Abstract:Objective To evaluate the effect of therapeutic hypercapnia on cerebral oxygen metabo?lism in the patients undergoing thoracoscopic surgery in beach chair position ( BCP ) . Methods Sixty pa?tients of both sexes, aged 18-32 yr, with body mass index of 19-24 kg∕m2 , of American Society of Anes?thesiologists physical statusⅠorⅡ, scheduled for elective bilateral thoracic sympathectomy performed via a thoracoscope, were divided into control group ( group C ) and hypercapnia group ( group H ) , with 30 patients in each group using a random number table. After induction of anesthesia, all the patients in both groups were tracheally intubated and mechanically ventilated using the ventilation regimen low tidal vol?ume intermittent positive pressure ventilation combined with low level of positive end?expiratory pressure ( 5 cmH2 O) , maintaining arterial carbon dioxide partial pressure ( PaCO2 ) at 35-45 mmHg. PaCO2 was maintained at 45-55 mmHg by adjusting the respiratory rate after the patients were placed in BCP in group H. Anesthesia was maintained with target?controlled infusion of propofol and intermittent intravenous boluses of rocuronium and sufentanil. Bispectral index value was maintained at 45-55. Before anesthesia induction ( baseline) , at 5 min after intubation, and at 5, 10, 15 and 20 min after the patients were placed in BCP, blood samples were taken from the radial artery and jugular bulb for blood gas analysis, jugular ve?nous bulb oxygen saturation was measured, and arteriovenous blood O2 content difference, cerebral O2 ex?traction rate, and venous to arterial blood lactate concentration difference were calculated. Results Com?pared with group C, PaCO2 and jugular venous bulb oxygen saturation were significantly increased, and ar?teriovenous blood O2 content difference and cerebral O2 extraction rate were significantly decreased at at 5, 10, 15 and 20 min after the patients were placed in BCP in group H ( P<0?05) , and there was no signifi?cant change in venous to arterial blood lactate concentration difference at each time point between the two groups ( P>0?05) . Conclusion Therapeutic hypercapnia can improve the cerebral oxygen metabolism in the patients undergoing thoracoscopic surgery in BCP .

Abstract:Patients with obstructive sleep apnoea (OSA), male sex, obesity, older age or hypertension are prone to hypoxemia during flexible bronchoscopy. This study investigated whether using a high-flow nasal cannula (HFNC) could reduce the incidence of oxygen desaturation during bronchoscopy under deep sedation in patients at risk of hypoxemia. A total of 176 patients at risk of hypoxemia who underwent flexible bronchoscopy under deep sedation were randomly assigned to two groups: the HFNC group (humidified oxygen was supplied via a high-flow nasal cannula at a rate of 60 L/min and a concentration of 100 www.chiCTR.org.cn Identifier: ChiCTR2100044105. Abstract:Background The effectiveness of high-flow nasal cannula (HFNC) therapy in patients with bronchiectasis experiencing hypercapnia remains unclear. Our aim was to retrospectively analyze the short-term outcomes of HFNC therapy in such patients, and to further explore the predictors of HFNC treatment failure in this particular patient population. Methods A retrospective review was conducted on patients with bronchiectasis who received HFNC ( n = 70) for hypercapnia (arterial partial pressure of carbon dioxide, PaCO 2 ≥ 45 mmHg) between September 2019 and September 2023. Results In the study population, 30% of patients presented with acidemia (arterial pH < 7.35) at baseline. Within 24 h of HFNC treatment, there was a significant reduction in PaCO 2 levels by a mean of 4.0 ± 12.7 mmHg (95% CI -7.0 to -1.0 mmHg). Concurrently, arterial pH showed a statistically significant increase with a mean change of 0.03 ± 0.06 (95% CI 0.01 to 0.04). The overall hospital mortality rate in our study was 17.5%. The median length of hospital stay was 11.0 days (interquartile range [IQR] 8.0 to 16.0 days). Sub-analysis revealed no statistically significant differences in hospital mortality (19.0% vs. 20.4%, p = 0.896), length of hospital stay (median 14.0 days [IQR 9.0 to 18.0 days] vs. 10.0 days [IQR 7.0 to 16.0 days], p = 0.117) and duration of HFNC application (median 5.0 days [IQR 2.0 to 8.5 days] vs. 6.0 days [IQR 4.9 to 9.5 days], p = 0.076) between the acidemia group and the non-acidemia group (arterial pH ≥ 7.35). However, more patients in the non-acidemia group had do-not-intubate orders. The overall treatment failure rate for HFNC was 28.6%. Logistic regression analysis identified the APACHE II score (OR 1.24 per point) as the independent predictor of HFNC failure. Conclusions In patients with bronchiectasis and hypercapnia, HFNC as an initial respiratory support can effectively reduce PaCO 2 level within 24 h of treatment. A high APACHE II score has emerged as a prognostic indicator for HFNC treatment failure. These observations highlight randomized controlled trials to meticulously evaluate the efficacy of HFNC in this specific population.

