Abstract:Time-of-flight (TOF) Magnetic Resonance Angiography (MRA) is a useful imaging technique which reflects blood flow and vasculature information. However, due to the low signal and contrast of arteries in TOF MRA, it is challenging to extract vascular features such as length, volume and tortuosity, through segmentation and tracing. Hence, in this paper, a simultaneous artery tracing and segmentation method is proposed to a generate quantitative intracranial vasculature map from TOF MRA. Instead of using original images, segmentation from a neural network model is used to initiate tracing, avoiding the low signal or contrast for small arteries. A tracing method is proposed based on cross-sectional best matching, followed by an optimization scheme from the multiplanar reformatted view. Centerline positions, lumen radii and centerline deviations are jointly optimized for robust tracing within artery regions. Finally, the refined artery traces are used for better artery segmentation. The method is validated on eight TOF MRAs of both healthy subjects and patients with cerebrovascular disease, showing good agreements with human supervised tracing and segmentation results for representative features such as artery length (<4% mean absolute difference), volume (>0.80 Dice), and tortuosity (<3% mean absolute difference). Our method out-performs three other popular tracing and segmentation methods by a large margin.
Motion-compensated 3D Turbo Spin-Echo for More Robust MR Intracranial Vessel Wall Imaging.

Abstract:Purpose: (1) To investigate the effect of internal localized movement on 3DMR intracranial vessel wall imaging and (2) to develop a novel motion-compensation approach combining volumetric navigator (vNav) and self-gating (SG) to simultaneously compensate for bulk and localized movements. Methods: A 3D variable-flip-angle turbo spin-echo (ie, SPACE) sequence was modified to incorporate vNav and SG modules. The SG signals from the center k-space line are acquired at the beginning of each TR to detect localized motion-affected TRs. The vNavs from low-resolution 3D EPI are acquired to identify bulk head motion. Fifteen healthy subjects and 3 stroke patients were recruited in this study. Overall image quality (0-poor to 4-excellent) and vessel wall sharpness were compared among the scenarios with and without bulk and/or localized motion and/or the proposed compensation strategies. Results: Localized motion reduced wall sharpness, which was significantly mitigated by SG (ie, outer boundary of basilar artery: 0.68 +/- 0.27 vs 0.86 +/- 0.17; P = .037). When motion occurred, the overall image quality and vessel wall sharpness obtained with vNav-SG SPACE were significantly higher than those obtained with conventional SPACE (ie, basilarartery outer boundary sharpness: 0.73 +/- 0.24 vs 0.94 +/- 0.24; P = .033), yet comparable to those obtained in motion-free scans (ie, basilarartery outer boundary sharpness: 0.94 +/- 0.24 vs 0.96 +/- 0.31; P = .815). Conclusion: Localized movements can induce considerable artifacts in intracranial vessel wall imaging. The vNav-SG approach is capable of compensating for both bulk and localized motions.
Spiral time-of-flight magnetic resonance angiography for intracranial vascular imaging: performance compared to conventional Cartesian angiogram

