Invasion Characteristics and Validation of the T Descriptor in the 8th Edition of TNM Staging System for Patients with Masaoka-Koga Stage III Thymoma

Abstract:To analyze the invasion characteristics and to validate T descriptor of the tumor, node, metastasis (TNM) staging system in an independent cohort of patients with Masaoka-Koga stage III thymoma. A total of 179 patients diagnosed as Masaoka-Koga stage III thymoma from January 2000 to June 2018 were analyzed retrospectively. Among them, 94 patients underwent complete resection, 39 patients underwent incomplete resection and 46 patients underwent biopsy only. All patients were reclassified according to the 8th edition of TNM staging system. Kaplan-Meier method was used to calculate the cumulative survival rate, Log-rank method was used to compare between groups, and Cox regression was used for multivariate analysis. The median follow-up time was 50 months. The most common site of invasion was mediastinal pleura (154/179, 86%), followed by pericardium (91/179, 50.8%), great vessel (73/179, 40.8%), lung (65/179, 36.3%), phrenic nerve (19/179,10.6%) and chest wall (3/179, 1.7%). The proportion of great vessel invasion in complete resection group (14.9%) was significantly lower than that in the incomplete resection group (79.5%) and biopsy only group (60.9%, P < 0.001). After reclassification, 179 patients were assigned to T1b (22 cases), T2 (26 cases), T3 (102 cases) and T4 (29 cases). There were overall survival (OS) and progression free survival (PFS) difference between T2 and T3 (P = 0.021 and P = 0.003, respectively), while no significant OS or PFS difference was found between T1b and T2 (P = 0.656 and P = 0.210, respectively), T3 and T4 (P = 0.955 and P = 0.777, respectively). When T1b combined with T2 compared with T3 combined with T4, statistically significant difference was found in both OS (P = 0.001) and PFS (P <0.001). The multivariate analysis showed that T stage classification was not an independent prognostic factor for OS or PFS, while surgical completeness was the most important independent prognostic factor for both OS (P<0.001) and PFS (P<0.001). For Masaoka-Koga stage III thymoma, mediastinal pleura is the most common site of invasion. The invasion of mediastinal pleura, pericardium and lung has little effect on resectability, while great vessel involvement can significantly affect surgical resectability. The TNM staging system was not found to be an independent prognostic factor. Surgical completeness was the most important factor affecting prognosis.
