Limitations of Microdialysis in Measuring Unbound Drug Concentrations in the Brain

Abstract:The purpose of this study was to discuss the suitability of microdialysis and alternative methods for assessing unbound drug concentration in the brain using caffeine (log P = -0.55), carbamazepine (log P = 2.1) and midazolam (log P = 3.33) as the model drugs. By comparison of the unbound concentration directly measured by microdialysis (C-md,C-b), concentration in cerebrospinal fluid (C-CSF), unbound concentration indirectly measured by brain homogenate binding (C-u,C-b) and plasma unbound concentration (C-u,C-p), it was concluded that the free drug hypothesis appears applicable to caffeine, carbamazepine and midazolam. Importantly, the application of microdialysis and the use of drug concentration as surrogate can only be used for compounds with low or moderate compounds, while the brain homogenate binding approach appears to be suitable for all compounds.