Abstract:The authors seek to find new connections between recent results of biology and older theories. This paper aims to assemble the jigsaw puzzle. The theoretical background of the hypothesis was described in the previous issue of the journal (Sikter et al. 2017a). Human stress response often coexists with persistent hypocapnia or hypercapnia - developing via psychosomatic pathomechanism - which can lead to mental and psychosomatic illnesses. Chronic hypocapnia mainly generates hyperarousal disorders which may be reversible for an extended time, however, vicious cycles may start when hypoxia and/or severe somatic diseases are simultaneously present (commonly in the elderly), which conditions often end with death without medical help. Chronic hypercapnia devastates the organism initially without symptoms, partly due to neurohumoral contraregulation, consequential dysregulation and metabolic remodeling. Psychosomatic disorders (e.g., diseases of civilization that evolve in people with disadvantaged psychosocial situations) develop over years and decades, causing irreversible changes. Hypercapnia usually occurs in clinical pictures of chronic obstructive pulmonary disease, obesity hypoventilation syndrome, obstructive sleep apnea, and its unobstructed version (sleep-related hypoventilation), generating various organic disorders (hypertension, type 2 diabetes, cardiovascular disorders, immunological diseases, depression, etc.). Because of the above, chronic hypocapnia and hypercapnia cannot be regarded as harmless accompanying phenomena. That is why we have to strive for restoring eucapnia and normalizing the induced ionic changes, which does not appear to be a hopeless task.

Abstract:SESSION TITLE: Sleep Disorders II SESSION TYPE: Original Investigation Poster PRESENTED ON: Saturday, April 16, 2016 at 11:45 AM - 12:45 PM PURPOSE: Obstructive sleep apnea hypopnea syndrome (OSAHS) is a chronic sleep respiratory disease with nocturnal hypoxemia and continuous hypercapnia. The key determinants of hypercapnia in OSAHS are unclear. METHODS: 70 OSAHS patients were measured by overnight polysomnography (PSG), Epworth Sleepiness Scale (ESS), carbon dioxide monitoring in end-tidal (PETCO2Q) and prolonged expiration (more than 5 seconds) (PETCO2P) at bedtime and in the morning. These patients were divided equally into 4 groups according to PSG results: control group with an apnea-hypopnea index (AHI) <5 (n=17), mild group (5 ≤ AHI <15) (n=17), moderate group (15 ≤ AHI <30) (n=18) and severe group (AHI ≥30) (n=18). RESULTS: There were no significant differences between PETCO2Q and PETCO2P at bedtime and in the morning respectively. But PETCO2P levels were higher than PETCO2Q. The difference of PETCO2P and PETCO2Q (△PETCO2) had no relationships with AHI, lowest oxygen saturation (LSaO2), sex, age and smoke. But △PETCO2 had positive relationships with the percent of SaO2<90% in one night sleep (SIT90), Body Mass Index (BMI), Neck circumference, waist circumference and ESS (r=0.625, r=0.429, r=0.424, r=0.335, r=0.387, p<0.05). CONCLUSIONS: Hypercapnia occurs in a large part of patients with OSAHS. Nocturnal hypoxemia and obesity are all contributed to the development of daytime CO2 retention in OSAHS. CLINICAL IMPLICATIONS: Nocturnal hypoxemia and obesity are all contributed to the development of daytime CO2 retention in OSAHS. Hypercapnic Respiratory Acidosis During An In-Flight Oxygen Assessment

