Relationship between hemodynamic and pulmonary function in chronic obstructive lung disease with ;pulmonary artery hypertension

Abstract:Objective To investigate the relationship between hemodynamic and pulmonary function in chronic obstructive pulmonary disease (COPD ) with pulmonary hypertension (PH ). Methods Totally 64 COPD with PH patients (hemodynamic criteria confirmed by right heart catheterization)were divided into two groups according to mean pulmonary artery pressure (mPAP)and cardiac index (CI).The pulmonary function of two group patients,36 severe COPD-PH and 28 non severe COPD-PH,were compared using the independent samples t-test.Pearson correlation coefficients were used to assess the relationship between hemodynamic and pulmonary function.Results In COPD-PH, forced vital capacity% pred [(55.03 ± 22.2 )%],FEV1 % pred [(31.4 ± 13.1 )%]were declined.The residual volume [(184.7 ± 63.4)%]and total airway resistance [(450.9 ± 296.8)%]were increased with decreased DL CO [(58.5 ± 31.1 )%].There was no difference between ventilatory function and diffusive function in severe or non-severe COPD-PH.Severe COPD-PH had more severe hypoxemia.PaO 2&nbsp;and SaO 2 tended to be lower (-10.6 mmHg and 6.3%,P < 0.01 )in severe COPD-PH,and were negatively related to mPAP in COPD-PH.Conclusions Severe hypoxemia was more obvious in severe COPD-PH,which might be correlated with pulmonary vasculopathy besides ventilatory function and diffusive function.
