Abstract:OBJECTIVE:To evaluate the correlation of the quality of life in patients with chronic obstructive pulmonary disease (COPD) with the lung function tests, especially the inspiratory capacity (IC).METHODS:Sixty-two stable COPD patients were enrolled between Jan. 2006 and Mar. 2007 (55 males and 7 females), with an age range of 43 - 79 (66 +/- 8) years. Lung volume, spirometry and small airway function measurements were performed in all patients. The St George respiratory questionnaire (SGRQ) score was collected. Pearson's correlation coefficients were calculated.RESULTS:The total score of the 62 COPD patients was 43 +/- 17. The 3 component scores were: symptoms (54 +/- 23), activity (54 +/- 19) and impact (33 +/- 20). The FEV(1) pred% was (43 +/- 13)%, FEV(1)/FVC was (52 +/- 11)%, residual volume (RV) pred% was (194 +/- 50)%, RV/total lung capacity (TLC) was (166 +/- 31)%, and IC pred% was (74 +/- 21)%. The SGRQ scores were significantly correlated with IC pred%, RV pred%, RV/TLC, FEV(1) pred%, FEV(1)/FVC and small airway function measurements (FEF(50%), FEF(25%), MMEF). The correlation coefficients between the parameters of spirometry and the total score of SGRQ were -0.336 to -0.479, while those between the parameter of small airway function and the total score of SGRQ were -0.368 to -0.411. Among all these lung function measurements, IC pred% was best correlated with the SGRQ score, and the correlation coefficients between the IC pred% and the total score and the other 3 component scores of SGRQ were -0.418 to -0.521, respectively, P < 0.05. The symptoms section score had the poorest correlation with the lung function test.CONCLUSIONS:The lung function test results correlate with the quality of life in patients with COPD. IC pred%, together with FEV(1) pred% should be used for clinical evaluation of patients with COPD.

Abstract:Objective:To evaluate the correlation between inspiratory capacity (IC) and health-related quality of life (HRQoL) and assess the role of IC in predicting HRQoL in patients with chronic obstructive pulmonary disease (COPD), asthma, or asthma-COPD overlap syndrome (ACO).Methods:Multilevel stratified cluster sampling method was used.We used a dataset of 5 271 adult residents in Shanghai who participated in cross-sectional stratified random sampling China Pulmonary Health study between 2021 and 2013.Participants were classified into five groups including COPD, asthma, ACO, other pulmonary disease, and healthy group.Results of spirometry tests were used, and HRQoL were evaluated by 12-Item Short Form Survey (SF-12) and COPD assessment test (CAT). We evaluated the associations between parameters of spirometry test and HRQoL scores using Spearman correlation coefficient.Results:Ventilatory dysfunction was more severe in ACO group, and relatively milder in asthma group when compared with COPD group.IC was negatively correlated with age in COPD, ACO, other pulmonary disease, and healthy group (all P<0.05). IC was positively correlated with other spirometry parameters in all groups (all P<0.05), with the correlation stronger in ACO group.IC was correlated with SF-12 score and CAT score in ACO group.For participants in ACO group, the correlation of IC with CAT score was stronger than with other spirometry parameters (ρ=-0.45, P<0.001). Conclusions:Our study suggested that IC is an important spirometry parameter that is related to quality of life in ACO patients, thus IC should be underlined in assessing disease severity of ACO.
Correlation Between Lung Function and Life Quality of the Patients with COPD in Stable Phase

Abstract:Objective To observe the correlation between quality of life and lung function in patients with chronic obstructive pulmonary disease(COPD)in stable phase.Methods The quality of life in 223 patients with COPD in stable phase were asked by concise health survey questionnaire(SF-12)developed by Boston Institute of Health.Physical component summary(PCS)scores and mental component summary(MCS)scores were counted in different classification of COPD.The 223 patients were divided into positive group and negative group according to the results of bronchial dilation test,and the relationship between lung function test and the quality of life were observed.Results PCS scores and MCS scores were negatively associated with grade of COPD disease(rPCS=-0.296,P0.05,rMCS =-0.146,P0.05).In the bronchodilation tests,77 patients were positive,146 patients were negative.PCS scores of the positive patients were higher than that of the negative ones(t=3.331,P0.01).Conclusions The lung function of COPD patients is closely related to the quality of life.Lung function test combined with quality of life in COPD patients could more roundly reflect the severity of disease.