Abstract:Background: Computed tomography angiography (CTA) and digital subtraction angiography (DSA) usually raise the risk of potential malignancies with cumulative radiation doses. Current time-of-flight magnetic resonance angiography (TOF-MRA) (dubbed as cTOF), which is based on Cartesian sampling mode, may show limited diagnostic conspicuity at sinuous or branching regions. It is also prone to relatively high false positive diagnoses and undesirable display of distal intracranial vessels. This study aimed to use spiral TOF-MRA (sTOF) as a noninvasive alternative to explore possible improvement, such that the application of magnetic resonance angiography (MRA) can be extended to facilitate clinical examination or cerebrovascular disease diagnosis and follow-up studies. Methods: Initially, 37 patients with symptoms of dizziness or transient ischemic attack were consecutively recruited for suspected intracranial vascular disease examination from Zhongshan Hospital of Xiamen University between July 2020 and April 2021 in this cross-sectional prospective study. After excluding 1 patient with severe scanning artifacts, 1 patient whose scanning scope did not meet the requirement, and 1 patient with confounding tumor lesions, a total of 34 participants were included according to the inclusion and exclusion criteria. Each participant underwent intracranial vascular imaging with both sTOF and cTOF sequences on a 3.0 T MR scanner with a conventional head-neck coil of 16 channels. Contrast CTA or DSA was also performed for 15 patients showing pathology. Qualitative comparisons in terms of image quality and diagnostic efficacy ratings, quantitative comparisons in terms of signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), vessel length, and sharpness were evaluated. Pair-wise Wilcoxon test was performed to evaluate the imaging quality derived from cTOF and sTOF acquisitions and weighted Cohen's Kappa was conducted to assess the rating consistency between different physicians. Results: Compared to cTOF, sTOF showed better performance with fewer artifacts. It can effectively alleviate false positives of normal vessels being misdiagnosed as aneurysm or stenosis. Improved conspicuity was observed in cerebral distal regions with more clearly identifiable vasculature at finer scales. Quantitative comparisons in selected regions revealed significant improvement of sTOF in SNR (P<0.01 or P<0.001), CNR (P<0.001), vessel length (P<0.001), and sharpness (P<0.001) as compared to cTOF. Besides, sTOF can depict details of M1 and M2 segments of middle cerebral artery (MCA) at metallic implant region, showing its resistance to magnetic susceptibility. Conclusions: The sTOF shows higher imaging quality and lesion detectability with reduced artifacts and false positives, representing a potentially feasible surrogate in intracranial vascular imaging for future clinic routines.
Comparison of three dimensional Fast Time-of-lfight and standard time-of-lfight MR angiography in intracranial arteries

Abstract:Objective:To investigate the value of 3D Fast-TOF MRA in intracranial arteries.Materials and Methods:3D Fast-TOF and 3D standard TOF sequences were performed in 31 patients. MIP images of intracranial arteries were reconstructed. The image quality (including artifacts), carotid artery branches were compared between the two groups. Signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) of the source images of both groups were compared.Results:The acquisition time of 3D Fast-TOF group and 3D standard TOF group were 3.31 min and 5.28 min. The image score of carotid artery branches in 3D Fast-TOF group and 3D standard TOF group was 3.21±0.86 and 2.51±0.64 respectively. Signiifcant statistical difference was existed (P<0.01). Demonstration of the carotid artery branches on 3D Fast-TOF group was superior to that of 3D standard TOF group. The image quality scores of the 3D Fast-TOF group and 3D standard TOF group were 4.84±0.58, 4.87±0.56. SNR of 3D Fast-TOF MRA group and 3D Standard TOF group was 161.17, 3D Fast-TOF MRA group; CNR was 124.13±11.28,113.04±29.90. No statistical difference of image quality, SNR and CNR was existed between these tow groups (P>0.05).Conclusion:We demonstrated that 3D Fast-TOF group is a new intracranial MRA technique which could shorten acquisition time, and preserve good imaging quality and high spatial resolution.
Optimization of 4D vessel‐selective arterial spin labeling angiography using balanced steady‐state free precession and vessel‐encoding

Abstract:Vessel-selective dynamic angiograms provide a wealth of useful information about the anatomical and functional status of arteries, including information about collateral flow and blood supply to lesions. Conventional x-ray techniques are invasive and carry some risks to the patient, so non-invasive alternatives are desirable. Previously, non-contrast dynamic MRI angiograms based on arterial spin labeling (ASL) have been demonstrated using both spoiled gradient echo (SPGR) and balanced steady-state free precession (bSSFP) readout modules, but no direct comparison has been made, and bSSFP optimization over a long readout period has not been fully explored. In this study bSSFP and SPGR are theoretically and experimentally compared for dynamic ASL angiography. Unlike SPGR, bSSFP was found to have a very low ASL signal attenuation rate, even when a relatively large flip angle and short repetition time were used, leading to a threefold improvement in the measured signal-to-noise ratio (SNR) efficiency compared with SPGR. For vessel-selective applications, SNR efficiency can be further improved over single-artery labeling methods by using a vessel-encoded pseudo-continuous ASL (VEPCASL) approach. The combination of a VEPCASL preparation with a time-resolved bSSFP readout allowed the generation of four-dimensional (4D; time-resolved three-dimensional, 3D) vessel-selective cerebral angiograms in healthy volunteers with 59 ms temporal resolution. Good quality 4D angiograms were obtained in all subjects, providing comparable structural information to 3D time-of-flight images, as well as dynamic information and vessel selectivity, which was shown to be high. A rapid 1.5 min dynamic two-dimensional version of the sequence yielded similar image features and would be suitable for a busy clinical protocol. Preliminary experiments with bSSFP that included the extracranial vessels showed signal loss in regions of poor magnetic field homogeneity. However, for intracranial vessel-selective angiography, the proposed bSSFP VEPCASL sequence is highly SNR efficient and could provide useful information in a range of cerebrovascular diseases. © 2016 The Authors. Assessment of extracranial-intracranial bypass in Moyamoya disease using 3T time-of-flight MR angiography: comparison with CT angiography

