Abstract:Objective: To assess the resection, reconstruction, complication and treatment outcomes of the pelvic chondrosarcoma. Methods: The data of 21 patients of pelvic chondrosarcoma were retrospectively reviewed. There were 12 females and 9 males with the median age of 46 years (range, 16-65 years). According to Enneking pelvic classification system, 2 patients were classified as type I, 5 as type I and II, 6 as type II, 7 as type II and III, 1 as type III. The pathological details were: 12 cases of well-differentiated chondrosarcoma, differentiated chondrosarcoma in 5 cases, 2 cases of poorly differentiated chondrosarcoma, mesenchymal chondrosarcoma in 2 patients. Allograft was used in 2 patients after inactivation of tumor curettage. One patient underwent skeletal reconstruction of rod-screw system and allograft was used for pelvic repair in 1 patient after iliac local resection. One patient used autograft and internal fixation for reconstruction after pubis resection. Two patients underwent hemipelvectomy and 5 patients underwent internal hemipelvectomy. Acetabular reconstructions were performed in 9 cases. Results: The average duration of follow-up was 39 months (range, 7-100 months). Seventeen cases survived freely, 2 survived with tumor, and 2 died. Local recurrence was observed in 5 of the 16 cases treated with widely excision, and it was also observed in 3 of the 5 cases treated with intralesional excision and marginal excision. The local recurrence rate and mortality rate in 21 patients with chondrosarcomas were 38.1% and 9.5%, respectively. The average ISOLS function score for the 19 patients was 19 (range, 7-25). The controllable complications occurred in 8 cases. Conclusions: The best method for the pelvic chondrosarcoma treatment is wide excision. Prevention and active treatment of complications is the key to success of surgical treatment.

Abstract:OBJECTIVES: To investigate the diagnosis and treatment of tibial intercondylar chondroblastoma. METHODS: We retrospectively analyzed the diagnosis and treatment of 12 patients with tibial intercondylar chondroblastoma admitted to the orthopedics department from May 2011 to February 2016; among them were 4 males and 3 females aged 10-19 years, with an average age of 15.7 years. Tibial intercondylar chondroblastoma was on the left and right side in 7 and 5 cases, respectively. The preoperative average Lysholm score of the knee joint was 68 (42-87). A posteromedial approach was applied in all cases. The incisions were approximately 5-8 cm in length. Complete curettage and inactivation were performed after fenestration, and allogeneic bone grafts were transplanted. Then, the posterior cruciate ligament insertion was fixed with 5.0 suture anchors. All patients were followed up with regularly to monitor for tumor recurrence, observe bone graft healing, and reassess the Lysholm score of the knee. RESULTS: Patients were followed for 7-55 months, and the median follow-up time was 19 months. One patient experienced tumor relapse 4 months after the operation. Incision, inactivation and cementation were performed. Then, the bone was fixed with anchors. In the other 11 patients, the bone graft healed over an average period of 6.2 months (4-10 months), with good functional recovery postoperatively. The average postoperative Lysholm score of the knee was 91 (81-95). CONCLUSION: Tibial intercondylar chondroblastoma has unique clinical and imaging characteristics and can effectively be treated by curettage followed by the inactivation, transplantation and fixation of allogeneic bone grafts with suture anchors through a posteromedial approach.
Multivariate Analysis of Chondrosarcoma Prognosis

Abstract:Objective To screen possible factors affecting prognosis of chondrosarcoma.Methods A total of 37 patients with chondrosarcoma,who had undergone surgical treatment from December 2005 to March 2008 in our hospital and had complete follow-up data and definitive pathological diagnosis,were enrolled in this retrospective study.There were 16 males and 21 females,ageing from eleven to eighty-four years (average,42.8 years).The univariate analysis of survival rate was performed with Kaplan-Meier method and tested with the Log-rank test.Chi-squared test or Fisher's exact test were used to analyze numeration data,and then the significant indexes after univariate analysis were performed multivariate analysis with COX regression model to screen the independent factors affecting prognosis.On the basis of literatures,8factors including gender,age,duration of disease,tumor site,Enneking stage,surgical approach,distant metastasis and local recurrence were analyzed.Results Statistical significance was found in Enneking stage,surgical approach and distant metastasis,which indicated that they were related to survival rate 3years after surgery.However,gender,age,duration of disease,tumor site,and local recurrence had no significant correlation with prognosis.After multivariate analysis of Enneking stage,surgical approach,distant metastasis,the results showed that Enneking stage and surgical procedure were the independent prognostic factors,while distant metastasis was not an independent prognostic factor.Conclusion Enneking stage and surgical approach are the independent prognostic factors for chondrosarcoma,which can be used to evaluate prognosis of chondrosarcoma.