Abstract:Background: Patients with respiratory disease are at risk of excessive hypoxemia in the hypobaric commercial aircraft cabin environment, and the consensus is that this is easily corrected with supplementary oxygen. However, despite the risks of hypercapnia with increasing inspired oxygen in some patients being well established, this issue is not currently addressed in medical guidelines for air travel. Case report: A 76-yr-old woman with chronic type 2 respiratory failure underwent hypoxic challenge testing (HCT) to assess in-flight oxygen requirements. She is stable on home ventilation, and baseline arterial blood gases showed mild hypoxemia (Pao2 9.12 kPa), normal P(a)co(2) (5.64 kPa) and pH (7.36) with 98% S(p)O(2). HCT was performed delivering 15% FIo(2) via a mask, and the patient desaturated to < 85%. HCT blood gases revealed significant hypoxemia (P(a)o(2) < 6.6 kPa), indicating in-flight oxygen. Continuous oxygen at 2 L · min⁻¹ via nasal cannula corrected the hypoxia, although P(a)co(2) increased to 6.9 kPa with reduction in pH to the threshold of severe respiratory acidosis (pH 7.25). The patient was advised against flying due to hypoxemia during HCT and the precipitous drop in pH on oxygen. Discussion: It is possible to hyperoxygenate patients with type 2 respiratory failure in flight with the minimum level of supplementary oxygen available on many aircraft. In these cases P(a)co(2) and pH should be scrutinized during HCT before recommending in-flight oxygen. No current guidelines discuss the risk of hypercapnia from in-flight oxygen; it is therefore recommended that this be addressed in future revisions of medical air travel guidelines, should further research indicate it.
Is Hypercapnia Associated with Poor Prognosis in Chronic Obstructive Pulmonary Disease? A Long-Term Follow-Up Cohort Study.

Abstract:OBJECTIVES:To assess whether hypercapnia may predict the prognosis in chronic obstructive pulmonary disease (COPD).DESIGN:Prospective cohort study comparing the survival of patients with COPD and normocapnia to those with chronic hypercapnia.SETTING:Patients with consecutive COPD were enrolled between 1 May 1993 and 31 October 2006 at two medical centres. Follow-up was censored on 31 October 2011.PARTICIPANTS:A total of 275 patients with stable COPD and aged 40-85 years were enrolled. Diagnosis of hypercapnia was confirmed by blood gas analysis. Patients with near-terminal illness or comorbidities that affect PaCO2 (obstructive sleep apnoea, obesity-related hypoventilation, or neuromuscular disease) were excluded. The outcome of 98 patients with normocapnia and 177 with chronic hypercapnia was analysed.OUTCOME MEASURES:Overall survival.RESULTS:Median survival was longer in patients with normocapnia than in those with hypercapnia (6.5 vs 5.0 years, p=0.016). Multivariate COX regression analysis indicated that age (HR=1.043, 95% CI 1.012 to 1.076), Charlson Index, which is a measure of comorbidity (HR=1.172, 95% CI 1.067 to 1.288), use of medication (HR=0.565, 95% CI 0.379 to 0.842), body mass index (BMI) (HR=0.922, 95% CI 0.883 to 0.963), PaCO2 (HR=1.026, 95% CI 1.011 to 1.042), Cor pulmonale (HR=2.164, 95% CI 1.557 to 3.006), non-invasive positive-pressure ventilation (NPPV) (HR=0.615, 95% CI 0.429 to 0.881) and per cent of forced expiratory volume in 1 s (FEV1%) (HR=0.979, 95% CI 0.967 to 0.991), were independent risk factors for mortality.CONCLUSIONS:Increased age, Charlson Index, chronic hypercapnia and Cor pulmonale, and decreased FEV1%, use of medication, BMI and NPPV, were associated with a poor prognosis in patients with COPD.
Influence of hypercapnia and hypercapnic hypoxia on the heart rate response to apnea

Abstract:We aimed to determine the relative contribution of hypercapnia and hypoxia to the bradycardic response to apneas. We hypothesized that apneas with hypercapnia would cause greater bradycardia than normoxia, similar to the response seen with hypoxia, and that apneas with hypercapnic hypoxia would induce greater bradycardia than hypoxia or hypercapnia alone. Twenty-six healthy participants (12 females; 23 ± 2 years; BMI 24 ± 3 kg/m2) underwent three gas challenges: hypercapnia (+5 torr end tidal partial pressure of CO2 [PETCO2]), hypoxia (50 torr end tidal partial pressure of O2 [PETO2]), and hypercapnic hypoxia (combined hypercapnia and hypoxia), with each condition interspersed with normocapnic normoxia. Heart rate and rhythm, blood pressure, PETCO2, PETO2, and oxygen saturation were measured continuously. Hypercapnic hypoxic apneas induced larger bradycardia (-19 ± 16 bpm) than normocapnic normoxic apneas (-11 ± 15 bpm; p = 0.002), but had a comparable response to hypoxic (-19 ± 15 bpm; p = 0.999) and hypercapnic apneas (-14 ± 14 bpm; p = 0.059). Hypercapnic apneas were not different from normocapnic normoxic apneas (p = 0.134). After removal of the normocapnic normoxic heart rate response, the change in heart rate during hypercapnic hypoxia (-11 ± 16 bpm) was similar to the summed change during hypercapnia+hypoxia (-9 ± 10 bpm; p = 0.485). Only hypoxia contributed to this bradycardic response. Under apneic conditions, the cardiac response is driven by hypoxia.