Abstract:Background: To evaluate the value of time-of-flight MR angiography (TOF MRA) for the assessment of extracranial-intracranial (EC-IC) bypass in Moyamoya disease in comparison with computed tomography angiography (CTA). Patients and methods: A consecutive series of 23 patients with Moyamoya disease were analyzed retrospectively. Twenty three patients underwent 25 procedures of extracranial-intracranial bypass. Cranial CTA was performed within one week after the surgery to assess bypass patency. Then TOF MRA was scanned within 24 h after CTA on a 3T MRI system. Using 5-point scales (0 = poor to 4 = excellent), two radiologists rated the image quality and vessel integrity of bypass for three segments (extracranial, trepanation, intracranial). Results: Image quality was high in both CTA and TOF MRA (mean quality score 3.84 ± 0.37 and 3.8 ± 0.41), without statistical difference (p = 0.66). Mean scores of TOF MRA with respect to bypass visualization were higher than CTA in the intracranial segment (p = 0.026). No significant difference of bypass visualization regarding the extracranial and trepanation segments was found between TOF MRA and CTA (p = 0.66 and p = 0.34, respectively). For the trepanation segment, TOF MRA showed pseudo lesions in 2 of all 25 cases. Conclusions: 3T TOF MRA, a non-contrast technique not exposing the patients to radiation, proved to be at least equal to CTA for the assessment of EC-IC bypass, and even superior to CTA with respect to the intracranial segment. In addition, readers should be aware of a potential overestimation showing focal pseudo lesions of the bypass at the trepanation segment in TOF MRA.

Abstract:<span><b><i>Objectives:</i></b> To explore the guidance value of preoperative 3-dimensional brain volume (3D-BRAVO) and 3-dimensional time-of-flight (3D-TOF) MRA scanning for microvascular decompression. <b><i>Methods:</i></b> One hundred thirteen patients treated with microvascular decompression from February 2016 to February 2018 in the First Affiliated Hospital of Dalian Medical University were retrospectively analyzed. All patients received 3D-BRAVO combined with 3D-TOF MRA sequence reconstruction before the operation. The anatomical relationship of neurovascular tissues was analyzed and compared with the results of intraoperative exploration. <b><i>Results:</i></b> The results of MVD showed that the number of positive cases was 108 (95.6%) on the diseased side. 3D-BRAVO combined with 3D-TOF sequence reconstruction resulted in 106 positive cases (93.8%), with a 98.1% positive coincidence rate and a 13.2% false positive rate (<i>p</i> #x3c; 0.05). 3D-BRAVO-TOF sequence reconstruction of trigeminal neuralgia showed a positive coincidence in 78 cases (92.8%) and for hemifacial spasm a positive coincidence was found in 27 cases (93.1%). <b><i>Conclusion:</i></b> 3D-BRAVO combined with 3D-TOF sequence reconstruction before microvascular decompression can fully evaluate the morphology, location, and anatomical relationship of lesions, which is of guidance value for clinical diagnosis and treatment.<br/>Stereotact Funct Neurosurg</span>

Accelerated Time-of-Flight Magnetic Resonance Angiography with Sparse Undersampling and Iterative Reconstruction for the Evaluation of Intracranial Arteries

