The effect of disease management model on secondary prevention in patients with stable coronary artery disease of non-revascularization treatment by FFR guidelines

Abstract:Objective To investigate the effect of disease management model on secondary prevention in patients with stable coronary artery disease (SCAD) of non-revascularization treatment by fractional flow reserve (FFR) guidelines. Method From January 2014 to December 2015, 216 patients with SCAD were randomly divided into intervention group (n = 108) and control group (n = 108). Intervention measures of intervention group included regular follow-up from the 24 hours after the coronary angiography to 1, 3, 6 months later, and provided telephone consultation, message and health forum, while control group only had irregular follow-up. The conditions of rehabilitation related indicators and secondary prevention of SCAD of the two groups 6 months later were observed. Result 209 patients completed 6-month follow up. After follow-up, the standard-reaching rate of smoking quitting, weight control, blood pressure, blood lipids, blood glucose, heart rate in intervention group was significantly higher than control group (P<0.05). The patient control of medication (aspirin, statin,β-blocker, ACEI/ARB and nitrates) in intervention group were higher than control group (P<0.05). There was no significant difference in event-free survival between the two groups (P>0.05). Conclusion The disease management model can effectively improve the standard-reaching rate of secondary prevention in patients with SCAD of non-revascularization treatment by FFR guidelines.