Abstract:Objective: The current study aimedto screen for possible factors which affect prognosis of chondrosarcoma. Methods: Thirty seven cases were selected and analyzed statistically. The patients received surgical treatment at our hospital between December 2005 and March 2008. All of them had complete follow-up data. The survival rates were calculated by univariate analysis using the Kaplan-Meier method and tested by Log-rank. chi 2 or Fisher exact tests were carried out for the numeration data. The significant indexes after univariate analysis were then analyzed by multivariate analysis using COX regression model. Based on the literature, factors of gender, age, disease course, tumor location, Enneking grades, surgical approaches, distant metastasis and local recurrence were examined. Results: Univariate analysis showed that there were significant differences in Enneking grades, surgical approaches and distant metastasis related to the patients' 3-year survival rate after surgery (P<0.001). No significant difference was not found in gender, age, disease course, tumor location or local recurrence (P>0.05). Multivariate analysis showed that Enneking grade (P =0.007) and surgical approaches (P = 0.010) were independent factors affecting the prognosis of chondrosarcoma, but distant metastasis was not (P = 0.942). Conclusion: Enneking grades, surgical approaches and distant metastasis are risk factors for prognosis of chondrosarcoma, among which the former two are independent factors.

Abstract:Primary chondroid tumors of the larynx represent less than 1% of all laryngeal tumors. Most of them are chondromas and they often involve to the cricoid cartilage. They are characterized by a low tendency to metastatic diffusion (low grade). The treatment of choice is surgery, which may be endoscopic or "open partial surgery", if extension of the cancer is limited. Prognosis is generally good. In this report, two cases of low grade chondrosarcoma of the larynx are presented, one was treated surgically with cricoidectomy and partial laryngectomy, and another was treated surgically with hemicricoidectomy. Laryngoscopy reveals tumefaction of the larynx, covered by intact mucosa. Computerized tomography imaging with contrast and magnetic resonance imaging defines not only coarse calcifications, pathognomonic of chondromatous neoformations but also the relationship of the neoformation with the surrounding tissues. Treatment is essentially surgical, given the importance of preserving the larynx to patients' quality of life, the only risk is recurrence, which is treated by a second surgery.
Chondroblastoma in the long bone diaphysis: a report of two cases with literature review.

Abstract:To investigate the clinical characteristics of chondroblastoma with an emphasis on lesions located in the long bone diaphysis, we reviewed the clinical data of 7 patients with histologically proven chondroblastoma treated in Tianjin Medical University Cancer Hospital and Fudan University Cancer Hospital between January 1995 and May 2009. There were two rare cases of chondroblastoma in the long bone diaphysis. One patient with a lesion in the tibial diaphysis underwent intralesional curettage and bone grafting, and the postoperative bone function was measured as excellent according to the Enneking scoring system. The patient was still alive upon follow-up at 60 months. The other patient with a lesion in the humeral diaphysis underwent resection, and the postoperative bone function was excellent at 48 months, at which there was no evidence of recurrence or metastasis. Thus, except for the distinctive site of the long bone diaphysis, which made diagnosis difficult, the patients' ages, symptoms, X-ray and CT images, treatment, and prognosis were in accordance with typical lesions in the epiphysis and metaphysis. The diagnosis of chondroblastoma in the long bone diaphysis significantly depends on histopathologic characteristics.
Chondroblastoma of bone:a clinicopathological analysis of 67 cases

