Abstract:OBJECTIVE:To investigate the application of CAD/CAM technique in reconstruction of mandible defect.METHODS:Before surgery, three-dimensional computed tomography was performed in 23 patients with mandible lesions, then the CT data were transformed into a readable format to produce facsimile models by using rapid prototyping (RP) techniques. When individual mandibular models were produced, evaluation and surgical simulation were performed on the model, which included measuring range of mandible lesions, prefabrication of mandibular reconstructive titanium plates or distractors, ascertaining precise position of transport disk, and shaping the free vascularized fibula, etc. According to the simulation, the mandible reconstruction was accomplished in operation.RESULTS:The surgical simulation on the model produced by CAD/CAM technique highly resembled the real operation. The measurements of the lesion on the model were completely correspondent with those found in operation, and the prefabricated plates and distractor fitted the contour of the mandible. The operation time shortened and the results were satisfactory. The post-operation X-ray showed that the mandible was symmetrical and the maxillofacial contour maintained.CONCLUSIONS:CAD/CAM technique was very helpful for simulation of anatomically complicated mandible defect. By preoperative simulation of the mandible and lesions, surgeons are able to improve or refine surgical planning so that better results can be assured.
An automatic algorithm for repairing dental models based on contours

Abstract:Dental model repair is a critical stage in the orthodontic treatment. The repaired teeth need to preserve the original morphological features, and the algorithm is required to be automatic for saving time and avoiding uncertainty caused by human handling. This paper presents an efficient and automatic algorithm to repair the tooth models' large holes. Our algorithm consists of three main steps: model's principal axis determination, model's contours processing (generation and repair), and hole surface reconstruction & fairing. We propose a novel LRBC (Linear Regression on Boundaries' Centers) algorithm to determine the model's principal axis. After generating contours based on the principal axis, we adopt point-normal cubic B-spline curve interpolation to repair each broken contour line. At last, we reconstruct holes' point-sets by constrained Delaunay triangulation, and smooth the patches with a modified Laplacian operator. Experiments on different dental models demonstrate that our algorithm can not only achieve a result with a high morphological similarity and a low error, but also satisfy the requirements of actual orthodontic applications.

Abstract:Orthodontic treatment is a lengthy process that requires regular in-person dental monitoring, making remote dental monitoring a viable alternative when face-to-face consultation is not possible. In this study, we propose an improved 3D teeth reconstruction framework that automatically restores the shape, arrangement, and dental occlusion of upper and lower teeth from five intra-oral photographs to aid orthodontists in visualizing the condition of patients in virtual consultations. The framework comprises a parametric model that leverages statistical shape modeling to describe the shape and arrangement of teeth, a modified U-net that extracts teeth contours from intra-oral images, and an iterative process that alternates between finding point correspondences and optimizing a compound loss function to fit the parametric teeth model to predicted teeth contours. We perform a five-fold cross-validation on a dataset of 95 orthodontic cases and report an average Chamfer distance of 1.0121 mm2 and an average Dice similarity coefficient of 0.7672 on all the test samples in the cross-validation, demonstrating a significant improvement compared with the previous work. Our teeth reconstruction framework provides a feasible solution for visualizing 3D teeth models in remote orthodontic consultations.

Abstract:Abstract Cone-beam computed tomography (CBCT) is one of the most widely used digital models in dental practices. A critical step in virtual dental treatment planning is to accurately delineate all tooth-bone structures from CBCT with high fidelity. Previous studies have established several methods for CBCT segmentation using deep learning. However, the inherent resolution discrepancy and the loss of occlusal information in CBCT largely limited its clinical applicability. Here, we present a Deep Dental Multimodal Analysis (DDMA) framework consisting of a CBCT segmentation model, an intraoral scan (IOS) segmentation model (the most accurate digital dental model), and a fusion model to generate 3D fused tooth-bone structures with high fidelity and accurate occlusal information. Our model was trained with a large-scale dataset with 503 CBCT and 28,559 IOS meshes manually annotated by human experts. For CBCT segmentation, our framework achieves an average Dice coefficient of 93.99%, significantly outperforming the baselines. For IOS segmentation, our model achieves a mIoU of 93.07% and 95.70% on the maxillary and mandible on a test dataset of 200 IOS meshes, 1.77% and 3.52% higher than the state-of-art method. In addition, our DDMA framework takes 20 to 25 minutes to generate the fused model, compared to over 5 hours by human experts. Notably, real-world clinical cases demonstrate that our model can visualize tooth-bone structures and relationships during the entire orthodontic treatment and predict clinical risks like dehiscence and fenestration. These findings demonstrate the potential of deep multimodal learning to improve the quality of digital dental models and guide clinical treatment and diagnostic decisions.

