Abstract:Hematopoietic stem cell transplantation(HSCT)has been widely used in clinical practice. It has become an important treatment for hematological disorders,solid tumors,genetic defects and autoimmune diseases.HSCT is associated with severe immune deficiency.As a result,patients receiving HSCT are at hish risk of infections.Innate immunity.including NK cell and dendritic cells.recovers within weeks after transplantation.By contrast.adaptive immunity recovers much slower.B-and T-cell counts return to normal level during the first few months after transplantation.However,T-cell immunity may remain impaired for years.To study the quantitative and functional of recovery of cells in the hematopoietic system is vital important for the understanding of how to accelerate immune reomstitution and to reduce transplant mortality.

Abstract:The timely reconstitution and regain of function of a donor-derived immune system is of utmost importance for the recovery and long-term survival of patients after allogeneic hematopoietic stem cell transplantation (HSCT). Of note, new developments such as umbilical cord blood or haploidentical grafts were associated with prolonged immunodeficiency due to delayed immune reconstitution, raising the need for better understanding and enhancing the process of immune reconstitution and finding strategies to further optimize these transplant procedures. Immune reconstitution post-HSCT occurs in several phases, innate immunity being the first to regain function. The slow T cell reconstitution is regarded as primarily responsible for deleterious infections with latent viruses or fungi, occurrence of graft-versus-host disease, and relapse. Here we aim to summarize the major steps of the adaptive immune reconstitution and will discuss the importance of immune balance in patients after HSCT.

Abstract:Objective To study the immune re-constitution after allogeneic hematopoietic stem cell transplantation (allo-HSCT) in patients with hematological malignancies.Methods From June 2011 to May 2015,65 patients with hematological malignancies were analyzed retrospectively.Lymphocyte subsets were determined by flow cytometry (FCM),including total T lymphocytes (CD3+),helper T cells (CD3+ CD4+),cytotoxic T cells (CD3+ CD8+),CD4/CD8 ratio,nature killer (NK) cells (CD3-CD56+),NKT cells (CD3+ CD56+),B lymphocytes (CD19+),naive T cells (CD3+ HLA-DR+),static T cells (CD3+ HLA-DR-),and regulatory T cells (CD4+ CD25high Foxp3+) on the day 14,28 and 42,and on the month 2,3,6,9,12,15,18 and 24 after allo-HSCT.Results The percentage of CD3+ T cells located normal range after hematological recovery,and its absolute number recovered to normal range at + 15 months.The percentage of CD3+ CD4+ T cells recovered to normal range at + 24 months.However,the absolute number of CD3+ CD4+ T cells did not recover to normal range until 24 months after allo-HSCT.The percentage of CD3+ CD8+ T cells was higher than normal range at + 42 day,and its absolute number was greater than normal range at + 3 months.Hence,low CD4/CD8 ratio was observed for a long period.The re-constitution time points of the percentage and absolute number of CD3+ HLA-DR+ T cells were + 3 months and + 24 months respectively.The re-constitution time points of the percentage and absolute number of CD3 + HLA-DR-T cells were + 2 months and + 15 months respectively.The re-constitution time points of the percentage and absolute number of regulatory T cells were + 12 months and + 15 months respectively.The percentage of NKT cells located in normal range after hematological recovery,and its absolute number retumed to normal range at + 12 months.The re-constitution time points of the percentage and absolute number of B cells were + 9 months and + 18 months respectively.The percentage of NK cells located in normal range after hematological recovery,and its absolute number returned nearly to normal range at + 3 months.Conditioning regimen containing ATG,source of stem cells,CD34+ cell number,GVHD,and CMV reactivation were all associated with immune re-constitution after allo-HSCT.Conclusion Different immune cells showed different re-constitution models after allo-HSCT,and the percentage recovered faster than absolute number for a certain kind of immune cells.Studying immune re-constitution and its associated factors may offer beneficial information for insight into transplantation immunology and improve the management of allo-HSCT.