Abstract:OBJECTIVETo evaluate the quality of life (QOL) of patients with chronic obstructive pulmonary disease (COPD) and correlated factors.METHODSData of 20 245 patients with COPD were collected from the cross-sectional survey of COPD, which was conducted between 2002 and 2004 in urban and rural areas of Beijing, Shanghai, Guangdong, Liaoning, Tianjin, Chongqing and Shanxi for residents aged over 40 years old. The recruited populations were interviewed with questionnaire and tested for spirometry. The quality of life was assessed with 12-item short-form health survey questionnaire (SF-12). Those with less than 70% of post-bronchodilator FEV(1)/FVC were identified as having COPD. The differences between groups in SF-12 scores converted by rank were compared using general linear model. Stepwise multiple linear regressions were conducted to study the main determinants of QOL.RESULTSCompared to subjects without COPD, those with COPD had impaired QOL (56 +/- 7 vs. 57 +/- 6 in mental component scores, F = 4.442, P < 0.05; 46 +/- 9 vs. 50 +/- 6 in physical component scores, F = 453.960, P < 0.05). Among COPD patients, the mental component score was associated with scores of dyspnea, BMI, comorbidities, sex and living areas, while the physical component score was associated with scores of dyspnea, severity of COPD, comorbidities, exposure to dusts/gases/fumes, sex, age, educational level and previous diagnosis of respiratory diseases (all P < 0.05).CONCLUSIONSThe QOL in patients with COPD was impaired and associated with scores of dyspnea, severity of COPD, comorbidities and BMI. Improvement of dyspnea, nutritional support, prevention of comorbidities and keeping away from risk factors may improve the QOL in COPD patients.

Abstract:Objective To provide direct interventions for patients with chronic obstructive pulmonary disease(COPD)and relevant bases of medical decision-making and resource allocation for the government by investigation of those biology-psychology-social factors which influence COPD patients medical behaviors. Methods COPD patients were followed in July in 2014 with general conditions, severity grading and questionnaire of quality of life(QOL). Telephoned patients every month, processed the health education every six months and recorded their attendance times and their compliance status. The study ended in December 2015.The final QOL questionnaire investigation was proceeded and those objects were divided into different groups according their outcomes: aggravating group, illness group without change, improved group. Multinomial Logistic regression were used to find those biologicalpsychological-social factors that influenced these COPD patients' diagnosis and outcome. Variance analysis were used in groups or between-group to find out the relationship between prognosis and QOL. Results Patients with reimbursement proportion 70％ higher, better compliance with health education, no complications, illness lighter(A/B)in the beginning would get better than the others(OR: 10.35 vs2.147 vs 5.791 vs 1.51, P＜0.05). Scores of aggravating group patients in the physiological, psychological and environmental area of QOL were significantly lower at the end of the study than that in the beginning of study(P＜0.05). Scores in psychological area of illness group without change were obviously higher at the end of the study than that in the beginning of study(P＜0.05). Scores in physiological area of improved group were obviously higher at the end of the study than that in the beginning of study(P＜0.05). In the beginning of the study the QOL scores in all area of improved group patients were obviously higher than that of aggravating group patients(P＜0.05).Scores in social relationship and environmental area of illness group without change patients were obviously higher than that of aggravating group patients(P＜0.05).At the end of the study the QOL scores in all area of both improved or illness group without change patients were obviously higher than that of aggravating group patients(P＜0.05). Conclusions Reimbursement proportion 70％ or higher, better compliance, no complications, illness lighter(A/B)are protective factors. Underweight or normal, frequent acute attack and lower compliance are risk factors of COPD. Patients with higher QOL scores got improved or stable conditions. The QOL scores in both illness group without change or improved group patients are higher than that in aggravating group patients.