Abstract:Objective: To evaluate the incidence and factors related to daytime hypercapnia in Han and Uygur patients with obstructive sleep apnea hypopnea syndrome (OSAHS). Methods: There were 221 patients with OSAHS (include 179 Han patients and 42 Uygur patients) in Sleep Center of Department of Respiratory and Critical Care Medicine of Karamay Central Hospital from 2015, Jan to Dec. All the patients underwent polysomnography (PSG), nocturnal oximetry, daytime blood gas analysis, pulmonary function test and Mouth occlusion pressure (P0.1) results were recorded. The features of hypercapnia was analyzed for patients with OSAHS, and linear regression analysis was used to evaluate the arterial carbon dioxide partial pressure (PaCO2) levels and related factors. Results: Daytime hypercapnia occurred in 16.7% (37/221) of the 221 patients with OSAHS. Compare with no hypercapnia groups, the body mass [(31.6±5.6) vs (27.9±1.7) kg/m2], sleep apnea index (AHI) [(40.9±26.3) vs (32.2±20.1) times/h], the percentage of time spent at oxygen saturation below 90 (SIT90) [(38.6±31.9)% vs (23.9±23.6)%], P0.1 [(3.08±2.86) vs (2.03±1.20) mmHg, 1 mmHg=0.133 kPa] were higher in hypercapnia groups, but the mean nocturnal arterial oxygen saturation (MSaO2) [(86.0±15.5)% vs (92.0±3.0)%], the nadir arterial oxygen saturation (LSaO2) [(68.9±13.0)% vs (75.3±9.9)%] and arterial partial pressure of oxygen (PaO2) [(74.5±23.0) vs (86.1±14.8) were lower in hypercapnia groups (all P<0.05). Compare with Han patients with OSAHS, MSaO2 and LSaO2 was lower, PaCO2 and P0.1 was higher in Uygur patients (all P<0.05). Conclusions: Uygur OSAHS patients with hypercapnia have a higher daytime PaCO2 than the Han counterparts. BMI, AHI, MSaO2, P0.1 level are all related with daytime hypercapnia in OSAHS.

Abstract:Abstract Background ARDS, a severe complication of sepsis, often exhibits hypercapnia, whose immunological mechanisms remain elusive. Our study aimed to investigate the pathogenesis of hypercapnia in pulmonary sepsis-associated ARDS and identify potential therapeutic targets. Methods Between December 2022 and April 2023, we conducted a single-center, prospective study with sixty-one severe pneumonia patients. Eleven patients without sepsis served as controls based on Sepsis-3 criteria. The remaining fifty patients with pulmonary sepsis met ESICM criteria for ARDS, with twenty-six developing hypercapnia. We comprehensively collected clinical data, respiratory parameters, and serial specimens of peripheral blood mononuclear cells (PBMCs) and bronchoalveolar lavage fluid (BALF) at early (within 24 hours) and late (Day 7) time points post-intubation. These specimens were analyzed using flow cytometry and a cytokine panel. Additionally, we performed single-cell RNA sequencing (scRNA-seq) on blood samples from four ARDS patients to characterize immunological and cellular profiles following pneumonia sepsis. Results Compared with non-hypercapnia ARDS patients and non-sepsis controls, septic associated-ARDS patients with hypercapnia exhibit poor prognosis and high mortality. To investigate their immunological response, we conducted scRNA-seq on PBMCs from four ARDS patients (one without hypercapnia and three with hypercapnia). Results showed a significant increase in monocytes, particularly classical monocytes (CD14+CD16−, CM), in hypercapnia patients. Flow cytometry analysis revealed a similar increase in CM throughout sepsis stages. This underscores the role of monocytes in hypercapnia ARDS. ScRNA-seq analysis also revealed elevated expression of cytokine storm-related genes, including IL-1β signaling, and genes linked to myeloid cell activation and recruitment. Elevated levels of IL-1β, IL-12p40, and IL-23 were observed in both blood and alveolar lavage fluid. Additionally, hypercapnia patients exhibited enrichment of profibrotic genes in monocytes and accumulation of monocyte-derived macrophages in airways, supported by clinical parameters, specifically the increased driving pressure and decreased static respiratory lung compliance, indicating pulmonary fibrosis. Conclusions We offer novel perspectives on monocyte-centered clusters and associated biomarkers, which play a pivotal role in driving hypercapnia-induced lung fibrosis. Our study provides fresh insights into the immunological mechanisms underlying hypercapnia in pulmonary sepsis-associated ARDS, laying the foundation for future therapeutic interventions to improve patient outcomes.

