Learning Curves and Influencing Factors of Laparoscopic Single-Site Myomectomy (LESS-M)

Abstract:Study Objective The objective of this study was to analyze the learning curves of laparoscopic single-site myomectomy (LESS-M) and to explore the influencing factors of operative time. Design Retrospective observational study. Setting University-based hospital. Single center. Patients or Participants 213 patients who underwent LESS-M by 5 surgeons with 15-25 years experiences of laparoscopic surgeries between March 2017 and November 2019. Interventions Laparoscopic single-site myomectomy (LESS-M). Measurements and Main Results Learning curves were obtained by cumulative sum (CUSUM) analysis of operative time, estimated blood loss and postoperative hospital stay. Variables such as age, body mass index, characteristics of myomas, type of suture and suturing layers, adhesion conditions, whether to enter uterine cavity, concomitant surgeries and surgeon experiences were reviewed to study the influencing factors of operative time by linear regression model. Each surgeon performed 29-53 cases of LESS-M. CUSUM curves of all 5 surgeons showed multiple fluctuation instead of ideal increasing-plateau-decreasing trend. Number, size and location of the myomas, adhesion condition, type of suture and the surgeon experiences were the determining influencing factors of surgical time of LESS-M procedure (P<0.05). Conclusion LESS-M procedure can be quickly learned and performed by surgeons with more than 15 years experiences of laparoscopy. The characteristics of myomas, suture and surgeon experiences may influence the operative time of LESS-M.
