesion. A thorough review of the literature has revealed that the correct diagnosis of HTT in cytological smears is achieved in only 8% of cases. In a further 6% of cases, diagnostic doubt has been indicated. Sixty percent of published cases of HTT have been misdiagnosed as suggestive, suspicious or positive for PTC. These findings underline the difficulties of a cytological-based diagnosis of such entity. In this article we review the cytomorphological features of HTT and their correlation to histological features to provide the reader with the tools to improve diagnostic performance in the identification of HTT on preoperative cytology.
Thyroid Langerhans Cell Histiocytosis Concurrent with Papillary Thyroid Carcinoma: A Case Report and Literature Review

Abstract:Langerhans cell histiocytosis (LCH) is a clonal neoplasm of myeloid dendritic cells, rarely involving the thyroid gland. Papillary thyroid carcinoma (PTC) is the most common histological subtype of thyroid cancer. We report a rare case of a 34-year-old Chinese woman who has LCH with PTC and cervical lymph node metastasis of LCH, with a review of the literature. The patient has thyroid nodules and cervical lymph node enlargement detected by neck ultrasound during physical examination. Fine needle aspiration cytology (FNAC) showed PTC with Hashimoto’s thyroiditis and BRAF V600E mutation after thyroidectomy and lymph node dissection. Histopathological examination suggests that LCH was concurrent with classical PTC, accompanied by LCH cervical lymph node metastasis. No BRAF, HRAS, and TERT promoter mutations were detected in LCH metastatic lesions. The patient is in stable clinical condition currently.
Hyalinizing Trabecular Tumor of the Thyroid Gland, a Diagnostic Challenge in Fine-Needle Aspiration Cytology: Case Report

Abstract:Hyalinizing trabecular tumor (HTT) is a rare thyroid tumor with low to minimal malignant potential. HTT is often misinterpreted as other thyroid tumors, including papillary thyroid carcinoma (PTC) and medullary thyroid carcinoma (MTC), on fine-needle aspiration (FNA) cytology, because of its overlapping cytologic features, such as nuclear grooves and intranulcear pseudoinclusions. Although cytopathologists cannot definitely conclude HTT by FNA cytology, suspicion of HTT is necessary to avoid misdiagnosing HTT as PTC or MTC and to avoid unnecessary aggressive treatment. Here, we report a case of HTT with novel cytologic features in CellPrep liquid based cytology that was diagnosed as suspicious for papillary carcinoma by FNA and finally diagnosed as HTT in the surgical specimen.
Clinicopathologic Analysis of HPV-Related Primary Squamous Cell Carcinoma of the Thyroid

Abstract:Object: This study aims to explore the clinical and pathologic characteristics of HPV-related primary thyroid squamous cell carcinoma (PSCCT), a rare tumor classified by WHO-5 as a subtype of anaplastic thyroid carcinoma (ATC). Methods: Clinical data, histomorphology, immunohistochemistry, HPV detection, and B-raf gene point mutations of 3 PSCCT cases were analyzed. Subsequent follow-up was conducted post-treatment. Results: All 3 cases involved female patients aged between 60 and 76. Microscopic examination revealed squamous cell carcinoma in cases 1 and 3, whereas case 2 exhibited both squamous cell carcinoma and papillary thyroid carcinoma components. Immunohistochemistry demonstrated CK19, PAX8, and TTF1 expression in the papillary thyroid carcinoma component, and CK5/6, p63, p40, and PAX8 expression in the squamous cell carcinoma component. P16 exhibited diffuse positivity in both squamous cell carcinoma and classic papillary carcinoma. HPV analysis identified low-risk type 6 positivity in cases 1 and 3, while both squamous cell carcinoma and papillary carcinoma areas in case 2 were positive for HPV-33. B-raf gene mutation was exclusive to case 2. Conclusion: Diagnosis of PSCCT necessitates multidisciplinary assessment, incorporating clinical symptoms, imaging, histomorphology, and immunohistochemistry. This study, for the first time, reveals the presence of HPV DNA in both PTC and PSCCT, occurring concurrently but separately. Given the limited scope of 3 case reports, definitive conclusions cannot be drawn, warranting further investigation.