Abstract:Objective To analyze the morphological characteristics,image features,treatments and diagnosis of chondroblastoma.Methods The pathological features were explored with HE and immunohistochemical staining in 67 cases of chondroblastoma.The image manifestations were studied with plain X-ray film,CT and MRI.Results It occured predominantly in adolescents(mean age,19 years),more commonly in males(male/female=3.2:1.0).Preferred sites were the epiphyses or apophyses of the long bones,especially in the femoral great trochanters(22/67,32.84%).In this group,lesions were located at femurs in 37 patients(55.22%);tibia in 12(17.91%);talus and pelvis each in 3;humeri,costae,vertebrae,calcaneus and patella each in 2 and sacrum and infraturbinal each in 1.On plain radiograph,it depicted as an ovoid lesion with well defined osteolytic destruction and thin sclerotic margin.CT was optimal to detect the tumors that appeared as low density area of bone with patchy and stippled calcification,periosteal reaction,soft tissue swelling and cystic changes.MRI imaging demonstrated iso-and hypo-intense on T1WI and heterogeneous signals on T2WI.Histologically,one of the most characteristic findings in chondroblastomas was so called "chicken-wire-like calcification" that assuming a grid pattern of tiny calcific bodies which enveloped the cells.However,it was only 31 cases(46.27%) in this group,compared with polygonally-shaped mononuclear cell with a grooved nuclei intermixed by eosinophilical chondroid matrixes and local narcoses.ImmunohistochemicaIly,among 45 cases,the strong positive case for S-100 protein was 45.Meanwhile,modest positive case for D2-40 was 9 and weak positive case was 11 in another 26 cases.And few tumor cells were high level expression for Ki-67 except for cases of 5(the positive rate<5%).Conclusion Chondroblastoma of bone is a rare,benign cartilaginous neoplasm.Apart from chicken-wire-like calcification,more emphasis on its eosinophilical chondroid stroma and S-100 and D2-40 expression are essential in making the correct diagnosis.
Total En Bloc Spondylectomy for Thoracic and Lumbar Chondrosarcoma

Abstract:Objective To investigate technique and clinical effect of total en bloc spondylectomy for thoracic and lumbar chondrosarcoma.Methods From January 2010 to March 2012,6 patients with thoracic or lumbar chondrosarcoma underwent total en bloc spondylectomy.There were 4 males and 2 females,aged from 25 to 54 years (average,38 years).The tumor ranged from T3 to L3; 1 located in T3 and T4,1 in T7,1in T11,1 in L1,1 in L2 and 1 in L3.According to Tomita surgical classification system,there was 1 case of type 2,1 case of type 4,3 cases of type 5 and 1 case of type 6.One patient underwent tumor resection through single posterior approach,while the other 5 patients underwent anterior dissection and posterior resection of tumor.All spines were reconstructed by posterior fixation with pedicle screws and anterior interbody fusion with titanium mesh cages or artificial vertebrae.Results The average amount of blood loss was 3200 ml (range,2100 to 6300 ml).The duration of operation ranged from 3.5 to 12 hours (average,5.5hours).Two patients obtained wide resection,3 obtained marginal resection,and 1 had intralesional margin.The complications included 2 cases of cerebrospinal leak,1 case of pleural effusion and 1 case of pulmonary infection.There was no wound infection and death during peroperative period.All patients were followed up for 6 to 32 months (average,19 months).The neurological function improved from preoperative Frankel C to postoperative Frankel E in 2 cases.All patients obtained bone union 6 to 12 months (average,8 months) after operation.At final follow-up,all patients could walk without aid,and there was no recurrence.Conclusion The total en bloc spondylectomy is an effective method for thoracolumbar chondrosarcoma,which could provide a satisfied tumor control and neurological function improvement.

Abstract:The research retrospectively analyzed cases of spinal chordoma and chondrosarcoma involving patients who received treatment at the two hospitals between 2001 and 2023. Among the 48 patients studied (39 chordoma and 9 chondrosarcoma cases), the average age was 53.9 ± 15.8 years, with a range of 17 to 86 years. Out of these patients, 43 underwent excision surgery and were categorized based on tumor margin into negative (R0) or microscopically positive (R1) margin ( n = 14) and macroscopically positive (R2) margin ( n = 29) groups. The mean overall survival (OS) for R0/R1 and R2 groups was 156.5 ± 19.3 and 79.2 ± 11.9 months, respectively ( p value = 0.012). The mean progression-free survival (PFS) for R0/R1 and R2 was 112.9 ± 24.4 and 25.5 ± 5.5 months ( p value < 0.001). The study showed that regardless of whether patients in the R0/R1 or R2 groups received radiation therapy (RT) or not, there was no significant improvement in OS or PFS. Specifically, the OS and PFS for the RT only group were 75.9 ± 16.6 and 73.3 ± 18.0 months. In conclusion, the recommended treatment approach for spinal chordoma and chondrosarcoma remains en bloc resection surgery with an appropriate margin. Patients who are unsuitable for or decline surgery may find a beneficial disease control rate with traditional external beam photon/proton therapy.

