Assessment of sub-clinical left ventricular dysfunction in asymptomatic heart transplantation patients with normal left ventricular ejection fraction by tri-plane imaging

Abstract:Objectives To evaluate whether heart transplantation patients with normal left ventricular ejection fraction(LVEF) have subclinical left ventricular systolic dysfunction. Methods Fifteen heart transplantation patients with normal echocardiography results were enrolled as Patient Group while twenty healthy subjects were enrolled as Control Group. Loops including apical four chamber view(A4C), apical long-axis view(LAX) and apical two chamber view(A2C) of the same cardiac cycle were acquired using GE E9 Ultrasound machine (4V-D transducer) and were then analyzed offline. Average longitudinal peak strain of A4C, LAX, A2C and left ventricle were measured. Time to peak strain of 18 segments was measured and their standard deviation (18-SD) was calculated.All the patients received blood routine exam within 12 hours before or after echocardiography. Results Longitudinal peak strains of the Patient Group were significantly smaller than those of the Control Group while its 18-SD was significantly larger. Moreover, in patients group, 18-SD showed positive relation to WBC accounts (r=0.54, P<0.05). Conclusions Tri-plane imaging can help us acquire loops of apical four chamber view, apical long-axis view and apical two chamber view of the same cardiac cycle, which is helpful in accurate synchronism assessment and the follow-up of heart transplantation patients. Heart transplantation patients have subclinical left ventricular systolic dysfunction and asynchrony, which may result from chronic rejection.
