Abstract:Abstract Background Hereditary hemorrhagic telangiectasia (HHT) is a rare autosomal dominant inherited vascular disorder that can involve multiple organs, thus can be associated with so many clinical departments that proper screening and diagnosis of HHT are needed for providing better management of both patients and their family members. Case presentation: we present a 58-year-old female patient with recurrent paradoxical brain embolism due to HHT. Though ischemic stroke caused by HHT-induced vascular disorders has been reported, our patient presented with two neurological complications at the same time: recurrent paradoxical brain embolisms and bilateral globus pallidus manganese deposition, which is rarely reported. We also review the literature on the clinical features and management of HHT for prompt diagnosis of this genetic disease behind paradoxical embolism. Conclusions When patients with ischemic stroke, especially recurrent ischemic stroke, have combined AVMs in single or multiple organs, or clues for AVMs like manganese deposition in globus pallidus, genetic diseases such as HHT may be the reason for ischemic stroke and shouldn't be missed in the evaluation of embolic sources.

Central Pontine Myelinolysis In A Patient With Epilepsia Partialis Continua And Hyperglycaemic Hyperosmolar State

Abstract:Central pontine myelinolysis (CPM) is a demyelinating disorder, which is associated most commonly with the rapid correction of hyponatraemia and other abrupt changes in physiological osmotic conditions. This includes the treatment of hyperosmolar hyperglycaemia in diabetes mellitus (DM) sufferers. Herein, we report a case of CPM in a 55-y-old patient with new-onset DM who presented with partial focal seizures and a sudden-onset right-sided hemiplegia. Magnetic resonance imaging revealed a lesion in the central pons. The patient responded to glucose control and antiepileptic medication, and achieved a recovery of limb function within 17 d of admission. CPM occurred in this patient before the correction of hyperglycaemic hyperosmolar state, and a disturbance in his initial electrolytes was not found. This report is the first documented case of long-term hyperglycaemic hyperosmolar state leading to CPM, and highlights that CPM can present as an isolated hemiplegia.

Abstract:Macular telangiectasia type 2 (MacTel 2) is an idiopathic entity which is characterized by dilated capillary network near,or around,the fovea of the macula.The spectrums secondary to macular retinal vascular inflammation or obstruction are excluded.It is a binocular disease in general.MacTel 2 is now widely recognized as a macular neurodegenerative disease rather than a simple vascular disease.This article reviews the epidemiological features,clinical features,staging,special examinations,pathogenesis and treatment of MacTel 2.

Abstract:Background: Epidermal nevus syndrome is a group of congenital neuroectodermal and/or mesodermal disorders characterized by the epidermal nevi in common association with cerebral, eye, skeletal, cardiovascular, and renal abnormalities. Epidermal nevus syndrome is a rare syndrome, and epidermal nevus syndrome with the mutation of PTCH1 gene and cerebral infarction is even rarer and has not been reported to the best of our knowledge. Case presentation: We report the case of a 10-month-old Chinese female patient who presented to our pediatric neurologic department, University of Wenzhou medical teaching Hospital, Hangzhou. She has mobility disorders on the right limbs and recurrent seizures. She had congenital disorder accompanied by brownish-black and verrucose plaques on the right side of the face as well as extensive brownish-black plaques and brown nevi on the right side of the trunk and the right arm. Epidermal nevus syndrome was diagnosed on the basis of her symptoms. Somatic sebaceous nevi and hypoplastic defects of skin, cerebra, eyes, skeleton, and cardiovascular and renal system were observed. However, in addition to the typical clinical characteristics, the patient also has a mutation (c.109G > T) in PTCH1 gene and cerebral infarction. We present a novel case report and literature review. Conclusion: To our knowledge, epidermal nevus syndrome with a mutation of PTCH1 gene and cerebral infarction has not been reported previously. This case report may contribute to characterizing the phenotype of epidermal nevus syndrome, help clinicians be aware of the association of this condition with PTCH1 gene and cerebral infarction, raise clinical suspicion, and improve early therapy.

