Analysis of Current Situation of Chronic Disease Management Model in the Domestic and Overseas

Abstract:OBJECTIVE:To draw lessons from chronic disease management model in the overseas,and to promote chronic dis-ease management in China. METHODS:Retrieved from PubMed,CNKI,Wanfang database,etc.,the literatures about chronic dis-ease management model were summarized and analyzed. The improvement methods were put forward for domestic chronic disease man-agement. RESULTS&CONCLUSIONS:The chronic disease model in the overseas mainly included chronic care model,chronic dis-ease self-management model,transitional care model and British chronic disease management system model,etc. In China,chronic dis-ease management model mainly included chronic disease information monitoring system model,chronic disease self-management mod-el,community chronic disease health management and community chronic disease clinical pathway management model,etc. Referring oversea experience,Chronic disease management make certain progress and still sheald be improved from aspects of clinical informa-tion system,health system,patient self-management,community resource and policy support,etc.