Abstract:A hyalinizing trabecular tumor (HTT) of the thyroid gland is a very rare type of tumor. It is usually diagnosed incidentally during the examination for thyroid gland diseases that need thyroidectomy. Here we report a case of HTT in a 60-year-old male patient who presented with anterior neck swelling and underwent total thyroidectomy for a Bethesda category V nodule. The final histologic diagnosis of the left lobe was consistent with a hyalinized trabecular adenoma of the thyroid gland, or paraganglioma-like adenoma. We discuss the clinical picture and diagnostic approach, including the role of fine needle aspiration biopsy, and the pathologic features of HTT, with particular reference to the possible differential diagnosis.

Abstract:OBJECTIVE:To investigate the clinical-pathological characteristics,diagnosis and differential diagnosis of primary squamous cell carcinoma of thyroid.METHODS: The clinical-pathological information was retrospectively reviewed in 5 cases of the tumor.The pathological characteristics were studied with HE,immunohistochemical stains and electron microscopy.RESULTS: Three of 5 cases of primary squamous cell carcinoma of thyroid were female,the mean age was 55 years old,and the average survival was 3-7 months after operation.The main symptoms were trachelophyma,dyspnea,hoarseness and dysphagia.Microscopically,the differentiation degree of squamous cell carcinoma of thyroid was Degree Ⅰ.Extensive infiltration of the peri-thyroidal soft tissue by squamous cell carcinoma was its characteristic.All the cases typically presented positivity for CKpan and CK19.The positive rate of Ki-67 expression was 30%-50%.TTF-1,TG and CD5 were absent in all the cases.Ultrastructure showed that the cells of tumors were heteromorphism,tonofibril in intracytoplasm,and well-developed desmosome between the cells.CONCLUSIONS: Primary squamous cell carcinoma of the thyroid is a rare type of thyroid carcinoma,but highly aggressive.Secondary squamous cell carcinoma and squamous metaplasia of other type of thyroid lesion should be excluded before diagnosis.
The Diagnosis of Hyalinizing Trabecular Tumor: A Difficult and Controversial Thyroid Entity

Abstract:Hyalinizing trabecular tumor (HTT) is a benign, follicular-derived neoplasm composed of thick trabeculae with round or elongated cells having irregular and clear nuclei, and containing intra-trabecular hyaline material. The cytological features of HTT resemble those of papillary carcinoma, which helps explain why these lesions are usually classified as indeterminate/suspicious according to the Bethesda system for reporting thyroid cytology. A review of the literature indicates that reaching the correct preoperative cytologic diagnosis of HTT remains elusive, as the correct interpretation was achieved in only 8% of cases. In contrast, the correct diagnosis posed a less significant diagnostic challenge in the majority of histological series, despite the reported controversy on the relationship of this tumor with papillary and medullary thyroid carcinomas. The aim of this review is to highlight the cytological and histological clues in the diagnosis of HTT, as well as its molecular profile.

Abstract:Hepatocellular adenoma (HCA) is a benign hepatocellular neoplasm, commonly occurs in young women with a history of oral contraceptive use. Complications including hemorrhage and malignant transformation necessitate the need for a thorough understanding of the underlying molecular signatures in this entity. Recent molecular studies have significantly expanded our knowledge of HCAs. The well-developed phenotype-genotype classification system improves clinical management through identifying "high risk" subtype of HCAs. In this article, we attempt to provide updated information on clinical, pathologic and molecular features of each subtype of HCAs.