Abstract:Aim:To explore the clinical and imaging characteristics of central pontine myelinolysis(CPM) induced by Sjgren's syndrome.Methods:Clinical data of one patient in our hospital and another case reported previously were analyzed retrospectively.Results:Clinical manifestation of the patient was CPM-a rare manifestation of CNS Sjgren's syndrome.Compared to another patient previously reported,the patient is a 41- year-old woman with Sjgren's syndrome who presented with distal renal tubular acidosis with severe hypokalemia and hypokalemic myopathy.The patient developed neurological symptoms including dysarthria, dysphagia and flaccid quadriparesis.ANA,anti-Ro antibodies were positive.Brain MRI showed a cleft in the mid ports which was equal-intense on T1 and hyperintense on T2.Serial MRI showed diminution of the lesion compared with initial enlargement after successful immunosuppressive therapy with methyprednisolone. Conclusion:CPM is a rare manifestation of CNS Sjgren's syndrome which possibly involved not alterations of sodium ions concentrations but self-immunological disorders,and the clinical improvement can be paralled by radiographic improvement after the immuno-supressive therapy.

Abstract:Hypereosinophilic syndrome (HES), characterized by an increased number of eosinophils in tissues and/or blood, presents with heterogeneous clinical manifestations. Studies have shown that HES can affect the nervous system and may be associated with cerebral infarction. Patent foramen ovale (PFO) is the most common congenital intracardiac defect that can cause right-to-left shunting and contribute to the paradoxical embolization of venous emboli, and even lead to stroke. We report the case of a young man who presented with cerebral infarction accompanied by both HES and PFO. The patient underwent thorough evaluation to determine the source of emboli and the potential pathogenesis. In this case, HES was confirmed and glucocorticoid treatment was conducted. Direct imaging using optical coherence tomography (OCT) confirmed that the embolus originated from the PFO. Therefore, we performed PFO occlusion. The patient recovered well, and no new cerebral infarction was observed at 6-month follow-up. Based on the results of our study, we conclude that it is important to consider unusual etiologies of cerebral infarction, particularly in younger patients.

Abstract:Patient one was a 38-year-old man with a 20-year history of red macules and angiotelectasis on the chest and the upper arms. Dermatological examination showed multiple telangiectatic macules on the chest and the upper arms, with a negative Darier's sign. Histopathology revealed mild epidermal atrophy, hyaline degeneration and scattered vasodilation in the superficial dermis. Perivascular infiltration of lymphocytes and mast cells was evident in the superficial dermis. CD117 staining demonstrated perivascular infiltration of mast cells in the dermis. Diagnosis: telangiectasia macularis eruptiva perstans. Patient two was a 49-year-old man with red macules and angiotelectasis on the trunk and the arms for over 2 years. Dermatological examination revealed demarcated erythematous maculopapular rashes on the upper limbs, the chest and the back, with a negative Darier's sign. Histopathological findings included reticular keratosis, perivascular infiltration of mast cells in the superficial dermis, and proliferation of collagen fibers. Toluidine blue staining showed degranulation of perivascular mast cells. The diagnosis was telangiectasia macularis eruptiva perstans. Both patient were treated with oral ketotifen at a dose of 1 mg/evening. They claimed no significant improvement after one-month treatments.

Abstract:The T12811C mitochondrial DNA (mtDNA) mutation has been reported in Leber hereditary optic neuropathy (LHON) previously, with vision loss as the main manifestation. The involvement of other organ systems, including the central and peripheral nervous system, heart, and extraocular muscles, has not been well described. This case series report investigated four patients with T12811C mtDNA mutation, verified through a next generation sequencing. Two male patients presented with bilateral subacute visual decrease combined with involvement of multiple organ systems: leukoencephalopathy, hypertrophic cardiomyopathy, neurosensory deafness, spinal cord lesion and peripheral neuropathies. Two female patients presented with progressive ptosis and ophthalmoplegia, one of whom also manifested optic atrophy. This study found out that patients harboring T12811C mtDNA mutation manifested not only as vision loss, but also as a multi-system disorder affecting the nervous system, heart, and extraocular muscles.