Abstract:Objective: To compare the image quality of three-dimensional time-of-flight (TOF) magnetic resonance angiography (MRA) with sparse undersampling and iterative reconstruction (sparse TOF) with that of conventional TOF MRA. Materials and Methods: This study included 56 patients who had undergone sparse TOF MRA for intracranial artery evaluation on a 3T MR scanner. Conventional TOF MRA scans were also acquired from 29 patients with matched acquisition times and another 27 patients with matched scanning parameters. The image quality was scored using a five-point scale based on the delineation of arterial vessel segments, artifacts, overall vessel visualization, and overall image quality by two radiologists independently, and the data were analyzed using the non-parametric Wilcoxon signed-rank test. Contrast ratios (CRs) of vessels were compared using the paired t test. Interobserver agreement was calculated using the kappa test. Results: Compared with conventional TOE at the same spatial resolution, sparse TOE with an acceleration factor of 3.5 could reduce acquisition time by 40 degrees Io and showed comparable image quality. In addition, when compared with conventional TOF with the same acquisition time, sparse TOF with an acceleration factor of 5 could also achieve higher spatial resolution, better delineation of vessel segments, fewer artifacts, higher image quality, and a higher CR (p < 0.05). Good-to-excellent interobserver agreement (kappa: 0.65-1.00) was obtained between the two radiologists. Conclusion: Compared with conventional TOE, sparse TOF can achieve equivalent image quality in a reduced duration. Furthermore, using the same acquisition time, sparse TOF could improve the delineation of vessels and decrease image artifacts.
Whole-Neck Non-Contrast-Enhanced MR Angiography Using Velocity Selective Magnetization Preparation

Abstract:This study aimed to optimize velocity-selective magnetic resonance angiography (VS-MRA) protocols for whole-neck angiography and demonstrate its feasibility in healthy subjects with comparisons to clinical 3D time-of-flight (TOF) angiography. To help optimize VS-MRA protocols, 2D phase-contrast (PC) flow imaging and 3D B0 and B1 field mappings were performed on five healthy volunteers. Based on these measurements, a slab-selective (SS) inversion preparation was applied prior to a VS saturation preparation to further suppress venous blood, while the VS preparation pulse was designed with compensation for field offsets. VS-MRA and 3D TOF were performed on six healthy subjects, and relative contrast ratios (CRs) between artery and muscle signals were calculated for twenty arterial regions for comparisons. The pre-compensated VS pulse improved the visualization of the subclavian arteries and suppression of background tissues, which involved large B0 and B1 field errors. The combination of SS and VS preparations effectively suppressed venous blood. While the relative CR values were 0.78 ± 0.08 and 0.72 ± 0.10 for VS-MRA and 3D TOF, respectively, over the twenty segments, VS-MRA outperformed 3D TOF in visualizing arterial segments of a small size or with a horizontal orientation, such as subclavian, facial, and occipital arteries. The proposed neck VS-MRA with the field-error-compensated VS preparation combined with the SS preparation is feasible and superior to 3D TOF in visualizing small and/or horizontally oriented arterial segments.

An mRS score <2 was defined as a good prognosis. 3D‐pcASL and tASL cerebral blood flow (CBF) maps were obtained, and the corresponding Alberta Stroke Program Early CT Score (ASPECTS)‐based scores were evaluated.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Statistical Tests</h3><p>The Kolmogorov–Smirnov test, intra‐class correlation coefficient, paired <i>t</i>‐test, receiver operating characteristic (ROC) curve, and multivariable logistic regression analysis. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>After recanalization, tASL derived absolute CBFs between the affected and contralateral sides were significantly higher than before the operation (mean: 34.3 ± 8.5 mL/100 g/min vs. 40.6 ± 9.2 mL/100 g/min, 42.6 ± 9.8 mL/100 g/min vs. 43.5 ± 9.9 mL/100 g/min, both <i>P</i> < 0.05). In ROC analysis, tASL provided good prognosis (area under ROC curve [AUC] = 0.829; 95% CI: 0.651–1.000, <i>P</i> < 0.05), while pcASL had lower prognostic value (AUC = 0.760; 95% CI: 0.574–0.946, <i>P</i> < 0.05). The NIHSS score before recanalization, pcASL, and tASL‐based ASPECTS scores were significantly associated with good clinical outcome (<i>P</i> < 0.05). Direct angiographic comparison of different velocity‐selective saturation, inversion, and DANTE labeling modules on cerebral arteries