Targeting Hypercapnia in Chronic Lung Disease and Obesity Hypoventilation: Benefits and Challenges

Abstract:Hypoventilation is a complication that is not uncommon in chronic obstructive pulmonary disease and calls for both medical treatment of the underlying disease and, frequently, noninvasive ventilation either during exacerbations requiring hospitalization or in a chronic state in the patient at home. Obesity hypoventilation syndrome by definition is associated with ventilatory failure and hypercapnia. It may or may not be accompanied by obstructive sleep apnea, which when detected becomes an additional target for positive airway pressure treatment. Intensive research has not completely resolved the best choice of treatment, and the simplest modality, continuous positive airway pressure, may still be entertained.

Abstract:In advanced chronic obstructive pulmonary disease (COPD), hypercapnia may occur due to severe bronchial obstruction with lung hyperinflation. Non-invasive ventilation (NIV) provides the standard of care intended to achieve physiological PCO 2 levels, thereby reducing overall mortality. The present study aimed to evaluate pulmonary function parameters derived from spirometry (forced vital capacity [FVC], forced expiratory volume in 1 s [FEV1]), body plethysmography (residual volume [RV], total lung capacity [TLC]), and lung diffusion capacity for carbon monoxide (single-breath method [DCO-SB], alveolar-volume corrected values [DCO-VA]) as predictors of chronic hypercapnia in patients with advanced COPD.

A Correlative Study of OSAHS and Daytime Hypercapnia

Abstract:Objective A comparative study of the prevalence of patients in obstructive sleep apnea hypopnea syndrome (OSAHS) and to understand of this disease. Methods For constantly collected 110 OSAHS patients with normal lung functions, a comparison on the 29 patients with daytime hypercapnia (the treatment group ) and the 81 without daytime hypercapnia (the control group )were conducted, to investigate their difference in age, body mass index (BMI), pulmonary function, daytime bloods gas, nocturnal arterial oxygen saturation (SaO2) and polysomnography (PSG). The statistical methods of correlation and regression analysis were used on PaCO2 related factors. Results There was no significant difference in the FEVi/FVC (P>0. 05). The subjects of the treatment group were younger than those of the control group. In the treatment group, BMI and AHI were higher (P< 0. 05), daytime PaO2 was lower (P<0.001), daytime and [HCO3] were higher (P<0.001), nocturnal mean of SaO2 (MSaO2) and the lowest SaO2 (LSaO2) were significantly lower, and SIT90 was significantly higher ( P<0.05). In the treatment group, an inverse correlation between daytime PaCO2 and MSaO2, LSaO2 was observed (r=-0. 625;-0. 429), a positive correlation between daytime PaCO2 and SIT90, AHI were found (r=0. 563; 0. 385), and the mean SaO2 accounted for 39. 1%of the total variance in daytime PaCO2 levels (R2 =0. 391 P=0. 001) using stepwise multiple regression analysis. Conclusion The prevalence of the patients in OSAHS with daytime hypercapnia was 26.4%. The young or middle-aged overweight patients were easier to suffer from OSAHS with daytime hy-percapnia. The daytime hypercapnia was associated with nocturnal hypoxia. MsaO2 was one of the important factors that conduce to OSAHS with daytime hypercapnia.
Differences in Anthropometric, Sleep and Respiratory Characteristics between Hypercapnic and Normocapnic Patients with COPD-OSA Overlap Syndrome

