Effects of Radix Salviae Miltiorrhizae Freeze-dried Powder on Pharmacokinetic Parameters of Nifedipine and Activities of Three CYP Isozymes

Abstract:This study investigated the pharmacokinetic interaction between Salvia miltiorrhiza freezedried powder injections and nifedipine tablets in rats. S. miltiorrhiza is beneficial for treating cardiovascular diseases, its use can be improved by analyzing its pharmacokinetic interactions with common drugs used in clinical practice, such as nifedipine. The experiment was divided into two parts. Part one, 24 rats were randomly divided into four groups: low, medium and high dose group (24, 42, and 84 mg/kg), S. miltiorrhiza freeze-dried powder group, and blank control group (saline). After 7 days of S. miltiorrhiza treatment, nifedipine (10 mg/kg) was administered to each group of 6 rats. The second part of the group is the same as the part one. Six rats per group were administered a cocktail of midazolam (1 mg/kg), dextromethorphan (1 mg/kg) and phenacetin (0.6 mg/kg), after 7 days of S. miltiorrhiza treatment. The plasma concentrations of nifedipine and each probe substrate were determined by HPLC and UPLC-MS/MS, respectively. After combined treatment, compared with saline group, MRT ((0-infinity)) , of nifedipine increased by 51.6% (P < 0.05), 1023% (P < 0.05), and 73.3% (P < 0.01) in the low, medium, and high-dose S. miltiorrhiza powder groups, respectively. The MRT ((0-infinity)) and MRT ((0-infinity) )of probe substrates (phenacetin, midazolam and dextromethorphan) in the middle-dose group were significantly increased (P < 0.05). S. miltiorrhiza freeze-dried powder could reduce nifedipine elimination in vivo, and its could inhibit Cypla2, Cyp3al, and Cyp2d2 in rats with continuous administration in medium concentration.