Abstract:Objective:To investigate the relationship between quality of life,lower airway bacterial colonization(LABC)and airway inflammation patients with COPD.Methods:A total of 30 patients with moderate or severe COPD but stable at baseline were included in this study.As contradistinction,30 healthy controls were included in this study.The induced sputum were collected at the visit.The samples were analyzed for their bacterial loads based on the semi-quantitate bacterial culture,meanwhile the blood routine tests,the lung functions,the blood gas analysis,the levels of interleukin(IL)-8 and the St.George's Respiratory Questionnaire(SGRQ)were measured.Results:Out of 30 patients,8 patients(26.7%)showed LABC or LRI(lower respiratory infection),the bacterial loads≥+++ in sputum at the visit.Pathogenic bacterias including Streptococcus pneumoniae,Hemophilus influenza,Hemophilus parainfluenzae,Pseudomonas aeruginosa were found;IL-8 in patients with LABC were significantly higher than those without LABC and healthy controls(P0.01),but FEV1(% predicted),FEV1/FVC were significantly lower than those without LABC(P0.05).The presence of SGRQ total and symptom score had a positive correlation with the index number of smoking.The presence of SGRQ total and symptom score had a positive correlation with the results of the semi-quantitate bacterial culture(P0.05).The presence of SGRQ total,symptom,mobility and impacts score had a positive correlation with the levels of IL-8(P0.01).Conclusion:LABC were present in a portion of stable COPD patients.LABC may be stimulators which can induce airway inflammatory and lead to degrading of lung function.The patients with LABC suffer from poor quality of life than those patients without LABC.
Investigation of Health-Related Quality of Life and Impact Factors in Elderly Patients with COPD in Beijing

Abstract:Objective To assess the health-related quality of life ( HRQoL) of elderly patients with COPD in Beijing and to explore the impact factors. Methods 100 outpatients diagnosed with irreversible COPD in Beijing completed the SF-36 questionnaire. The scores of 8 dimensions of questionnaire in elderly patients with COPD were calculated and compared with those of the rural general elderly people in Tianjin. Multiple regression analysis was applied to determine the impact factors of different dimensions of SF-36. Results The scores of all the dimensions in the patients were significantly lower than those in general old rural people of Tianjin. The correlations between the different dimensions of the SF-36 and variables were: Physical functioning was well correlated with ADL, ability to carrying activities (6MWD) and anxiety. Role-physical was well correlated with anxiety, admission times in a year and nutrition ( BMI). Body pain was well correlated with anxiety. General health was well correlated with depression, social support, the number of combination chronic disease, ability to carrying activities (6MWD) and the severity of disease (FEV1. 0%). Vitality was well correlated with depression. Social functioning was well correlated with ADL, carrying activities (6MWD), social support and depression. Mental heath was well correlated with depression ,anxiety and nutrition (BMI).No variable showed significant correlation with the Role-emotional dimension. Conclusions The HRQoL of elderly patients with COPD is declined compared with general elderly people. There are a range of factors that impact different dimensions of HRQoL, chiefly emotional status and carrying activities. Targeted measures of health interventions could be carried out to improve the HRQoL of elderly patients with COPD.

Abstract:Objective:To probe into the influence of sleep quality and fatigue symptoms of patients with chronic obstructive pulmonary disease (COPD) on their living quality.Methods:A total of 91 COPD patients in stable period one month later discharged from hospital were investigated by measuring Pittsburgh sleep quality index(PSQI),fatigue scale(FS-14) and St.George’s respiration questionnaire (SGRQ) for living quality assessment.Results:In terms of sleep quality,there was statistical significant difference between COPD patients in stable period and the norm in China (P<0.01).Polynary factors regression analysis showed that both sleep quality and physical fatigue symptoms of COPD patients had influences on their living quality.Conclusion:The sleep quality of COPD patients was poor.And physical fatigue symptoms of COPD patients were severe. Either poor sleep quality or severe physical fatigue symptoms of COPD patients may lower their quality of life.