Clinical Analysis of 18 Cases of Chordomas in the Skull Base

Abstract:Objective: To investigate the diagnosis and treatment of chordomas in the skull base.Methods: The clinical data,including image presentation,operative finding,histological diagnosis and prognosis of 18 patients with chordomas in the skull base who underwent surgery 1996-2004,were reviewed.Results: No distinctive symptoms occurred in these patients. Total resection was achieved in 5 cases,and subtotal resection in 13 cases.17 cases were typical chordomas and 1 case was chondroid chordomas.14 patients were followed up,4 died,10 survived.Conclusion: Chordomas in the skull base is an rare tumor and difficult to perform total resection.Microsurgery combined with radiotherapy or(γ-knife) therapy is recommended.The prognosis is very poor.
Surgical management of chondrosarcomas of the skull-base and temporal bone

Abstract:Objectives: To analyze the overall long-term outcome of surgically treated skull base and temporal bone chondrosarcomas. Methods: The medical records of patients with surgically treated skull base and temporal bone chondrosarcomas between 1983 and 2024 were thoroughly evaluated. Results: Out of a total of over 5000 skull base surgeries performed at our center, only 29 patients had histopathologically confirmed chondrosarcomas of the skull base and temporal bone. The mean of patients age was 45.6, and the male-to-female ratio was 1.9:1. The most common symptoms included hearing loss (58.6%), tinnitus (41.4%), diplopia (31%), dysphonia (24.1%), dysphagia (20.7%), vertigo (10.3%), and dizziness (10.3%). The most frequent locations of lesions among the 29 patients are as follows: petroclival region (34.5%), jugular foramen (27.6%), petrous apex (17.2%), middle ear (13.8%), others (3.4%). TO, IFTA, IFTB, IFTC, POTS, and combined surgical approaches were commonly used. The rate of gross total removal and recurrence was 82.6% and 13.8% respectively. The follow-up duration of 6 patients was more than five years and less than ten years whereas ten patients had more than ten years of follow-up. Conclusions: Chondrosarcoma of the skull base and temporal bone is a very rare pathology. Due to its multiple potential sites of origin and histological specificity, it presents us with significant challenges. Gross total removal is the primary treatment for chondrosarcoma of the skull base and temporal bone. Personalized decision-making should be considered based on the following aspects: tumor, patient, and surgeon's factors. Postoperative radiation therapy is complementary to surgical treatment in grades II and III lesions to achieve long-term survival.

Abstract:OBJECTIVE:To study the clinical, pathologic and radiologic features of chondroblastoma occurring in sites other than epiphysis and apophysis of long bones, and to investigate possible reasons for misdiagnosis.METHODS:The clinical, pathologic and radiologic data of 18 chondroblastoma cases occurring in atypical sites were collected from 5 major hospitals in Shanghai during the past 12 years. S-100 immunostaining was performed to confirm the cartilaginous differentiation of the tumor cells.RESULTS:Chondroblastoma occurred in small bones of feet in 10 of the 18 cases (55.6%) studied, being commonest in the talus and calcaneus bones. Mean age of the patients was 27.8 years, with 55.6% over 25 years of age. Radiologic examination revealed expansive, multilocular and well-demarcated radiolucent lesions in most cases. There was local cortical destruction in 5 cases (28%) and soft tissue infiltration in 1 case. In 10 cases (55.6%), the tumor was associated with aneurismal bone cyst or simple bone cyst formation. None of the cases studied was accurately diagnosed clinically before the operation. In 2 cases, the pathology was also misdiagnosed, often being diagnosed as aneurismal bone cyst or giant cell tumor.CONCLUSIONS:Chondroblastoma occurring in atypical sites are often associated with atypical age, radiologic features and pathologic findings at presentation. Thorough understanding of the potential pitfalls is essential in order to avoid misdiagnosis.
Clinical and pathological analysis of clear cell chondrosarcoma