Abstract:Background: Overlap syndrome (OS), the coexistence of chronic obstructive pulmonary disease and obstructive sleep apnea, is frequently characterized by the presence of daytime hypercapnia (pCO2 ≥ 45 mmHg). The aim of this study was to investigate potential differences in anthropometric, sleep and respiratory characteristics between hypercapnic and normocapnic patients with OS. Methods: Consecutive patients who underwent polysomnography, pulmonary function testing and arterial blood gases and had been diagnosed with OS were enrolled in the study. Results: According to pCO2 levels in wakefulness, the patients were divided into group A, consisting of OS patients without hypercapnia (n = 108) or group B, consisting of OS patients with hypercapnia (n = 55). The majority of included patients in both groups were males (n = 92 in group A vs. n = 50 in group B). Group B had increased BMI (p = 0.001), neck (p = 0.017) and waist circumference (p = 0.013), higher scores in Epworth sleepiness scale (ESS) (p = 0.008), increased sleep efficiency (p = 0.033), oxygen desaturation index (p = 0.004) and time with oxyhemoglobin saturation <90% (p = 0.006) than group A. Also, Group B had decreased average and minimum oxyhemoglobin saturation during sleep (p < 0.001). Hypercapnic patients had lower FEV1% (p = 0.003), FVC% (p = 0.004), pO2 and pCO2 (p < 0.001 for both) values compared with normocapnic patients. In binary regression analysis, which assessed various predictors on the likelihood of having hypercapnia, it was found that BMI (OR: 1.313, 95% CI: 1.048–1.646, p = 0.018) and FVC (OR: 0.913, 95% CI: 0.845–0.986, p = 0.020) were the major determinants of hypercapnia in OS patients. Conclusions: Hypercapnic OS patients were more obese and sleepy and presented worse respiratory function in wakefulness and sleep hypoxia characteristics compared with normocapnic OS patients.

Abstract:Abstract Background One-lung ventilation (OLV) can disrupt the respiratory cycle and lead to changes in oxygenation and local cerebral oxygen saturation (rSO2), which may predispose elderly patients to an imbalance in the ventilation/perfusion ratio and an increased risk of hypoxemia. Permissive hypercapnia has been gradually recommended as a lung-protective mechanical ventilation strategy in recent years, in theory could enhance cerebral blood perfusion and oxygen supply. However, the actual clinical benefits of this physiological response remain uncertain. Our study hypothesizes that permissive hypercapnia during OLV may have a beneficial effect on the cerebral oxygen supply and demand balance in elderly patients with a history of cerebrovascular disease undergoing thoracoscopic lung surgery. Methods This is a prospective, single-center, randomized controlled, double-blind clinical study. 270 participants(≥65years) scheduled for thoracoscopic surgery will be included in this study. Participants in the treatment group will use protective lung ventilation strategy during operation, maintain end-tidal partial pressure of carbon dioxide at 50-65mmHg; Participants in the placebo group will not use protective lung ventilation strategy during operation, maintain end-tidal partial pressure of carbon dioxide at 35-45mmHg. Discussion The primary hypothesis of the forthcoming study is that permissive hypercapnia during OLV might have a beneficial effect on the cerebral oxygen supply-demand balance in elderly patients with a history of cerebrovascular disease who are undergoing thoracoscopic lung surgery, in term of reducing the incidence of event rate of cerebral desaturation. Effect of Acute Hypoxia and Hypercapnia on Arterial Stiffness and Blood Pressure in Patients with Obstructive Sleep Apnea Hypopnea Syndrome

Abstract:OBJECTIVE To investigate the effect of acute hypoxia and/or hypercapnia on cardio-ankle vascular index (CAVI) and blood pressure (BP) in patients with obstructive sleep apnea hypopnea syndrome (OSAHS). METHOD CAVI and blood pressure were measured before and after isocapnic hypoxic, hyperoxia hypercapnic, and hypoxic and hypercapnic challenge in 28 non-hypertensive patients with OSAHS (AHI > 10/h) and 26 healthy controls (AHI < 5/h), respectively. They were matched for age and sex. Hypoxia and hypercapnia were induced by re-breathing technique. RESULTS The 2 groups had no differences in regard to systolic (SBP) and diastolic BP(DBP) and CAVI. After hypercapnic challenge, SBP increased significantly in both groups. CAVI decreased significantly in controls, but not in OSAHS. Hypoxia induced significant increase of CAVI, but not in OSAHS. SBP and DBP maintained to the pre-challenge levels in both group. Hypercapnia and hypoxia together caused increase of SBP in both groups, and CAVI increased significantly in controls, but not in OSAHS. CONCLUSIONS Acute hypoxia and hypercapnia exposure caused change of arterial stiffness and BP in both control and patients with sleep apnea hypopnea syndrome. However, CAVI responses to hypoxic and/or hypercapnic challenge were blunted in patients with OSAHS.