Abstract:Objective To study the relationship between quality of life and coping style of patients with COPD.Methods A descriptive and correlative study was performed.A total of 102 patients were investigated by SF-36,AQ20-R,and MCMQ,the result of which was analyzed by SPSS 13.0.Results Of the dimensions of SF-36,the lowest score was role-physical(RP),and general health(GH) in turn.The total score of AQ20-R was(8.73±3.74).The score of RP was positively correlated with confrontation coping mode,the scores of vitality(VT) and role-emotional(RE) were negatively correlated with avoidance coping mode.The total score of AQ20-R was positively correlated with avoidance and resignation.The scores of SF-36 were negatively correlated with resignation coping mode,except bodily pain(BP).Conclusion Coping style of COPD patients exerts certain influence on their quality of life.Nursing staff should make an overall evaluation of health status of patients in physiological,psychological and social aspects and help patients to adopt active coping styles.
Analysis on the Correlation Between the Self-Efficacy Level and the Quality of Life of Patients with Chronic Obstructive Pulmonary Disease

Abstract:Objective To analyze the correlation between the quality of life and the self-efficacy of patients with chronic obstructive pulmonary disease(COPD),then to provide the theory frame for the intervention based on the Self-efficacy Theory.Methods A total of 68 cases with COPD were investigated with the St George's Respiratory Questionnaire(SGRQ) and the COPD Self-efficacy Scale(CSES).Results The total score of self-efficacy was(80.50±11.15) and the self-efficacy of COPD patients was at a moderate level.The overall SGRQ scores was(51.26±14.45).Pearson correlation analysis showed that the overall score of self-efficacy and it's dimensions score were negatively related with the overall SGRQ score and its three dimensions score(P0.01 or P0.05).Conclusion The level of self-efficacy of COPD patients was not at a high level.To improve the patient's physical functional status and quality of life,the clinical and community health workers should improve the self-efficacy level of patients on disease management by various means,and enhance the establishment of self-management behaviors,and ultimately improve the patient health status.

Abstract:BACKGROUND:Chronic obstructive pulmonary disease (COPD) is a serious disease with impaired quality of life (QoL). Few studies have focused on the QoL for mild COPD patients in Chinese urban communities. This study aimed to evaluate the QoL and explore its associated factors among patients with mild COPD in Shanghai. METHODS:A cross-sectional survey of 275 mild COPD patients was conducted in 6 communities of Pudong New Area in Shanghai, China, in 2016. Data on socioeconomic and health conditions were acquired through a face-to-face interview and a physical examination. EuroQoL five-dimension questionnaire (EQ-5D)- Chinese version was applied to evaluate their QoL. Logistic regression was used to estimate odds ratios (ORs) and their 95% CI for risk factors associated with QoL. RESULTS:Among 275 subjects, the overall visual analog scale score and utility of QoL were 70.6 and 0.889, respectively. Logistic regression analysis indicated that exacerbation in recent year were negatively associated with the top 25% utility of QoL (OR =2.02, 95% CI: 1.03-3.97), and the worse exercise capacity (distance of 6-minute walking test <525.3 m) was negatively correlated the outcome (OR =2.92, 95% CI: 1.10-7.75) too. In multi-factor logistic regression, the associations were slightly weakened. CONCLUSIONS:The QoL was impaired in mild COPD patients living in urban communities. Exacerbation in the past year and exercise capacity were significantly correlated to QoL. More interventions should be developed to improve the QoL at the early stage of COPD.

