Abstract:Mycoplasma pneumoniae is an important pathogen of community-acquired pneumonia in children.Although Mycoplasma pneumoniae infection is likely to be self-limited, severe or refractory cases of Mycoplasma pneumoniae pneumonia are increasing, with severe condition and prolonged course, prone to pulmonary and extra-pulmonary complications, some with sequelae, which challenges the treatment.Here, we reviewed the therapeutic progress of Mycoplasma pneumoniae pneumonia.

Abstract:Background: Inflammatory response, oxidative stress, and immunologic mechanism are involved in the pathogenesis of Mycoplasma pneumoniae pneumonia (MPP). However, the role of immune system of pediatric interstitial pneumonia due to M. pneumoniae infections remains poorly understood. The aim of this study was to analyze the immunologic features of pediatric interstitial pneumonia due to Mycoplasma pneumoniae (M. pneumoniae). Methods: A retrospective study was conducted on a primary cohort of children with MPP. Propensity score analysis was performed to match interstitial pneumonia and pulmonary consolidation children. Results: The clinical characteristics strongly associated with the development of interstitial pneumonia were boys, age >5 years, wheezing history, hydrothorax free, lymphocytes (>3.0 × 109/L), CD19+ (>0.9 × 109/L), CD3+ (>2.5 × 109/L), CD4+ (>1.5 × 109/L), CD8+ (>0.9 × 109/L), interleukin-6 (IL-6, <30 pg/ml), IL-10 (<6 pg/ml), and interferon-γ (IFN-γ, <15 pg/ml). After propensity score analysis, children with interstitial pneumonia showed significantly higher CD19+, CD3+, and CD4+ T cell counts, and lower serum IL-6, IL-10, and IFN-γ levels. The final regression model showed that only CD4+ T cells (>1.5 × 109/L, OR = 2.473), IFN-γ (<15 pg/ml, OR = 2.250), and hydrothorax free (OR = 14.454) were correlated with the development of interstitial pneumonia among children with MPP. Conclusions: The M. pneumoniae-induced interstitial pneumonia showed increased CD4+ T cells and lower serum IFN-γ level. Specific immunologic profiles could be involved in the development of pediatric interstitial pneumonia due to M. pneumoniae infections.

Abstract:Mycoplasma pneumoniae(MP)remains to be an important pathogen of childhood community-acquired pneumonia,and also has close relationship to asthma exacerbation. There is a much higher incidence of MP infection in older children than younger children. The latter,although with a lower infection rate,usually show a more severe clinical picture,and is prone to have tachypnea and retractions. High fever and irritative cough are characteristics of MP infections in older children. Serological method is the standard one for diagnosing MP infection,while PCR is more suitable for early diagnosis of MP infection in younger and immunocompromised patients. Macrolide is still the drug of choice for treatment of childhood MP pneumonia,but how to manage the refractory MP pneumonia has become a big challenge to clinical researchers.

Abstract:Mycoplasma pneumoniae is a major cause of communityacquired respiratory infections in children and adults. A number of severe pulmonary complications have been reported with M. pneumoniae infection. These complications include obliterative bronchitis, bronchiectasis, pleural effusion, interstitial fibrosis, lung abscess, acute respiratory distress syndrome (ARDS) and necrotizing pneumonitis. Necrotizing pneumonitis, which has been described as a suppurative complication of Streptococcus pneumoniae infection, is a rare manifestation of M. pneumoniae infection. Herein we report on five pediatric patients with necrotizing pneumonitis due to M. pneumoniae and review the relevant literature.

Abstract:Mycoplasma pneumoniae( MP) is a frequent cause of community-acquired bacterial respiratory infections in children and adult. There are three kinds of diagnosis methods for MP infection: culture,serology and PCR assay. At the present,serology and PCR assay are the two main laboratory tests for MP infection. Tetracyclines,fluoroquinolones and macrolides are three main drugs for MP treatment in clinics. In recent years,it has been reported that some MP strains are acquired resistance to these antimicrobials.The strains with resistance to macrolides and fluoroquinolones have been reported in vitro. Macrolides-resistant isolates are significantly increasing in clinics. However,no clinical isolates resistant to fluoroquinolones have been reported in the literature.