Abstract:A complete digital tooth model is needed for computer-aided orthodontic treatment. However, current methods mainly use computed tomography (CT) images to reconstruct the tooth model which may require multiple CT scans during orthodontic progress, and the reconstructed model is also inaccurate in crown area. This study developed a tooth model reconstruction method based on integration of CT images and laser scan images to overcome these disadvantages. In the method, crown models and complete tooth models are first reconstructed, respectively, from laser scan images and CT images. Then, crown models from laser scan images and tooth models from CT images are registered. Finally, the crown from laser scan images and root from CT images were fused to obtain a new tooth model. Experimental results verified that the developed method is effective to generate the complete tooth model by integrating CT images and laser scan images. Using the proposed method, the reconstructed models provide more accurate crown than CT images, and it is feasible to obtain complete tooth models at any stage of orthodontic treatment by using one CT scan at the pretreatment stage and one laser scan at that stage to avoid multiple CT scans.

Abstract:Purposes: Large-scale jaw reconstruction can hardly achieve satisfactory results only by relying on doctors' experience. In this study, we assessed a new approach using a machine learning algorithm based on jaw feature points to assist complex jaw reconstruction in patients with maxillary and mandibular defects. Methods: One hundred and two computed tomography (CT) data on the jaw were collected and 16 skeletal marker points on the jaw were selected. The machine learning algorithm learned the positional relationship between points and built a model, which was used to predict the coordinate position of an unknown point. Then the model was used for a surgical plan in clinical cases. Results: The linear regression model based on machine learning can control the error within 3 mm. In linear models, Lasso has a slight advantage over the others. We used Lasso to predict the missing points for two patients with maxillary and mandibular defect, respectively. The operation was carried out as planned, and the defects were successfully repaired. Conclusions: The restoration of jaw feature points based on a machine learning algorithm is expected to solve large-scale jaw defects without contralateral reference. Abstract:Tooth arrangement is a crucial step in orthodontics treatment, in which aligning teeth could improve overall well-being, enhance facial aesthetics, and boost self-confidence. To improve the efficiency of tooth arrangement and minimize errors associated with unreasonable designs by inexperienced practitioners, some deep learning-based tooth arrangement methods have been proposed. Currently, most existing approaches employ MLPs to model the nonlinear relationship between tooth features and transformation matrices to achieve tooth arrangement automatically. However, the limited datasets (which to our knowledge, have not been made public) collected from clinical practice constrain the applicability of existing methods, making them inadequate for addressing diverse malocclusion issues. To address this challenge, we propose a general tooth arrangement neural network based on the diffusion probabilistic model. Conditioned on the features extracted from the dental model, the diffusion probabilistic model can learn the distribution of teeth transformation matrices from malocclusion to normal occlusion by gradually denoising from a random variable, thus more adeptly managing real orthodontic data. To take full advantage of effective features, we exploit both mesh and point cloud representations by designing different encoding networks to extract the tooth (local) and jaw (global) features, respectively. In addition to traditional metrics ADD, PA-ADD, CSA, and MErot , we propose a new evaluation metric based on dental arch curves to judge whether the generated teeth meet the individual normal occlusion. Experimental results demonstrate that our proposed method achieves state-of-the-art tooth alignment results and satisfactory occlusal relationships between dental arches. Abstract:Automatic 3-dimensional tooth segmentation on intraoral scans (IOS) plays a pivotal role in computer-aided orthodontic treatments. In practice, deploying existing well-trained models to different medical centers suffers from two main problems: (1) the data distribution shifts between existing and new centers, (2) the data in the existing center is usually not allowed to share while annotating additional data in the new center is time-consuming and expensive. In this paper, we propose a Model Adaptive Tooth Segmentation (MATS) framework to alleviate these issues. Taking the trained model from a source center as input, MATS adapts it to different target centers without data transmission or additional annotations, as inspired by the source data-free domain adaptation (SFDA) paradigm. The model adaptation in MATS is realized by a tooth-level feature prototype learning module, a progressive pseudo-labeling module and a tooth-prior regularized information maximization loss. Experiments on a dataset with tooth abnormalities and a real-world cross-center dataset show that MATS can consistently surpass existing baselines. The effectiveness is further verified with extensive ablation studies and statistical analysis, demonstrating its applicability for privacy-preserving tooth segmentation in real-world digital dentistry.