Abstract:Objective To explore the correlation between health knowledge level and self-efficacy of patients with chronic obstructive pulmonary disease(COPD) so as to provide the basis for future implementation of self-management project.Methods Sixty-eight COPD patients were investigated with the self-designed COPD knowledge questionnaire and COPD self-efficacy scale.The correlation between the health knowledge level and self-efficacy were analyzed.Results The scores of self-efficacy(2.38±0.50) and health knowledge of COPD(18.03±5.12) were at a moderate level.By Spearson correlation analysis,the overall of self-efficacy(r=0.339,P0.01) and scores score of diffiallt breathing mcenagement(r=0.551,P0.01) and intense emotional arousal(r=0.374,P0.01) were positively related with the level of the health knowledge of COPD(P0.01),but no significant correlation between the other three dimensions of self-efficacy with the level of the health knowledge(P0.05).Conclusion Since the self-efficacy and the health knowledge of COPD were not at a high level,we must attach attention to the self-management intervention of COPD,so as to improve the level of the health knowledge and self-efficacy,ultimately to promote the establishment of self-management behavior.
A Canonical Correlation Analysis of Factors that Influence Quality of Life Among Patients with Chronic Obstructive Pulmonary Disease Based on QLICD-COPD (V2.0)

Abstract:Abstract Background Chronic obstructive pulmonary disease (COPD) is one of the diseases with the highest morbidity and mortality globally. The Quality of Life Instrument for Chronic Diseases(QLICD)-COPD(V2.0) was designed to assess the health condition of patients with COPD. The objective of this study was to evaluate the quality of life (QOL) of patients, the influential clinical factors, and the relationships between QOL and clinical objective indicators. Methods Two hundred and sixty-one in-patients with COPD in the acute exacerbation stage were evaluated using the QLICD-COPD(V2.0) and data on clinical objective indicators were collected. The relationships between QOL and the clinical objective indicators were determined using canonical correlation analysis. Results The standardized scores for the patients in four domains, namely physical function, psychological function, social function, and a disease-specific module, were 49.00 ± 12.91, 59.89 ± 13.51, 68.59 ± 11.94, and 51.84 ± 13.58, respectively. The total score for the QOL of patients was 57.17 ± 10.26. Two pairs of canonical correlation variables were statistically significant (P < 0.05), with coefficients of 0.35 and 0.26. These variables respectively accounted for 45.8% and 33.8% of the variance. The levels of total protein, albumin, serum sodium, and alkaline phosphatase and the percentages of neutrophils and lymphocytes were correlated with the QOL. Conclusion During COPD treatment, clinicians should pay close attention to the levels of total protein, albumin, serum sodium, and alkaline phosphatase and the percentages of neutrophils and lymphocytes to improve the QOL of patients.

Abstract:Objective To assess the severity distribution proportion and quality of life (QOL) of chronic obstructive pulmonary disease(COPD)among population in Southern China.Methods Data were from the cross-sectional survey of COPD which was conducted in an urban and a rural of Guangdong for residents aged over 40 years old.The recruited populations were interviewed with questionnaire and were tested with spirometry.Those with less than 70％ of post-bronchodialator FEV1/FVC were identified as the COPD.The quality of life was assessed with 12-item Short-Form health survey questionnaire (SF-12).Groups (A,B,C and D) were classified according to GOLD using combination of mMRC,FEV1 ％pred and acute exacerbation of COPD in last year.Results Three hundred and three patients with COPD indentified by spirometry testing among population,there were 109 (36.0 ％),77 (25.4 ％),38 (12.5 ％)and 79 (26.1 ％) patients in Group A,B,C and D,respectively.There was statistically difference between each two groups in physical component scores of SF-12 (F =39.593,P ＜0.001) except for difference between Group B and C,with 49.28±4.25,43.21±7.94,44.67±7.90 and 37.45±9.68 for Group A,B,C and D,respectively.A statistically difference was found between four groups in mental component scores of SF-12 (F =2.978,P =0.032),with 57.62±3.31,57.89±4.25,57.22±4.83,55.63±7.95 for Group A,B,C and D,respectively.Group D had a lower mental component scores of SF-12 compare to Group A and C (both P ＜0.05).Conclusions Group A is the highest proportion in population with a minimum of impaired quality of life,and Group D has the most serious damage of quality of life.

