Abstract:Objective To assess the morphologic and immunophenotypic characteristics of intraductal proliferative lesions and differentiation of committed stem cell in mastectomy specimens of breast carcinoma and their correlation with invasive carcinoma.Methods Forty-three cases of breast carcinoma with much tissues adjacent to the tumor in mastectomy specimens and 30 cases of benign lesions in breast(control group) were randomly selected at the same period.Immunohistochemical staining of the specimens was performed with antibodies against CK5/6,CK34βE12,α-SMA,CK8,and S-100 protein.Results Intraductal proliferative lesions were demonstrated in 37 of 43 cases in the group of breast carcinoma,including 19 cases of ordinary-type ductal hyperplasia,15 cases of columnar cell change / hyperplasia,7 cases of atypical ductal hyperplasia,2 cases of flat epithelial atypia and 27 cases of ductal carcinoma in situ;19 cases with the lesions more than two types and 4 cases with four types of the lesions.Twenty-five cases of ordinary-type ductal hyperplasia,5 cases of columnar cell change / hyperplasia and 1 case of atypical ductal hyperplasia were demonstrated in the control group.Immunohistochemical staining showed that CK5/6 was obviously expressed in glandular epithelial cells of ordinary-type ductal hyperplasia,while it was not expressed in columnar cell change / hyperplasia,atypical ductal hyperplasia,flat epithelial atypia and ductal carcinoma in situ.CK34βE12 was expressed more strongly than CK5/6 in ordinary-type ductal hyperplasia,1 case of low-grade ductal carcinoma in situ expressed CK34βE12 focally.S-100 protein was obviously expressed in glandular epithelial cells of ordinary-type ductal hyperplasia while these cells did not express α-SMA.The expression of S-100 protein in the myoepithelial cells was similar to that of α-SMA in ordinary-type ductal hyperplasia.Among the 27 cases of ductal carcinoma in situ,expression of S-100 protein was much lower than that of α-SMA in the myoepithelial cells in 6 cases,and no expression was detected in 12 cases,while these myoepithelial cells distinctly expressed α-SMA.Conclusions 86.0% of invasive breast carcinoma coexisted with various type of intraductal proliferative lesions which had different morphologic and immunophenotypic characteristics.They were corresponding to the immnunophenotypes in the differentiation process of committed stem cells.These characteristics may be helpful to diagnosis and differential diagnosis.