Abstract:Dendritic cells (DCs) are professional antigen-presenting cells and initial stimulators for immune response. DCs can shape their functions based on their immune states, which are crucial for the balance of immunity and tolerance to preserve homeostasis. In the immune response involved in stem cell transplantation, DCs also play important roles in inducing immune tolerance and antitumor immunity. After the rapid development of stem cell transplantation technology in recent years, the risks of graft rejection, tumor recurrence, and tumorigenicity are still present after stem cell transplantation. It is important to understand the mechanisms of DC-mediated immune tolerance and stimulation during stem cell transplantation. In this review, we will summarize and analyze the regulatory mechanisms of DCs in stem cell transplantation and their application in clinical settings. It may help to promote the innovation in basic theories and therapeutic approaches of stem cell transplantation.

Abstract:As the most important antigen present cells in the processes of post-transplant complications in vivo, dendritic cells (DCs) play an important role in graft versus host disease (GVHD), infection, and relapse. DCs have became one of the most critical problems in the transplantation immune research recently. The reconstitution kinetics of circulating DCs after allogeneic hematopoietic stem cell transplantation (allo-HSCT) and its relationship with the post-transplant complications, even the promoter or inhibitor affecting this process are summarized in this review.

Abstract:Dendritic cells (DCs) induce and regulate adaptive immunity through migrating and maturing in the kidney. In this procedure, they can adopt different phenotypes-rejection-associated DCs promote acute or chronic injury renal grafts while tolerogenic DCs suppress the overwhelmed inflammation preventing damage to renal functionality. All the subsets interact with effector T cells and regulatory T cells (Tregs) stimulated by the ischemia-reperfusion procedure, although the classification corresponding to different effects remains controversial. Thus, in this review, we discuss the origin, maturation, and pathological effects of DCs in the kidney. Then we summarize the roles of divergent DCs in renal transplantation: taking both positive and negative stages in ischemia-reperfusion injury (IRI), switching phenotypes to induce acute or chronic rejection, and orchestrating surface markers for allograft tolerance via alterations in metabolism. In conclusion, we prospect that multidimensional transcriptomic analysis will revolute researches on renal transplantation by addressing the elusive mononuclear phagocyte classification and providing a holistic view of DC ontogeny and subpopulations.

Abstract:Haploidentical hematopoietic stem cell transplantation (HSCT) offers the benefits of rapid and nearly universal donor availability and has been accepted worldwide as an alternative treatment for patients with hematologic malignancies who do not have a completely HLA-matched sibling or who require urgent transplantation. Unfortunately, serious infections and leukemia relapse resulting from slow immune reconstitution remain the 2 most frequent causes of mortality in patients undergoing haploidentical HSCT, particularly in those receiving extensively T cell-depleted megadose CD34(+) allografts. This review summarizes advances in immune recovery after haploidentical HSCT, focusing on the immune subsets likely to have the greatest impact on clinical outcomes. The progress made in accelerating immune reconstitution using different strategies after haploidentical HSCT is also discussed. It is our belief that a predictive immune subset-guided strategy to improve immune recovery might represent a future clinical direction.

Abstract:Objective To study the immune reconstitution after allo-peripheral blood hematopoietic stem cell transplantation(allo-PBSCT), and to find the second category of immunotherapy which is effective in reducing the post-transplantation or postchemotheray residuals. Methods Peripheral blood of 14 patients with allo-PBSCT was analyzed through the hospital clinical laboratory to measure the total white blood cell count. Lymphocyte subsets were enumerated by multiparameter flow cytometry for the first year after transplantation. Results After allo-PBSCT, the relative number of CD3~ + cells, which was significantly decreased during the first month after transplantation, returned to the normal level within 3 months. During the initial months following allo-PBSCT, the number of CD3~+CD4~+ lymphocytes was decreased, but returned to the normal level 6 months later.While the number of CD3~+CD8~+ lymphocytes returned to the normal level by 1 month after transplantation. The relative and absolute numbers of circulating cells expressing CD19 and CD20, two markers of mature B cells, were decreased during the first 3 months following transplantation and increased to a plateau from the third to the sixth month. NK cells were recovered within 2 months after grafting, which was quicker than T and B cells. Conclusion After allo-PBSCT, the relative number of CD3~ + cells would return to a normal level within 3 months. The numbers of B cells could reach a plateau in 3 to 6 months. Recovery of NK cells might be within 2 months after grafting, which is quicker than T and B cells.

