Convolutional Neural Network Learning from Combinations of RT Dose Distribution, CT and PET Improves Predicting Locoregional Recurrence for Head and Neck Cancer

Abstract:To investigate the feasibility and performance improvement of introducing convolutional neural network (CNN) into locoregional recurrence prediction from combinations of RT dose distribution, CT and PET for head and neck cancer cases. A VGG-19 CNN was constructed and connected to a Cox proportional hazards model to jointly established a survival CNN framework. Inputs were set three channels with respective to the RT dose distribution map, CT and PET image all at the slice which contained the maximum GTV pixels. Output was the patient locoregional recurrence event. A cohort of 237 patients with head and neck cancer from four different institutions was obtained from The Cancer Imaging Archive and utilized to train and validate the survival CNN model. 141 were used to train the model and the other 96 were to validate. Inputs were reinforced with flipping and rotating, and 1000 epochs were used with each minibatch containing 30 patient data during model training. A negative partial log-likelihood was used for the loss function and an adam to choose to solve it. Model performance was explicated with comparison to CI and the Kaplan–Meier curves analysis generated from traditional radiomics based model.
