Effcacy of continuous blood puriifcation combined with Ulinastatin in the treatment of septic shock

Abstract:Objective To investigate the efficacy of continuous blood purification (CBP) combined with Ulinastatin in the treatment of septic shock. Method 72 cases of patients with septic shock treated in ICU of our hospital from March 2015 to April 2016 were selected, all patients were divided into observation group (36 cases) and control group (36 cases) according to the random number table method. The patients of the two groups were all treated with conventional treatment to maintain the average pulse pressure (MAP) 65 mmHg or more. Control group patients were treated with CBP. On the basis of control group, observation group patients were treated with Ulinastatin (20000 U/h, intravenously infused). The patients of the two groups were all treated for 7 days. The adverse reactions and the levels of blood gas indexes, oxygen metabolism indexes, renal function and inflammatory cytokines of the two groups were compared. Result After 7 days of treatment, heart rate (HR), oxygen consumption (VO2) were significantly lower than those before treatment in both groups (P<0.05), MAP, cerebral extraction of oxygen (CEO2), central venous oxygen saturation (ScvO2) were significantly higher than that before treatment in both groups (P<0.05). The levels of interleukin (IL)-1, IL-6, IL-10, C reactive protein (CRP), tumor necrosis factor-α (TNF-α) were significantly lower than those before treatment in both group (P<0.05). After 7 days of treatment, HR and VO2 of observation group were significantly lower than those of control group (P<0.05), MAP, CEO2 and ScvO2 were significantly higher than that of control group (P<0.05). The levels of IL-1, IL-6, IL-10, CRP, TNF-α of observation group were significantly lower than those of control group (P<0.05). There were headache, allergic inflammation, nausea, electrolyte abnormalities and other adverse reactions occurred in both two groups, but there were no statistically significant between the two groups (P>0.05). Conclusion CBP combined with Ulinastatin can get the exact effect on patients with septic shock, effectively improve the hemodynamic condition, blood gas indexes, oxygenation indexes and renal function of patients, reduce the inflammatory response, which is conducive to the prognosis of patients.