Abstract:Objective To study the reconstitution of nature kill cell early after non-myeloablative allogeneic stem cell transplantation (NAST). Methods Cell phenotypes and in vitro immune functions were analyzed by direct immune fluorescence with FCM, T-cell activation test and MTT assay, respectively. Results The percentages of CD+3, CD+4 cells were low, (2.03±15.60) % and (22.69±12.29)%, respectively, while CD+8 lymphocytes were normal or high [(29.26±8.99)%] 1 month after NAST. Proliferative response to a T lymphocyte activator (PHA) was blunted, 94.60±44.87. The percentage of NK cells was within normal limits or high (n=7) in allotransplanted patients. It is (18.77±9.11) %. The percentage of CD+56 NK cells expressing IL-2R (CD25) was high and values obtained from transplanted patients were significantly different from control values . They are (3.71±2.23) % (t = 2.116, P = 0.044). NK cell activity in part of patients was higher than that observed in control cells (25.30±12.39) % vs (16.60±3.53) % (t = 2.135, P= 0.047). Conclusion NK cell has recovered early after transplantation and may be particularly relevant as a first line of defence in immunosurveillance against neoplastic cells or microbial infections, until a full reconstitution of T cellmediated immune response can be achieved.

Abstract:Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a widely used treatment for a variety of hematopoietic disorders, and also provides a valuable platform for investigating the development of donor-derived immune cells in recipients post-HSCT. The immune system remodels from the donor to the recipient during allo-HSCT. However, little is known about the cell profile alterations as donor homeostasis rebalances to recipient homeostasis following HSCT. Here, multi-omics technology is applied at both the single cell and bulk sample levels, as well as spectrum flow cytometry and fluorescent transgenic mouse models, to dissect the dynamics of the rebalanced homeostatic immune system in recipients after allo-HSCT. The data reveal that all immune subpopulations observed in donors are successfully restored in recipients, though with varying levels of abundance. The remodeling of immune homeostasis exhibits different patterns in HLA-matched and haploidentical HSCT, highlighting distinct biases in T cell reconstitution from the central and peripheral pathways. Furthermore, ZNF683 is critical for maintaining the persistence and quiescence of CD8 T-cell in haploidentical HSCT. The research can serve as a foundation for developing novel strategies to induce immune tolerance.

Abstract:Objective To study immune reconstitution and immunotherapy after autologous peripheral blood stem cell transplantation. Methods Fifty-six cases of patients after APBSCT were detected the count and proportion of lymphocyte, content of serum globulin, proportion of CD4 + and CD8 + lymphocyte and CD4 +/CD8 +.Meanwhile, they were treated with IL-2. Results There were significant differences in content of serum globulin,count and proportion of lymphocyte,proportion of CD3 +, CD4 + lymphocyte and CD4 +/CD8 + before and after APBSCT one month. The proportion of CD8 + lymphocyte increased quickly and came to the level before APBSCT after transplantation one month. Of the patients, twelve died of infection or disease relapse three to eighteen months post-transplantation, and other patients are still alive and disease-free survival for five to thirty months.Conclusion The count and function of lymphocyte have obvious reducction after APBSCT. High dose IL-2 has define therapeutic effect and no serious ill effect, fitted clinical widespread.

Abstract:Hematopoietic stem cell transplantation (allo-HSCT) is the only curative treatment for various hematological malignancies and immunodeficiency diseases.Umbilical cord blood transplantation (UCBT),as one of HSCT,has been applied more and more frequently in treatment of many diseases with its many advantages.However,UCBT is associated with delayed immune reconstruction and more infections,due to a low number of nucleated cells.In order to learn more about immune reconstitution in patients after UCBT,this review summarizes our current understanding of the peculiar characteristics and influence factors of T lymphocyte immune reconstitution,and approaches to improve it following UCBT.

Abstract:While organ transplantation has emerged as a successful treatment option for certain human diseases, failure to induce life-long graft tolerance remains the most significant obstacle to achieving optimal post-op outcomes. To better cope with this challenge, it is particularly important to understand and distinguish between the different mechanisms that induce graft rejection. After transplantation, host immune responses can be triggered by both stranger and danger signals. Genetic differences between individuals contribute to the identification of foreign entities by recipient immune cells. The recovery and preservation of organs lead to the cell stress or death, inducing inflammation and further aggravating graft damage. Herein, I aim to summarize the modern understanding of transplant immunology and compare the current medical scheme with innovative strategies. This review focuses on recent progresses in stem cell transplant and compiles technological breakthroughs to improve autologous iPSC therapeutics.