Abstract:Purpose This study evaluated the velocity‐selective (VS) MRA with different VS labeling modules, including double refocused hyperbolic tangent, eight‐segment B1‐insensitive rotation, delay alternating with nutation for tailored excitation, Fourier transform–based VS saturation, and Fourier transform–based inversion. Methods These five VS labeling modules were evaluated first through Bloch simulations, and then using VSMRA directly on various cerebral arteries of healthy subjects. The relative signal ratios from arterial ROIs and surrounding tissues as well as relative arteria‐tissue contrast ratios of different methods were compared. Results Double refocused hyperbolic tangent and eight‐segment B1‐insensitive rotation showed very similar labeling effects. Delay alternating with nutation for tailored excitation yielded high arterial signal but with residual tissue signal due to the spatial banding effect. Fourier transform–based VS saturation with half the time of other techniques serves as an efficient nonsubtractive VSMRA method, but the remaining tissue signal still obscured some small distal arteries that were delineated by other subtraction‐based VSMRA, allowing more complete cancelation of static tissue. Fourier transform–based inversion produced the highest arterial signal in VSMRA with minimal tissue background. Conclusion This is the first study that angiographically compared five different VS labeling modules. Their labeling characteristics on arteries and tissue and implications for VSMRA and VS arterial spin labeling are discussed.

Abstract:Background: Multi-contrast vessel wall cardiovascular magnetic resonance (CMR) has demonstrated its capability for atherosclerotic plaque morphology measurement and component characterization in different vasculatures. However, limited coverage and partial volume effect with conventional two-dimensional (2D) techniques might cause lesion underestimation. The aim of this work is to evaluate the performance in a) blood suppression and b) vessel wall delineation of three-dimensional (3D) multi-contrast joint intra- and extracranial vessel wall imaging at 3T.Methods: Three multi-contrast 3D black blood (BB) sequences with T1, T2 and heavy T1 weighting and a custom designed 36-channel neurovascular coil covering the entire intra- and extracranial vasculature have been used and investigated in this study. Two healthy subjects were recruited for sequence parameter optimization and twenty-five patients were consecutively scanned for image quality and blood suppression assessment. Qualitative image scores of vessel wall delineation as well as quantitative Signal-to-Noise Ratio (SNR) and Contrast-to-Noise Ratio (CNR) were evaluated at five typical locations ranging from common carotid arteries to middle cerebral arteries.Results: The 3D multi-contrast images acquired within 15mins allowed the vessel wall visualization with 0.8 mm isotropic spatial resolution covering intra- and extracranial segments. Quantitative wall and lumen SNR measurements for each sequence showed effective blood suppression at all selected locations (P < 0.0001). Although the wall-lumen CNR varied across measured locations, each sequence provided good or adequate image quality in both intra- and extracranial segments.Conclusions: The proposed 3D multi-contrast vessel wall technique provides isotropic resolution and time efficient solution for joint intra- and extracranial vessel wall CMR.
The application value of 3D ASL in evaluating the effectiveness of stent implantation for vertebrobasilar artery stenosis

Abstract:Objective: To evaluate the effect of stent implantation for vertebrobasilar artery stenosis，by using 3D arterial spin labeling (3D ASL) technique. Methods: A retrospective analysis was conducted on the clinical and 3D ASL data of 48 patients who underwent vertebral-basilar artery stenting. Post-labeling delay times (PLD) of 1.5 s and 2.5 s were chosen, and the average regional cerebral blood flow (rCBF) values were measured in nine brain regions of the posterior circulation: bilateral thalamus, bilateral occipital lobes, bilateral cerebellar hemispheres, midbrain, pons, and medulla. The 48 patients were divided into two groups based on the presence or absence of acute ischemic stroke in the posterior cerebral circulation region detected by diffusion-weighted imaging (DWI). The preoperative and postoperative rCBF results were statistically analyzed. Results: In the infarct group, there were significant increases in rCBF values of all nine brain regions at both PLD = 1.5 s and 2.5 s postoperatively compared to preoperatively. At PLD = 1.5 s, statistically significant differences in rCBF values between the preoperative and postoperative periods were found in the right thalamus, left cerebellum, midbrain, and pons regions (P < 0.05). At PLD = 2.5 s, statistically significant differences in rCBF values between the preoperative and postoperative periods were observed in the left occipital lobe, right cerebellum, midbrain, and pons regions (P < 0.05). When analyzing the rCBF values of the brain regions with recent infarcts in the infarct group, there was a significant increase in postoperative rCBF values compared to preoperative values (P < 0.05). After excluding the data from brain regions with recent infarcts, the CBF values in the remaining brain regions were also increased postoperatively, and some brain regions showed statistically significant differences in rCBF values between the preoperative and postoperative periods (P < 0.05). In the non-infarct group, there were no statistically significant differences in the preoperative and postoperative rCBF values in all brain regions at both PLD = 1.5 s and 2.5 s (P > 0.05). Conclusion: The application of 3D ASL technology shows significant value in assessing the surgical efficacy of vertebral-basilar artery stenting, especially in patients with acute posterior circulation infarction.