Abstract:Due to the large number of medical images generated each year, data-driven machine learning methods hold great promise in the field of medical image diagnosis and analysis. In recent years, the application of deep neural networks in oral and cranio-maxillofacial surgery has gradually increased, helping surgeons to obtain more comprehensive information and develop more accurate treatment planning, including segmentation and recognition of anatomical structures, disease diagnosis, and automatic surgical planning. This paper presents a new dataset of maxillofacial defect images, containing computed tomography (CT) data of 125 normal human cranial skeletal 3D reconstructions and 2625 corresponding craniomaxillofacial bone defects. The data are classified according to seven different types of defects which are anterior skull and forehead, zygomatic arch, zygomatic bone, Naso-orbital-ethmoidal (NOE), wall of the maxillary sinus, mandibular body, mandibular ramus, and condylar respectively. Each kind of defect is further divided into three categories according to the different locations, including left side, right side, and bilateral sides (or defect passes through the midline of the face). There are three main contributions: first, a dataset is publicly available to assist researchers in the development of data-based methods for generating missing portions of the skull; second, a repair method is proposed to evaluate the dataset; third, a review of various related datasets. The release of this dataset aims to advance artificial intelligence methods in the field of oral and cranio-maxillofacial surgery and plastic surgery helping physicians to reduce the difficulty and time consumption while improving the efficiency and accuracy of prosthesis design.

Abstract:Orthodontic treatment usually requires regular face-to-face examinations to monitor dental conditions of the patients. When in-person diagnosis is not feasible, an alternative is to utilize five intra-oral photographs for remote dental monitoring. However, it lacks of 3D information, and how to reconstruct 3D dental models from such sparse view photographs is a challenging problem. In this study, we propose a 3D teeth reconstruction framework, named TeethDreamer, aiming to restore the shape and position of the upper and lower teeth. Given five intra-oral photographs, our approach first leverages a large diffusion model's prior knowledge to generate novel multi-view images with known poses to address sparse inputs and then reconstructs high-quality 3D teeth models by neural surface reconstruction. To ensure the 3D consistency across generated views, we integrate a 3D-aware feature attention mechanism in the reverse diffusion process. Moreover, a geometry-aware normal loss is incorporated into the teeth reconstruction process to enhance geometry accuracy. Extensive experiments demonstrate the superiority of our method over current state-of-the-arts, giving the potential to monitor orthodontic treatment remotely. Digitized occlusal reconstruction for dental prostheses

Abstract:The aim of this study was to explore a method for automatically reconstructing the occlusal surface by CAD technology. A 3D graphic database of standard tooth crown (STC) was established from physical models. Referring to the contact rule, the occlusal surface of STC was partitioned into several functional regions, and the regions related with occlusal adjustment were further defined as some sets of discrete points. Additionally, geometrical marks were added to the database to embody dimensional contours and orientation of STC. Then an adaptive redesign methodology was developed to form occlusal surface: firstly, a STC similar to patient's anatomical characteristics was scaled and registered to the abutment teeth; secondly, the distances between the point sets and the antagonist were calculated to quantify the occluding relation; finally, the occlusal surface was reconstructed by local deformation. The program provided a novel method to automatically adjust the size and location of the restoration, avoid interpenetrations and create necessary contacts. The final occlusal surface fulfills the esthetic and morphologic criteria in clinical dentistry and meets the physiologic demands of occlusal and proximal contacts.