Abstract:Objective The purpose of the current study was to deternmine the correlation between the parameters quantified by chest low-dose CT scan (LDCT) and airflow limitation examined by spirometry.Methods This study included 48 patients above 40 years old and with smoking history who underwent LDCT and spirometry on the same day.The regression model was generated on the base of the LDCT value which correlated best to airflow limitation using ROC curve.Results With age,gender and body mass index adjusted,EV and El significantly correlated negatively with FEV1,FEV1％predieted,FEV1/FVC and TLC％ predicted (P＜0.05),but had no correlation with RV/TLC％ predicted ( P＞0.05).The regression model were FEV1/FVC％=94.17+25.31 X＜gender-0.58×age-10.84×In (EI (％)),FEV1％predicted=141.76-0.78×age-14.07×In (EI(％)).The best LDCT value for airflow limitation estimation was EI.Conclusions El may be used to estimate whether airflow limitation exited and to calculate its severity by regression model.LDCT might be a new way for early detection of COPD.
Relationship Between Disease Severity and Life Quality in Patients with Chronic Obstructive Pulmonary Disease

Abstract:Objective To compare BODE index with GOLD staging for the severity evaluation in patients with chronic obstructive pulmonary disease(COPD).To investigate the relationship between BODE index and quality of life.Methods A total of 100 patients with stable COPD were enrolled.All participants were examined with lung function test and St George's Respiratory Questionnaire (SGRQ) for evaluation of quality of life.BODE index and it's four components including body mass index(BMI),airflow obstruction(FEV1%pred),dyspnea(MMRC),and exercise capacity(6MWD) were calculated.The participants were divided into four groups of severity using GOLD guidelines and BODE index quartiles.Results The associations between SGRQ total score and SGRQ subscales,and BODE index were significant(P<0.01).BODE index was a significant predictor of the quality of life,explaining 41.3% of the total score of the SGRQ(P<0.01).However,GOLD classification showed a correlation only with SGRQ activity score(P<0.05) but not with other SGRQ scores.There were significant differences in SGRQ total score and SGRQ subscales between different severity groups of BODE index(P<0.01).But there was no difference in SGRQ score between different severity groups of GOLD classification,except SGRQ activity score.Conclusion BODE scoring system is better correlated with the quality of life as assessed by the disease severity in patients with COPD than the GOLD staging criteria.
Profile and Correlates of Health-related Quality of Life in Chinese Patients with Coronary Heart Disease.

Abstract:BACKGROUND:Although coronary heart disease (CHD) is one of the major cardiovascular diseases, risk factors associated with the health-related quality of life (HRQoL) of CHD patients remain unclear. The present study was designed to determine the profile and significant factors of the HRQoL in CHD patients.METHODS:A cross-sectional study was conducted in rural communities of Fangshan District, Beijing, China. Socio-demographic, lifestyle, and comorbidity information of CHD patients were collected by a structured questionnaire and medical records. HRQoL was measured using European Quality of Life 5-dimensions (EQ-5D) scale and EQ Visual Analog Scale (EQ-VAS). Multiple linear and logistic regressions were performed to explore the association of potential risk factors with HRQoL scores and each EQ-5D, respectively.RESULTS:Totally, 1928 CHD patients (mean age 61.64 ± 9.24 years; female:male = 2.4:1) were enrolled in the study. The mean score of EQ-5D index and EQ-VAS were 0.889 ± 0.172 and 71.56 ± 17.65, respectively. Multiple linear regression revealed that marital status, physical activity, moderate alcohol drinking, and family's population were positive independent correlates of EQ-VAS, whereas diabetes mellitus and stroke were negative independent correlates (all P < 0.05). Age and stroke were negatively while physical activity, moderate alcohol drinking, family's population and household income were positively correlated with EQ-5D index (all P < 0.05) independently. In addition, each of the five HRQoL dimensions had various specific determinants, including obesity, underweight, smoking or education.CONCLUSIONS:Findings of the study highlight certain socio-demographic, lifestyle factors, and comorbid stroke or diabetes mellitus as correlates of HRQoL in Chinese CHD patients. Large-scale cohort studies should be carried out to confirm our results in the future.