Abstract:Purpose To develop a novel framework to improve the visualization of distal arteries in arterial spin labeling (ASL) dynamic MRA. Methods The attenuation of ASL blood signal due to the repetitive application of excitation RF pulses was minimized by splitting the acquisition volume into multiple thin 2D (M2D) slices, thereby reducing the exposure of the arterial blood magnetization to RF pulses while it flows within the brain. To improve the degraded vessel visualization in the slice direction due to the limited minimum achievable 2D slice thickness, a super‐resolution (SR) convolutional neural network (CNN) was trained by using 3D time‐of‐flight (TOF)‐MRA images from a large public dataset. And then, we applied domain transfer from 3D TOF‐MRA to M2D ASL‐MRA, while avoiding acquiring a large number of ASL‐MRA data required for CNN training. Results Compared to the conventional 3D ASL‐MRA, far more distal arteries were visualized with higher signal intensity by using M2D ASL‐MRA. In general, however, the vessel visualization with a conventional interpolation was prone to be blurry and unclear due to the limited spatial resolution in the slice direction, particularly in small vessels. Application of CNN‐based SR transferred from 3D TOF‐MRA to M2D ASL‐MRA successfully addressed such a limitation and achieved clearer visualization of small vessels than conventional interpolation. Conclusion This study demonstrated that the proposed framework provides improved visualization of distal arteries in later dynamic phases, which will particularly benefit the application of this approach in patients with cerebrovascular disease who have slow blood flow.

Abstract:To evaluate parallel-imaging methods in free-breathing whole-heart 3D coronary magnetic resonance angiography and assess the navigator techniques and visualization rates of the major coronary arteries. We compared key parameters of the generalized autocalibrating partially parallel acquisition and modified sensitive encoding images in vitro phantom MRI; performed the MRA with GRAPPA parallel imaging in healthy volunteers; compared 1D- and 2D-prospective acquisition correction and analyzed the differences; and evaluated the visualization of major coronary arterial branches. GRAPPA images had higher signal-to-noise ratio and contrast-to-noise ratio and fewer aliasing artifacts. The coronary arteries were adequately visualized in 38 volunteers. 2D-PACE had a higher navigator efficiency, shorter scan time, and gave clearer reconstructed images in comparison with 1D-PACE. GRAPPA images were superior to mSENSE images. Whole-heart 3D coronary MRA along with parallel-imaging technique is a potential clinical method, and 2D-PACE is a better navigation technique than 1D-PACE.

Abstract:Although there is uncertainty about whether extracranial-intracranial arterial bypass is useful for the treatment of steno-occlusive cerebrovascular disease in general, there is some argument for its continued use in particular patients. In the present study, we evaluated the efficacy of superficial temporal artery-middle cerebral artery (STA-MCA) anastomosis combined with encephalo-duro-myo-synangiosis (EDMS) in the treatment of non-moyamoya steno-occlusive cerebrovascular disease by retrospectively reviewing clinical and radiological data from 66 patients treated between January 2006 and April 2011. Forty-six double STA-MCA anastomoses and 20 single anastomoses were completed, and all remained patent in the perioperative phase, as confirmed by CT angiography. Postoperative CT perfusion imaging demonstrated immediate improvement in perfusion in the revascularized hemisphere. On discharge, 50 of the 66 patients (75.8%) had an improved National Institutes of Health Stroke Scale (NIHSS) score relative to preoperative values. After at least 6 months of follow-up, 54 (88.5%) patients had improved NIHSS scores relative to discharge values. CT perfusion imaging showed significant improvement compared to post-operative images. Follow-up digital subtraction angiography confirmed that 95 of 96 (99%) anastomoses remained patent. There was no significant difference between the single and double bypasses in terms of either clinical or hemodynamic outcomes on the revascularized side. STA-MCA combined with EDMS was effective for the treatment of non-moyamoya steno-occlusive cerebrovascular disease with hemodynamic impairment. Hemodynamic evaluation before and after surgery may help identify patients in need of a bypass and may be useful for predicting outcome.