Abstract:Summary High-fidelity three-dimensional (3D) models of tooth-bone structures are valuable for virtual dental treatment planning; however, they require integrating data from cone-beam computed tomography (CBCT) and intraoral scans (IOS) using methods that are either error-prone or time-consuming. Hence, this study presents Deep Dental Multimodal Fusion (DDMF), an automatic multimodal framework that reconstructs 3D tooth-bone structures using CBCT and IOS. Specifically, the DDMF framework comprises CBCT and IOS segmentation modules as well as a multimodal reconstruction module with novel pixel representation learning architectures, prior knowledge-guided losses, and geometry-based 3D fusion techniques. Experiments on real-world large-scale datasets revealed that DDMF achieved superior segmentation performance on CBCT and IOS, achieving a 0.17 mm average symmetric surface distance (ASSD) for 3D fusion with a substantial processing time reduction. Additionally, clinical applicability studies have demonstrated DDMF's potential for accurately simulating tooth-bone structures throughout the orthodontic treatment process.
ToothCR: A Two-Stage Completion and Reconstruction Approach on 3D Dental Model

Abstract:Deep neural networks have made a number of achievements both in tooth segmentation and arrangement on complete 3D dental models. But few studies have used deep learning methods on the tooth completion and reconstruction on the incomplete dental models. To rebuild the missing tooth from incomplete dental model, we propose a two-stage approach ToothCR which takes advantage of the powerful learning capabilities of deep neural networks. In the first stage, ToothCR introduces a geometry-aware transformer encoder into the 3D dental model completion task. Self-attention mechanism in transformers could better model long-range dependencies in point cloud and ensure the predicted missing parts to have precise geometric structures. In the second stage, ToothCR uses a novel surface reconstruction algorithm to recover the surface of the predicted missing tooth. The reconstruction algorithm guarantees the generated surface to be watertight and avoids holes or redundant meshes which traditional methods may produce. Extensive experiments conducted on 3D dental datasets show that our approach outperforms state-of-the-art methods both in qualitative and quantitative results.
The Application of Digital Model Surgery in the Treatment of Dento-Maxillofacial Deformities

Abstract:A retrospective study was conducted to explore the application of digital model surgery in the treatment of dento-maxillofacial deformities. Fifteen patients with dento-maxillofacial deformities were enrolled in this study. A digital craniomaxillofacial-dentition model was established by combining three-dimensional (3D) computed tomography (CT) and laser-scanned dentition models. Based on this composite model, surgical planning, simulations, and postoperative predictions were performed. Under the guidance of a 3D printed splint and guide, orthognathic surgeries and asymmetry corrections were performed. Clinical examinations were performed 3 months after surgery. Surgical accuracy was evaluated by comparing the postoperative CT 3D model with preoperative surgical planning. All patients successfully underwent virtual surgical planning, splint and guide 3D printing, and orthognathic surgery. Facial symmetry was greatly improved after surgery. The mean deviation between the preoperative design and actual surgical results was less than 2 mm. With preoperative planning, surgical simulation, and postoperative prediction, digital model surgery demonstrates great value in improving the accuracy of orthognathic surgery and restoring facial symmetry. It is regarded as a valuable technique to improve the results and execution of this potentially complicated procedure.

Abstract:The design of a maxillary anterior tooth crown is crucial to post-treatment aesthetic appearance. Currently, the design is performed manually or by semi-automatic methods, both of which are time-consuming. As such, automatic methods could improve efficiency, but existing automatic methods ignore the relationships among crowns and are primarily used for occlusal surface reconstruction. In this study, the authors propose a novel method for automatically reconstructing a three-dimensional model of the maxillary anterior tooth crown. A pose estimation network (PEN) and a shape estimation network (SEN) are developed for jointly estimating the crown point cloud. PEN is a regression network used for estimating the crown pose, and SEN is based on an encoder–decoder architecture and used for estimating the initial crown point cloud. First, SEN adopts a transformer encoder to calculate the shape relationship among crowns to ensure that the shape of the reconstructed point cloud is precise. Second, the initial point cloud is subjected to pose transformation according to the estimated pose. Finally, the iterative method is used to form the crown mesh model based on the point cloud. The proposed method is evaluated on a dataset with 600 cases. Both SEN and PEN are converged within 1000 epochs. The average deviation between the reconstructed point cloud and the ground truth of the point cloud is 0.22 mm. The average deviation between the reconstructed crown mesh model and the ground truth of the crown model is 0.13 mm. The results show that the proposed method can automatically and accurately reconstruct the three-dimensional model of the missing maxillary anterior tooth crown, which indicates the method has promising application prospects. Furthermore, the reconstruction time takes less than 11 s for one case, demonstrating improved work efficiency.