Abstract:Objective:To investigate the clinical manifestations, imaging features, histopathological features, diagnostic pitfalls, treatment and prognosis of clear cell chondrosarcoma (CCCS).Methods:23 cases of CCCS admitted and operated from January 2010 to January 2020 were analyzed retrospectively. Among the 23 cases, 21 were males and 2 were females. There were 8 cases (35%) aged 21-40, 10 cases (43%) aged 41-60 and 5 cases (23%) aged 61-80. There were 8 femurs, 7 pelvis, 4 thoracolumbar spine, 3 sacrum and 1 tibia. The specimens were fixed with 10% phosphate-buffered formalin, decalcified with 5% nitric acid, embedded in paraffin and stained with hematoxylin and eosin (HE) and immunohistochemistry (Envision). The preoperative imaging and clinical symptoms, and the postoperative histopathological and immunophenotype under the microscope were collected. And the relevant literature was reviewed to summarize the clinical, imaging and pathomorphological characteristics of CCCS.Results:23 cases of CCCS showed bone destruction in imaging, some cases were well-circumscribed lytic lesions, some cases had sclerotic margin. The serum alkaline phosphatase was increased in 7 patients before operation. The tumor tissue was gray-white and gray-red in general and some cases showed porcelain white cartilage-like areas. Microscopically, the tumor cells are round or polygonal, some of them have clear cytoplasm and boundary, some of them are eosinophilic, some of them have round and centrally located nuclei, and mitotic image is rare. It is often seen that there are nodular distribution of cartilage-like matrix and immature woven bone, multinucleated osteoclast-like giant cell scattered in those components. Immunohistochemical staining: S-100, D2-40, EMA, Vimentin, p16, SATB2 can be positive in varying degrees. The surgical treatment is mainly through en bloc excision. 10 patients had recurrence and no distant metastasis.Conclusion:CCCS is a rare subtype of chondrosarcoma, which has low-grade malignant biological behavior and is easy to be misdiagnosed clinically and pathologically. Pathological diagnosis needs to be careful. Careful observation of microscopic histology is necessary in order to avoid over-diagnosis of osteosarcoma leading to clinical treatment errors. Once the biopsy is confirmed, it needs en bloc excision in order to reduce the recurrence rate. Long-term follow-up is needed after the operation, the overall prognosis was good.

Abstract:Objective To review the long-term oncologic and functional outcomes of surgical treatment for pelvic chondrosarcoma at a single institution to further delineate surgical strategy of resection and function reconstruction.Methods All of 165 patients with pelvic chondrosarcoma that had been surgically treated between July 2000 and Dec 2013 were reviewed retrospectively.There were 102 male and 63 female patients who had a mean age of 43.5 (range,13 to 75) years.The resection type included Type T 16,type Ⅱ 18,type Ⅲ 11,type Ⅰ+Ⅱ 19,type Ⅱ+Ⅲ 63,type Ⅰ+Ⅳ 15,type Ⅰ+Ⅱ+Ⅲ 10 and type Ⅰ+Ⅱ+Ⅳ 13 cases.The pathology diagnosis was grade Ⅰ in 15,grade Ⅱ in 88,grade Ⅲ in 20,dedifferentiated in 28,mesenchymal in 12 and clear cell chondrosarcoma in 2 patients.Nine cases were secondary lesion following multiple exostoses.114 patients received no previous treatment while others were referred due to recurrent tumor.En bloc resection was achieved in 142 patients and intralesional resection in 23 cases.Twelve patients had a hemipelvectomy to achieve local tumor control,whereas 153 patients underwent a limb-salvage procedure.Spinal screw-rod system was used to reconstruct anterior pelvic ring after ilium resection.Modular hemipelvic endoprosthesis was utilized to reconstruct periacetabular defect after tumor resection while no reconstruction was performed after ischium and pubis resection.Results The median duration of follow-up was 54.1 (range,l0 to 159) months.147 (89.1％) patients were alive at the final follow-up,seven of which were alive with tumor.The estimated 5-year survival was 81.5％.Thirty-three patients (20％) had local recurrence,and twenty (12.1％) had lung metastasis.Recurrent case,sacroiliac joint involvement and intralesional resction were three risk factors for postoperative recurrence.Patients who received type Ⅱ+Ⅲ resection and reconstruction had a mean functional score of 23 according to MSTS 93.The mean MSTS 93 for patients with type Ⅰ+Ⅱ and type Ⅰ+Ⅱ+Ⅳ resection and reconstruction was 18.The wound complication rate was 15.8％.Two aseptic loosening and 2 pubic connection plate breakage were observed in hemipelvic endoprosthesis reconstruction.Conclusion Tumor grade correlates with overall or disease-free survival.For low-grade chondrosarcoma,adequate surgical margin can provide long-term local control while for high-grade cases,distant metastasis is the main case of treatment failure.Adequate surgical margin can be achieved in cases with acetabulum involved.However,aggressive surgical resection of pelvic chondrosarcoma still remains as a challenge for orthopaedic oncologists when the tumor invades the iliosacral joint,which necessitates careful preoperative plan and skilled surgical techniques.
Surgical Treatment of Pelvic Chondrosarcoma: Retrospective Study of 13 Cases