Abstract:Mycoplasma pneumoniae , an obligate parasitic pathogen without cell wall, can cause severe upper and lower respiratory tract symptoms. It is the pathogen of human bronchitis and walking pneumonia , and named community-acquired pneumonia. In addition to severe respiratory symptoms, there are clinical extrapulmonary manifestations in the skin, brain, kidney, musculoskeletal, digestive system, and even blood system after M . pneumoniae infection. Hereby, we comprehensively summarized and reviewed the intrapulmonary and extrapulmonary pathogenesis of M. pneumoniae infection. The pathogenesis of related respiratory symptoms caused by M. pneumoniae is mainly adhesion damage, direct damage including nutrient predation, invasion and toxin, cytokine induced inflammation damage and immune evasion effect. The pathogenesis of extrapulmonary manifestations includes direct damage mediated by invasion and inflammatory factors, indirect damage caused by host immune response, and vascular occlusion. The intrapulmonary and extrapulmonary pathogenic mechanisms of M. pneumoniae infection are independent and interrelated, and have certain commonalities. In fact, the pathogenic mechanisms of M. pneumoniae are complicated, and the specific content is still not completely clear, further researches are necessary for determining the detailed pathogenesis of M. pneumoniae . This review can provide certain guidance for the effective prevention and treatment of M. pneumoniae infection.

Abstract:Genus Mycoplasma, belonging to the class Mollicutes, encompasses unique lifeforms comprising of a small genome of 8,00,000 base pairs and the inability to produce a cell wall under any circumstances. Mycoplasma pneumoniae is the most common pathogenic species infecting humans. It is an atypical respiratory bacteria causing community acquired pneumonia (CAP) in children and adults of all ages. Although atypical pneumonia caused by M. pneumoniae can be managed in outpatient settings, complications affecting multiple organ systems can lead to hospitalization in vulnerable population. M. pneumoniae infection has also been associated with chronic lung disease and bronchial asthma. With the advent of molecular methods of diagnosis and genetic, immunological and ultrastructural assays that study infectious disease pathogenesis at subcellular level, newer virulence factors of M. pneumoniae have been recognized by researchers. Structure of the attachment organelle of the organism, that mediates the crucial initial step of cytadherence to respiratory tract epithelium through complex interaction between different adhesins and accessory adhesion proteins, has been decoded. Several subsequent virulence mechanisms like intracellular localization, direct cytotoxicity and activation of the inflammatory cascade through toll-like receptors (TLRs) leading to inflammatory cytokine mediated tissue injury, have also been demonstrated to play an essential role in pathogenesis. The most significant update in the knowledge of pathogenesis has been the discovery of Community-Acquired Respiratory Distress Syndrome toxin (CARDS toxin) of M. pneumoniae and its ability of adenosine diphosphate (ADP) ribosylation and inflammosome activation, thus initiating airway inflammation. Advances have also been made in terms of the different pathways behind the genesis of extrapulmonary complications. This article aims to comprehensively review the recent advances in the knowledge of pathogenesis of this organism, that had remained elusive during the era of serological diagnosis. Elucidation of virulence mechanisms of M. pneumoniae will help researchers to design effective vaccine candidates and newer therapeutic targets against this agent.

Abstract:Introduction: Mycoplasma pneumoniae (MP) is a major pathogen of community-acquired pneumonia in children. However, the specific pathogenesis of the progression of Mycoplasma pneumoniae pneumonia (MPP) is unclear. We aimed to reveal the landscape of microbiota and the host immune response in MPP. Methods: This self-controlled study analyzed the microbiome and transcriptome of bronchoalveolar lavage fluid (BALF) from the severe side (SD) and opposite side (OD) of 41 children with MPP from January to December 2021 and revealed the differences of the peripheral blood neutrophil function among children with mild MPP, severe MPP, and healthy children through transcriptome sequencing. Results: The MP load or the pulmonary microbiota had no significant difference between the SD group and OD group, and the deterioration of MPP was related to the immune response, especially the intrinsic immune response. Discussion: The immune response plays a role in MPP, which may inform treatment strategies for MPP.

Abstract:Mycoplasma are the smallest prokaryotic microbes present in nature. These wall‑less, malleable organisms can pass through cell filters, and grow and propagate under cell‑free conditions in vitro. Of the pathogenic Mycoplasma Mycoplasma pneumoniae has been examined the most. In addition to primary atypical pneumonia and community‑acquired pneumonia with predominantly respiratory symptoms, M. pneumoniae can also induce autoimmune hemolytic anemia and other diseases in the blood, cardiovascular system, gastrointestinal tract and skin, and can induce pericarditis, myocarditis, nephritis and meningitis. The pathogenesis of M. pneumoniae infection is complex and remains to be fully elucidated. The present review aimed to summarize several direct damage mechanisms, including adhesion damage, destruction of membrane fusion, nutrition depletion, invasive damage, toxic damage, inflammatory damage and immune damage. Further investigations are required for determining the detailed pathogenesis of M. pneumoniae.

