Target Vessel Coronary Artery Wedge Pressure Measurement Method ST-segment Elevation Myocardial Infarction in Patients with Coronary Collateral Circulation on the Degree of Myocardial Viability

Abstract:Objective To measure target vessel coronary artery wedge pressure changes in patients with the ST-segment elevation myocardial infarction (STEMI) having underwent primary percutaneous coronary intervention (PCI),compare it with positron emission tomography/X-ray computed tomography (PET-CT) myocardial perfusion imaging,and investigate the clinical significance of target vessel coronary artery wedge pressure level on myocardial viability assessment.Methods Thirty-three patients with first STEMI after primary PCI were collected.According to Rentrop coronary collateral circulation classification method,they were divided into two groups,group 1 (Rentrop collateral grade 0,23 cases) and group 2 (Rentrop collateral grade 1-3,10 cases).Myocardial infarction trial flow and myocardial perfusion grade of the infarct-related artery before and after PCI was evaluated,and coronary artery wedge pressure (Pcw)/mean aortic pressure (Pa) changes were observed by pressure guide wire.7 d after PCI,all patients received PET-CT detection.Results The Pcw/Pa was significantly correlated with 18F-fluorodeoxyglucose (18F-FDG) uptake rate in group 1 (r =-0.561,P =0.010),but there was no correlation in group 2 (r =-0.262,P =0.464).Best cut off value of the Pcw/Pa for 50% of 18F-FDG uptake was 0.33 (area under cure =0.667,sensitivity 42.1%,specificity 92.9%).Conclusions The Pcw/Pa in STEMI may be a useful indicator of the severity of microvascular damage of infarct-related myocardium in patients without developed collateral circulation,but it may not represent the severity of microvascular damage in patients with developed collateral circulation.