Abstract:Objective To investigate procedures and complications in the treatment of pelvic chondrosarcoma.Methods A total of 13 patients suffered from pelvic chondrosarcoma were reviewed retrospectively.According to Enneking's division,there were 1 case with lesion in region Ⅰ,3 in region Ⅱ,4 in region Ⅲ,2 in region Ⅰ and Ⅱ,2 in region Ⅰ and Ⅳ,and 1 in region Ⅰ,Ⅱ and Ⅲ.The surgical procedures included 5 cases with resection,1 with hemipelvectomy,2 with resecion,TSRH instrumentation and autograft reconstruction,2 with hemipelvic prosthesis,1 with resection,ipsilateral femoral autograft reconstruction and conventional hip joint replacement,2 with resection,ischiofemoral arthodesis,1 with a second resection and iliofemoral arthrodesis after local recurrence.Results The median follow-up time was 27.5 months(3-57 months).The patients with lesion in region Ⅰ and Ⅲ maintained better function after tumor resection.The average function evaluation score was 76.4%.The function was worse in the patients with hemipelvectomy.Lung metastasis was not found in all the patients.There were 4 patients with complications including incision infection in 2 patients and delayed healing of incision in 2 patients.The surgical margin was as follow: marginal resection in 3 patients,wide resection but tumor contamination in 1 patient,wide resection in the other patients.Conclusion The surgical resection was a primary treatment for pelvic chondrosarcoma.Wide resection was a significant prognosis factor for the long-term survival and local recurrence decrease.Individual reconstruction was a key to maintain a good function.

Abstract:Purpose To investigate characteristics of clear cell chondrosarcoma (CCCS) in clinicopathology, imaging, biological behavior, and differential diagnosis. Methods There were four cases of clear cell chondrosarcoma, and their clinical data, pathologic record, and follow-up materials were collected and reviewed. Immunohistochemical stains were applied for differentiation. Results All patients were middle-aged to elderly male with long duration. All lesions were located at the epiphyses of the tubular bone. X-ray showed a bone lytic defect with sharp demarcation in the early phase and expansion progressing toward diaphyses in the late phase. The lesion did not penetrate the cortex, and there was no extraskeletal soft tissue mass. Grossly they appeared grayish white. Two of them had coexistent aneurysmal bone cysts. Microscopically, clear cells arranged in an indistinct lobular pattern characterized the tumor. The centers of the lobules contained short trabeculae of bone and osteoclast-like giant cell intermingled with the cells between the lobules. Tumor cells were well delineated with clear and vaculated cytoplasm. S-100 protein and type Ⅱ collagen were positive. Clinical treatment and follow up were shown: case 1 was recurred 6 years postoperatively. Cases 2 and 3 had tumor removal and then radiation therapy. All three cases were followed up from 2.4 years to 6 years, and currently were fine. Case 4 did right proximal femoral amputation and lost contact. Conclusions CCCS is a rare variant of low grade chondrosarcoma. The features of CCCS are different from conventional chondrosarcoma in terms of the clinical development, imaging,and pathology.In the practice,it should be differentiated from not only benign chondroblastoma and osteoblastoma,but also high grade clear cell osteosarcoma and metastatic clear cell carcinoma.