Abstract:With the development of 3-dimensional (3D) scanning technologies in dentistry, high accuracy optical scanning data from the crown and cone beam computed tomography data from the root can be acquired easily. In many dental fields, especially in digital orthodontics, it is useful to fuse the data from the crown and the root. However, the manual fusion method is complex and difficult. A novel automatic fusion method for 2-source data from the crown and the root was researched, and its accuracy was evaluated in this study. An occlusal splint with several alumina ceramic spheres was fabricated using heat-curing resin. A multipoint (center of each sphere) alignment method was performed to achieve rapid registration of the crown data from optical scanning and the root data from cone beam computed tomography. The segmentation algorithm based on heuristic search was adopted to perform extraction and segmentation of the crown from whole optical scanning data. The level set algorithm and the marching cubes (MC) algorithm were used to reconstruct digital imaging and communications in medicine data into a 3D model. A novel multisource data fusion algorithm, which is based on iterative Laplacian deformation (ILD), was researched and applied to achieve automatic fusion. Finally, the 3D errors of the method were evaluated. The 3 groups of typical tooth data were automatically fused within 2seconds. The mean standard deviation was less than 0.02mm. The novel method can aid the construction of a high-quality 3D model of complete teeth to enable orthodontists to safely, reliably, and visually plan tooth alignment programs.

Abstract:In digital orthodontic treatment, the high-precision reconstruction of complete teeth, encompassing both the crown and the actual root, plays a pivotal role. Current mainstream techniques, prioritizing the high resolution of intraoral scanned models (IOS), are confined to using IOS data for orthodontic treatments. However, the lack of root information in the IOS data may lead to complications such as dehiscence. In contrast, Cone Beam Computed Tomography (CBCT) data encompasses comprehensive dental information with roots. Nonetheless, the radiative character of CBCT scans renders patients unsuitable for repeated examinations in a short time. In addition, lower scanning precision of CBCT leads to suboptimal teeth segmentation outcomes, hindering the accurate representation of dental occlusal relationships. Therefore, in order to fully utilize the complementarity between dental multimodal data, we propose a method for high-precision 3D teeth model reconstruction based on IOS and CBCT, which mainly consists of global rigid registration and local nonrigid registration. Specifically, we extract the priori information of dental arch curves for coarse alignment to provide a good initial position for the Iterative Closest Point (ICP) algorithm, and design a conformal parameterization method for a single tooth to effectively obtain the point correspondence between IOS and CBCT crowns. The rough crown of the CBCT will gradually fit towards the IOS through iterative optimization of nonrigid registration. The experimental results show that our method robustly fuses the advantageous features of IOS and CBCT. The 3D teeth model reconstructed by our method contains the high-precision crown of IOS and the real root of CBCT, which can be effectively used in clinical orthodontic treatment.

Abstract:Segmentation of the masseter muscle (MM) on cone-beam computed tomography (CBCT) is challenging due to the lack of sufficient soft-tissue contrast. Moreover, manual segmentation is laborious and time-consuming. The purpose of this study was to propose a deep learning-based automatic approach to accurately segment the MM from CBCT under the refinement of high-quality paired computed tomography (CT). Fifty independent CBCT and 42 clinically hard-to-obtain paired CBCT and CT were manually annotated by two observers. A 3D U-shape network was carefully designed to segment the MM effectively. Manual annotations on CT were set as the ground truth. Additionally, an extra five CT and five CBCT auto-segmentation results were revised by one oral and maxillofacial anatomy expert to evaluate their clinical suitability. CBCT auto-segmentation results were comparable to the CT counterparts and significantly improved the similarity with the ground truth compared with manual annotations on CBCT. The automatic approach was more than 332 times shorter than that of a human operation. Only 0.52% of the manual revision fraction was required. This automatic model could simultaneously and accurately segment the MM structures on CBCT and CT, which can improve clinical efficiency and efficacy, and provide critical information for personalized treatment and long-term follow-up.