Abstract:The alpha beta T-cell-depleted hematopoietic stem cell transplantation (HSCT) leads to lower relapse and better outcome, and may correlate strongly with expansion of donor-derived gamma delta T cells. gamma delta T cells play an important role in immune reconstitution and can exert a graft-versus-leukemia effect after HSCT. This review showed the recent literature on immune functions of gamma delta T cells after HSCT. The discrepancies between studies of gamma delta T cells in graft-versus-host disease may cause by its heterogeneous and various distinct subsets. And reconstitution of gamma delta T cells may play a potential immunoregulatory role in the infections after HSCT.

Abstract:Natural killer (NK) cells play a significant role in immune tolerance and immune surveillance. Killer immunoglobin-like receptors (KIRs), which recognize human leukocyte antigen (HLA) class I molecules, are particularly important for NK cell functions. Previous studies have suggested that, in the setting of hematopoietic stem cell transplantation (HSCT), alloreactive NK cells from the donor could efficiently eliminate recipient tumor cells and the residual immune cells. Subsequently, several clinical models were established to determine the optimal donors who would exhibit a graft-vs. -leukemia (GVL) effect without developing graft-vs. -host disease (GVHD). In addition, hypotheses about specific beneficial receptor-ligand pairs and KIR genes have been raised and the favorable effects of alloreactive NK cells are being investigated. Moreover, with a deeper understanding of the process of NK cell reconstitution post-HSCT, new factors involved in this process and the defects of previous models have been observed. In this review, we summarize the most relevant literatures about the impact of NK cell alloreactivity on transplant outcomes and the factors affecting NK cell reconstitution.

The transplantation procedure/conditioning regimen generally leads to a prolonged state of immunodeficiency characterized by persisting low levels of naïve T cells. Abstract:OBJECTIVE:To search for differences in early immune reconstitution after allogenic or autologous hematopoietic stem cell transplantation (HSCT). METHODS:The peripheral blood (PB) from 31 adult patients undergoing allogenic HSCT (allo-HSCT, 15 patients) or autologous HSCT (auto-HSCT, 16 patients) for the treatment of hematological malignancies and from 20 related healthy controls (HC) from December 2011 to August 2014 was used to analyze the kinetic recovery of lymphocyte subsets by means of flow cytometry during 12 months after HSCT. The T cell receptor rearrangement excision circle (TREC) levels among CD3(+) T cells were measured in the patients and HC to evaluate the thymic-dependent T cell reconstitution. RESULTS:The allo- and auto-HSCT recipients did not differ significantly in CD4(+) T cells, CD8 naive T cells, effecter memory T cells (TEM), CD4 central memory T cells (TCM), mid-activated T cells and dendritic cells (DC)during the follow-up (P>0.05). But they both differed significantly from HC (P<0.05). CD8(+) T cells and NK cells reconstructed rapidly. There was no significant difference in the numbers of B cells between the allo- and auto-HSCT groups from M1 to M3 (P>0.05). B cells in both the groups were lower than those in HC (P<0.05). The recovery of B cells in auto-HSCT group was faster than in allo-HSCT group at M6 and M12 (P<0.05). The frequencies of CD4 naive T cells and later activated T cells in allo-HSCT group were significantly higher than in auto-HSCT group at M6 and M12 (P<0.05). The frequencies of CD8 TCM in auto-HSCT group were significantly higher than in allo-HSCT group at M6 and M12 (P<0.05). The TREC levels were significantly lower than in both the groups compared with the age-matched HC during the follow-up (P<0.05). No significant difference was observed between allo-HSCT and auto-HSCT groups (P>0.05). CONCLUSION:The differences of the nature and the speed of lymphocyte reconstitution observed between the two patents groups were minor. This leads us to conclude that in allografted patients, immune reconstitution and subpopulations of peripheral blood lymphocytes are probably not related to the allogenicity of the graft, but due to the impaired thymus functions and slow differentiation of T lymphocytes in thymus.