Abstract:TEMPI syndrome (telangiectasias, elevated erythropoietin level and erythrocytosis, monoclonal gammopathy, perinephric fluid collections, and intrapulmonary shunting) is a newly defined multisystemic disease with its pathophysiology largely unknown. Here, we report the whole-genome sequencing (WGS) analysis on the tumor-normal paired cells from a patient with TEMPI syndrome. WGS revealed somatic nonsynonymous single-nucleotide variants, including SLC7A8, NRP2, and AQP7. Complex structural variants of chromosome 2 were found, particularly within regions where some putative oncogenes reside. Of potential clinical relevance, duplication of 22q11.23 was identified, and the expression of the located gene macrophage migration inhibitory factor (MIF) was significantly upregulated in 3 patients with TEMPI syndrome. Importantly, the level of serum MIF in one patient with TEMPI syndrome was significantly decreased in accordance with the downtrend of plasma cells, M-protein, hemoglobin, and erythropoietin and the improvement of telangiectasias, perinephric fluid collections, and intrapulmonary shunting after treatment with plasma cell–directed therapy. In conclusion, our study provides insights into the genomic landscape and suggests a role of MIF in the pathophysiology of TEMPI syndrome.

Hereditary Endotheliopathy with Retinopathy and Encephalopathy: Pathological and Genetic Studies of a Family.

Abstract:Background: The objective of this study was to examine the clinical, pathological and genetic features of a family suffering from hereditary endotheliopathy with retinopathy and encephalopathy. Methods: The index case was male, and his symptoms were detected at 18 years of age. The clinical manifestation included recurrent headache, fever, consciousness disturbances and haemiplegia. Bilateral cerebral hemispheric lesions were detected via MRI as low signals on T1 and high signals on T2 and FLAIR, with moderate enhancement. Video EEG revealed an increase in the slow wave frequency. An EMG displayed neurogenic atrophy. Similar clinical and imaging characteristics were detected in his mother and his uncle. Pathological examinations of the brain, muscle and sural nerve were performed on the index case. Sequence analysis of the TREX1 gene was performed on the index case, his sister and his father. Results: A brain biopsy revealed spongiform alterations as well as inflammatory cell infiltration in a few small vessels. Neurogenic muscular atrophy was detected based on a biopsy of the muscle. Demyelination was detected based on a biopsy of the sural nerve. Electron microscopic examination of the sural nerve revealed thickening and delamination of the basement membrane. No reported TREX1 gene mutation was detected for any of the patients. Conclusion: Hereditary endotheliopathy presented with peripheral nerve involvement. Multi-laminar thickening of the basement membrane of the capillaries also appeared in the extracerebral tissue. The involvement of a novel gene should be further examined.

Abstract:Objective To explore the clinical characteristics of hereditary hemorrhagic telangiectasis(HHT) in order to improve the diagnosis rate.Methods Retrospective analysis of one case of HHT suspected as dilated cardiomyopathy was made.Results The patient was admitted for convulsivum dyspnea,short breath for five years and aggravation for nine days.The patient was diagnosed as having heart failure in a local hospital,and was treated with diuretics and the symptom was temporarily relieved.The patient had a long history of spontaneous rhinorrhagia and anaemia,and manifested telangiectasis in skin and mucous membrane.The patient was diagnosed as having HHT combined with cardiac dysfunction concomitantly by the results of laboratory examination,ECG,UCG and CTA.She was discharged after proper treatment.Conclusion Clinicians should caution against HHT in patients with clinical manifestations of suspected dilated cardiomyopathy,accompanied with chronic hemorrhage,telangiectasis in skin and mucous membrane and internal organ injuries.
A SUNCT‐like headache associated with lateral pontine infarction – case series and systematic review

Abstract:Background and Aim Short‐lasting unilateral neuralgiform headache attacks with conjunctival injection and tearing (SUNCT) and autonomic symptoms (SUNA) are trigeminal autonomic cephalalgias (TACs). The study explores the potential association between SUNCT/SUNA‐like headaches and lateral pontine infarctions. Methods Case series and systematic review. Results We present three cases diagnosed with SUNCT following lateral pontine infarction on magnetic resonance imaging (MRI), along with a review of these cases and 10 others from the literature. Discussion and Conclusion This review suggests a connection between SUNCT/SUNA‐like symptoms and lateral pontine infarctions. The section also delves into the anatomy and pathophysiology of these symptoms, proposing a mechanism involving neural pathway remodelling in the lateral brainstem.