Abstract:Angiography is critical for visualizing cerebral blood flow in intracranial steno-occlusive diseases. Current 4D magnetic resonance angiography (MRA) techniques primarily focus on macrovascular structures, yet few have quantified hemodynamic timing. This study introduces a novel model to estimate macrovascular arterial transit time (mATT) derived from arterial spin labeling (ASL)-based 4D-MRA. We provide examples of our method that visualize mATT differences in patients with intracranial steno-occlusive disease (moyamoya and atherosclerosis) and changes in mATT resulting from the cerebrovascular reactivity response to an acetazolamide (ACZ) injection. Furthermore, we present a method that projects sparse arterial signals into a 3D native brain-region atlas space and correlate regional mATT with other hemodynamic parameters of interest such as tissue transit time and cerebrovascular reactivity. This approach offers a non-invasive, quantitative assessment of macrovascular dynamics, with potential to enhance understanding of large-vessel and tissue-level hemodynamics and augment monitoring of treatment outcomes in steno-occlusive disease patients. Furthermore, it sets the stage for more in-depth investigations of the macrovascular contribution to brain hemodynamics.

Abstract:Background and purpose: 3D pseudo-continuous arterial spin labeling (3D pCASL) is commonly used to measure arterial cerebral blood flow (CBF). The aim of this study was to assess the clinical feasibility and accuracy of 3D pCASL in comparison with dynamic susceptibility contrast (DSC) perfusion imaging in moyamoya disease (MMD). Materials and methods: A total of 174 MMD patients underwent 3D pCASL and DSC-MRI for evaluating cerebral blood perfusion. 3D-pCASL with two single post-labeling delay (PLD) times (1,500 and 2,500 ms) was used to measure CBF. The values of DSC-CBF and ASL-CBF were calculated for major arterial territories including the anterior, middle, and posterior cerebral arteries as well as the areas based on the Alberta Stroke Program Early CT Score (ASPECTS) template. The correlation between DSC-CBF and ASL-CBF was analyzed. The consistency and accuracy between the two methods in assessing the cerebral ischemic state before and after surgery were analyzed. Results: The correlation between ASL (2,500 ms) and DSC-MRI was slightly better than the correlation between ASL (1,500 ms) and DSC-MRI in major vascular territories before revascularization. Significant correlations were observed between ASL (2,500 ms) and DSC-MRI and between ASL (1,500 ms) and DSC-MRI in major vascular territories after revascularization. For 44 surgically treated patients, the scores of ASPECTS for CBF on the operated side were significantly different before and after revascularization ( p < 0.05) and showed good consistency on all the examination methods. A comparison of the scores of ASPECTS of the three parameters before and after revascularization showed that there was no statistical difference between them ( p > 0.05). Conclusion: Compared to DSC-MRI, 3D pCASL can assess the cerebral blood perfusion in MMD before and after revascularization effectively. 3D pCASL showed the feasibility and clinical utility value in patients with MMD.

Abstract:Purpose: To characterize the intracranial vascular features extracted from time of flight (TOF) images and their changes from baseline to follow-up in patients undergoing carotid revascularization, using arterial spin labeling (ASL) cerebral blood flow (CBF) measurement as a reference. Methods: In this retrospective study, brain TOF and ASL images of 99 subjects, acquired before, within 48 h, and/or 6 months after, carotid revascularization surgery were analyzed. TOF images were analyzed using a custom software (iCafe) to quantify intracranial vascular features, including total vessel length, total vessel volume, and number of branches. Mean whole-brain CBF was calculated from ASL images. ASL scans showing low ASL signal in the entire flow territory of an internal carotid artery (ICA), which may be caused by labeling failure, were excluded. Changes and correlations between time points were analyzed separately for TOF intracranial vascular features and ASL CBF. Results: Similar to ASL CBF, TOF vascular features (i.e. total vessel length, total vessel volume and number of branches) increased dramatically from baseline to post-surgery, then returned to a level slightly higher than the baseline in long-term follow-up (All P < 0.05). Correlation between time points was observed for all three TOF vascular features but not for ASL CBF. Conclusion: Intracranial vascular features, including total vessel length, total vessel volume and number of branches, extracted from TOF images are useful in detecting brain blood flow changes induced by carotid revascularization surgery.