Clinical analysis of 13 cases of primary squamous-cell thyroid carcinoma

Abstract:Objective: To analyze the clinical features, ultrasonographic manifestations, pathological features, treatment and prognosis of primary thyroid squamous cell carcinoma (PSCTC) and summarize the experience in diagnosis and treatment of this condition. Methods: A retrospective analysis was conducted on patients who were admitted to Zhejiang Cancer Hospital from 2007 to 2021 due to thyroid nodules or thyroid malignant tumors that were ultimately confirmed by postoperative pathology as primary thyroid squamous cell carcinoma. We summarize the general situation, clinical information, laboratory examination, ultrasonic image characteristics, pathological examination, clinical treatment and prognosis of the patients. Results: PSCTC is most often seen in older men and progresses rapidly. In laboratory tests, some patients had elevated levels of tumor markers (CA199, squamous cell carcinoma antigen level), thyroglobulin levels and tumor-related substances, but all these indicators lacked specificity. The ultrasound features of PSCTC are mainly hypoechoic, hard, substantial nodules with gross borders and a grade 1-2 blood flow signal, sometimes with signs of necrosis and calcification. In terms of treatment, PSCTC is mainly surgically resected, though some patients in this study underwent iodine-131 radiation therapy, local radiotherapy, and chemotherapy with unclear results. None of the patients survived for very long after treatment, but the prognosis of patients with highly differentiated squamous carcinoma was significantly better than that of patients with poorly differentiated squamous carcinoma. Papillary thyroid carcinoma may be one of the causes of PSCTC. Conclusion: PSCTC is a malignant tumor with high malignancy and rapid clinical progression. Treatment options are mainly based on surgical resection and can be supplemented with radiotherapy and chemotherapy, but there is still a lack of a standardized treatment management system, and more cases and reports are needed to accumulate data.

Abstract:Background Differentiated high-grade thyroid carcinomas (DHGTCs) are a new diagnostic entity most recently defined in the 2022 World Health Organization's (WHO) Classification of Endocrine and Neuroendocrine Tumors. This new entity has been minimally described in the literature, and additional cases classified as such are missing. Materials and methods Cases of DHGTCs diagnosed at our institution from 2012 to 2022 were identified, and the following were reviewed: cytologic and histologic diagnoses, ancillary testing, immunohistochemical staining, treatments, and patient outcomes. Immunohistochemical staining for Ki67 was performed on selected cases lacking this immunostain. A systematic literature review of the English literature on DHGTCs from 2013 to 2023 was performed using PubMed and Embase. Results Case cohort included 32 cases of DHGTCs, with an average age of 52.6 years (range 17–84 years) and a male:female ratio of 1.3:1. All cases underwent fine needle aspiration (FNA) and were categorized by The Bethesda System for Reporting Thyroid Cytopathology (TBSRTC) as follows: 14 cases as malignant (43.8 %), 10 as follicular neoplasm (31.3 %), 5 as atypia of undetermined significance (15.6 %), 2 as suspicious for malignancy (6.2 %), and 1 as non-diagnostic (3.1 %). The average tumor size was 5.15 cm, and most were papillary thyroid carcinoma (28, 87.5 %), with classic subtype being the most common. Twenty-one cases revealed tumor necrosis and the mitotic activity in lesions without necrosis averaged to 5.5 mitoses per 2 mm 2 (range 0–7). The average Ki67 proliferative index was 5.6 %. Extrathyroidal extension was seen in 17, angioinvasion in 21, lymphatic invasion in 7, and perineural invasion in 1 case. Foci of solid or trabecular growth were identified in five cases. Lymph node metastases at the time of diagnosis were noted in 10 cases and 7 demonstrated distant metastases or locoregional recurrence. To date, 25 patients are alive, and one has died from disease. Conclusions Our institutional experience demonstrates that DHGTC is a rare, but aggressive thyroid tumor subtype that requires consideration in the setting of a well-differentiated thyroid neoplasm to appropriately assess for possible disease recurrence and determination of patient prognosis.