Abstract:Objective To summarize the characteristics of epidemiology,clinic and radiography of mycoplasma pneumoniae pneumonia(MPP) in children.Methods Two hundred and one MPP patients were selected as our subjects who hospitalized in Kailuan General Hospital Affiliated to Hebei United University from Jan.to Dec.2013.All clinical data were recorded.Results Of 412 children with pneumonia,201 children were developed MPP(48.97％).There was significant difference among different age groups regarding of MPP incidence rate(x2 =62.04,P ＜ 0.05),and the highest incidence was in school-age group(71.59％).There was significant difference among the different month,and the MPP cases were more occurred at January,July,September,October and November.The main clinical manifestations were cough (201cases,100％),fever(136 cases,67.66％),moist rales (168 cases,83.58％),breathing (58 casses,28.86％).And parts of children (23.38％) were complicated with extra pulmonary symptoms.The blood WBC of most MPP patients were without obvious increase and blood WBC of 136 cases(67.66％) was less than 10 × 109/L.The cases with hsCRP (＜ 5 mg/L or-10 mg/L) accounted for 36.32％ or 22.39％.Imaging examination results showed that 64 cases were with bronchovascular shadow,73 cases with spot and schistic shadow,64 cases with lobar pneumonia and 3 cases with pleural effusion.Conclusion Clinicians sholud make the early diagnosis and treatment of MPP based on epidemiology,clinical and image comprehensive characteristics of MPP.

Abstract:Mycoplasma pneumoniae (MP) is a common pathogen that causes respiratory infections. Mycoplasma pneumoniae pneumonia (MPP) accounts for 10% to 40% of community-acquired pneumonia in children under 18 years old. [ 1 ] MPP usually has mild symptoms and manifests as a self-limited disease. However, in recent years, an increasing number of severe MPP [ 2 , 3 ] and refractory MPP (RMPP) [ 4 ] have emerged in clinical practice, posing great challenges to pediatricians. RMPP is a disease that does not improve after 7 days of treatment with standard macrolide drugs. [ 5 ] Macrolides are the preferred drugs for treating childhood MP infections, but with increasing clinical use, drug resistance issues have become more severe, and RMPP cases are also increasing year by year. [ 6 ] The most common clinical manifestations of MPP include dry cough and fever, usually accompanied by headache, myalgia, sore throat, as well as laboratory tests (mainly elevated inflammatory markers) and abnormal imaging tests (chest computed tomography shows thickening of the bronchial wall, central lobular nodules, lymph node enlargement or ground glass attenuation, etc). [ 7 ] The duration of fever and hospitalization in RMPP is much longer, and the likelihood of developing extrapulmonary complications, including pleural effusion and multiple organ dysfunction, may also lead to more serious long-term sequelae, including obliterative bronchiolitis and bronchiectasis, [ 8 , 9 ] posing a serious threat to the life and health of children. [ 10 ]

Abstract:Mycoplasma pneumoniae is an important cause of respiratory tract infections in children as well as adults that can range in severity from mild to life-threatening. Over the past several years there has been much new information published concerning infections caused by this organism. New molecular-based tests for M. pneumoniae detection are now commercially available in the United States, and advances in molecular typing systems have enhanced understanding of the epidemiology of infections. More strains have had their entire genome sequences published, providing additional insights into pathogenic mechanisms. Clinically significant acquired macrolide resistance has emerged worldwide and is now complicating treatment. In vitro susceptibility testing methods have been standardized, and several new drugs that may be effective against this organism are undergoing development. This review focuses on the many new developments that have occurred over the past several years that enhance our understanding of this microbe, which is among the smallest bacterial pathogens but one of great clinical importance.

Abstract:Objective To investigate the effect of TNF-α,IL-10 and CysLTs on the pathogenesis of Mycoplasma pneumoniae pneumonia(MPP).Methods The levels of TNF-α,IL-10 and CysLTs in the serum of 40 MPP patients during acute stage,23 MPP patients during convalescence stage,20 children with bacterial pneumonia and of 20 healthy children were respectively determined by ELISA method,and compared.Results The serum CysLTs level of MPP patients during acute stage was higher than that of the healthy children,and reduced along with the convalescence stage but still higher than that of the healthy children.The serum IL-10 level of MPP patients during acute and convalescence stage were both lower than that of the healthy children.There were no significant differences between MPP patients during acute or convalescence stages and healthy children in the serum TNF-α level.Conclusions The changes of CysLTs and IL-10 in the serum may be one of the reasons of the MPP immunological damage.