Poems Syndrome with Vascular Transformation of the Lymph Node Sinuses: A Case Report

Abstract:POEMS syndrome is a rare multisystem disorder associated with the clinical signs of polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy and skin changes. However, there is often a delay in the diagnosis due to a lack of overall consideration of the symptoms collectively. For this reason, POEMS syndrome is frequently mistaken for other diseases, such as chronic inflammatory demyelinating polyneuropathy. The present study reports the case of a 40-year-old female patient, who presented with a progressive lack of strength in the lower limbs and a unilateral cervical lump. The patient's enlarged cervical lymph nodes were mistaken for local hemangioma. However, subsequently POEMS syndrome with vascular transformation of the lymph node sinuses (VTS) was diagnosed. The patient received glucocorticoid treatment (20 mg prednisone acetate, daily), which is ongoing. The most recent follow-up examination revealed that the patient's strength had improved and at the time of writing the patient remained alive. The study discusses the clinical manifestations, auxiliary examinations and reason for the misdiagnosis. Hematoxylin and eosin staining and cluster of differentiation 31 immunostaining were adopted to identify the VTS. To the best of our knowledge, this is the first report of POEMS syndrome with VTS.
Seizure syndrome as a first manifestation of solitary tumor-like mass lesion of PACNS: Two case reports.

Abstract:RATIONALE:Primary angiitis of the central nervous system (PACNS) is an inflammatory disease involving cerebrovascular and parenchymal, and solitary tumor-like mass lesion of PACNS (TLML-PACNS) is frequently misdiagnosed as neoplastic or other inflammatory diseases. However, seizure syndrome as a first manifestation of TLML-PACNS has rarely reported before. PATIENT CONCERNS:Here, we report 2 cases of seizure syndrome, which was the first sign that presented prior to the diagnosis of TLML-PACNS by brain biopsy. DIAGNOSES:A mass lesion in the white and gray matters was detected by magnetic resonance imaging. The pathology for leptomeningeal lesion biopsy observed a transmural inflammation of the artery, with T lymphocyte infiltration. Patients were diagnosed with PACNS and epileptic seizure by biopsy and electroencephalogram. INTERVENTIONS:Patients were treated with glucocorticoid pulse therapy for 3 days, and subsequently oral prednisone was continued, in combination with immunosuppressant. OUTCOMES:Luckily, both two patients were improved after treatment, and only mild cognitive impairment remained without adverse event. LESSONS:Patient with mass lesion in CNS, which is similar to tumor, presented with seizure, headache, or cerebrovascular events without any other risk factors for stroke or tumor, should be considered the feasible with the disease of TLML-PACNS.
Moyamoya disease associated with tuberculum sellae meningioma and cavernous sinus hemangioma: case report.

Abstract:BACKGROUND: Primary moyamoya disease associated with skull base tumors has been reported only rarely in the literature. Surgical treatment can be complicated due to the compensatory collateral circulation through meningeal and leptomeningeal anastomosis. A standard frontotemporal craniotomy may interrupt critical transdural anastomoses. CASE DESCRIPTION: We report a case of primary moyamoya disease coexisting with tuberculum sellae meningioma and left cavernous sinus hemangioma. Simultaneous management of tuberculum sellae meningioma and moyamoya disease was performed using a left modified pterional incision. Two separate bone windows were opened to protect the transdural anastomosis via the middle meningeal artery. The tuberculum sellae meningioma was successfully removed through a small frontal craniotomy, and encephaloduromyosynangiosis was used to treat moyamoya disease through a temporoparietal craniotomy. Finally, CyberKnife radiotherapy was used to treat the left cavernous sinus hemangioma at 6 weeks after the initial operation. The patient recovered well without complications. This is the first report of moyamoya disease associated with tuberculum sellae meningioma and cavernous sinus hemangioma. CONCLUSIONS: With careful bone flap design, moyamoya disease and skull base tumors can be treated simultaneously. Care should be taken to avoid interruption of critical dural-pial collaterals and injury to fragile moyamoya vessels.