Abstract:Objective: To assess the surgical outcome of patients with mesenchymal chondrosarcoma(MCS) treated in our institute.This study was also designed to describe the clinical characteristics,treatment,and outcome of MCS to provide a better understanding of its clinical management.Methods: A total of 27 patients with MCS were treated in Peking University People's Hospital,Beijing,China from October 1997 to March 2011.Demographic information and follow-up data were obtained and statistically analyzed.Results: Among the 27 patients,9 were males and 18 were females with a mean age of 30.4 years(ranging from 14 years to 51 years).The median follow-up time was 42.6 months(ranging from 6 months to 104 months).Among the total number of tumor cases,22 and 5 were detected in bone tissues and extra-skeletal sites,respectively.A total of 25 patients underwent surgery,but only 17 achieved the standard surgical margin of wide excision.Among these patients,16 and 13 were subjected to chemotherapy and irradiation.The threeand five-year survival rates were 65% and 49.5%,respectively.Conclusion: MCS is a rare tumor resulting in morbidity with local recurrences and long-term metastases.In this study,standard multimodal regimens were proposed to treat MCS.The results recommended wide resection with suitable surgical margins as the preferred treatment.However,further studies should be conducted because the information about the benefits of chemotherapy and radiotherapy for the control of local or systemic symptoms of MCS remains insufficient.

Abstract:A 14‐year‐old male patient with chondroblastoma affecting the proximal tibia and tibial eminence. The procedure involved thorough removal of the lesion using direct visualization with the management of the cavity utilizing a substitute for autologous bone graft. Background Chondroblastoma is a rare bone tumor that originates from the epiphysis, constitutes around 1% of all primary bone tumors and is recognized for its tendency to exhibit local invasiveness, as well as the possibility of metastasis and recurrence in nearby areas. Currently, the main surgical treatment for chondroblastoma is open surgery, involving excision of the lesion. There are relatively few reports on arthroscopic surgery for the treatment of chondroblastoma. However, open surgical curettage is associated with operation‐related trauma and potential for damage to the osteoepiphysis resulting in growth disturbances. Case Presentation This case study presents the application of an arthroscopic technique in a 14‐year‐old male patient with chondroblastoma affecting the proximal tibia and tibial eminence. The procedure involved thorough removal of the lesion using direct visualization with the management of the cavity utilizing a substitute for autologous bone graft. After 1 year of follow‐up, the patient remains free from symptoms, exhibits normal knee functionality, and radiographic analysis reveals a good autologous bone graft fusion without any signs of recurrence. Conclusions Based on the existing cases of arthroscopic treatment for chondroblastoma and the report of this case, arthroscopic treatment for chondroblastoma can be considered as a specific treatment option for certain patients. In some cases, this technique could be an effective alternative to open surgery.

Abstract:Study Design: Retrospective Cohort Study Objectives: Chondrosarcoma of mobile spine is a rare aggressive malignant tumor and postsurgical local recurrence rates remain high. En bloc resection is currently the preferred treatment. Resection that achieves non-intralesional removal of the tumor may enable more complete removal of tumor tissue but significantly increases the complexity and risk of surgery and results in more postoperative complications. We sought to compare surgical outcomes, complications, and prognoses between patients who underwent en bloc resection with and without intralesional removal of the tumor. Methods: We reviewed 56 patients with spinal chondrosarcoma who underwent en bloc tumor resection and reconstructive surgery at our center between 2000 and 2024 with a minimum postoperative follow-up of 1 year. We collected and analyzed data regarding surgical procedures, complication characteristics, and local tumor control and recurrence. Results: We included 56 patients. Of these, 36 patients underwent the first surgery, and 20 experienced recurrences. All patients underwent en bloc tumor resection; 36 and 20 underwent intralesional and non-intralesional resections, respectively. We recorded 83 complications; the incidence and the number of major complications were significantly higher in the non-intralesional surgery group. Thirty patients experienced tumor recurrence and 26 patients died. Non-intralesional en bloc resection and conventional-type chondrosarcoma were predictive factors for reduced long-term postoperative recurrence and mortality risk. Conclusions: Non-intralesional resection carries higher risks and is associated with a greater number of perioperative complications, but reduces the risk of local tumor recurrence and prolongs recurrence-free survival and overall survival, providing patients with better prognoses.