Abstract:Objective To investigate the relationship between nutritional risk and exercise tolerance in patients with severe chronic obstructive pulmonary disease (COPD).Methods The nutritional risk of 58 severe COPD patients was assessmented according to the nutritional risk screening 2002 (NRS-2002).There were 33 patients without nutritional risk (NRS<3,group A) and 25 patients with nutritional risk (NRS≥3,group B).Arterial blood gas (ABG) analysis,pulmonary function test (PFT) and cardiopulmonary exercise test (CPET) were performed in all patients.Results There were no significant differences in age and height between two groups (P>0.05).The weight [(51.24±4.68)kg],BMI (18.34±1.33) in group B were lower than those in group A (P<0.01).There no significant differences in blood gas parameters PaO2 and PaCO2 between two groups (P>0.05).Pulmonary function tests showed that the DLCO of group B was significantly lower than that of group A (P<0.01);while there were no significant differences in other indicators (FEV1,FEV1%pred,FVC,FEV1/FVC,RV,RV/TLC) between two groups (P>0.05).In cardiopulmonary exercise test,Peak V· O2,Peak V· O2%pred and Peak O2 pulse in group B were singificantly lower than those in group A (P<0.05 or P<0.01).There were no significant differences in Peak V· O2/kg and Peak Load between the two groups (P>0.05).Conclusion The nutritional risk of patients with severe COPD can worsen pulmonary ventilation function and decrease exercise tolerance significantly.Nutritional risk assessment may help to understand the severity of patients with severe COPD and to predict clinical risk.
A Preliminary Study on the Relationship Between High-Resolution Computed Tomography and Pulmonary Function in People at Risk of Developing Chronic Obstructive Pulmonary Disease

Abstract:Objectives Patients with chronic obstructive pulmonary disease (COPD) have high morbidity and mortality, the opportunity to carry out a thoracic high-resolution CT (HRCT) scan may increase the possibility to identify the group at risk of disease. The aim of our study was to explore the differences in HRCT emphysema parameters, air trapping parameters, and lung density parameters between high and low-risk patients of COPD and evaluate their correlation with pulmonary function parameters. Methods In this retrospective, single-center cohort study, we enrolled outpatients from the Physical Examination Center and Respiratory Medicine of The First Affiliated Hospital of Wenzhou Medical University. The patients who were ≥ 40 years-old, had chronic cough or sputum production, and/or had exposure to risk factors for the disease and had not reached the diagnostic criteria is considered people at risk of COPD. They were divided into low-risk group and high-risk group according to FEV 1 /FVC ≥ 80% and 80%>FEV 1 /FVC ≥ 70%. Data on clinical characteristics, clinical symptom score, pulmonary function, and HRCT were recorded. Results 72 COPD high-risk patients and 86 COPD low-risk patients were enrolled in the study, and the air trapping index of left, right, and bilateral lungs of the high-risk group were higher than those of the low-risk group. However, the result of mean expiratory lung density was opposite. The emphysema index of left, right, and bilateral lungs were negatively correlated with FEV 1 /FVC (correlation coefficients were -0.33, -0.22, -0.26). Consistently, the air trapping index of left and right lungs and bilateral lungs were negatively correlated with FEV 1 /FVC (correlation coefficients were -0.33, -0.23, -0.28). Additionally, the mean expiratory lung density of left and right lungs and bilateral lungs were positively correlated with FEV 1 /FVC (correlation coefficients were 0.31, 0.25, 0.29). Conclusion The emphysema index, air trapping index and the mean expiratory lung density shows significantly positive correlation with FEV 1 /FVC which can be used to assess the pulmonary function status of people at risk of COPD and provide a useful supplement for the early and comprehensive assessment of the disease.

Abstract:Objective To evaluate effect of chronic obstructive pulmonary disease (COPD) clinical questionnaire (CCQ) in evaluating the quality of life of patients with COPD in stable period.Methods A total of 105 patients COPD in stable period admitted during December 2014 and December 2015 received survey of pulmonary function,St.George Respiratory Questionnaire (SGRQ),COPD Assessment Test (CAT),test to assess the British Medical Research Council (MMRC) questionnaire and COPD CCQ.The relationship between different evaluation indexes was analyzed.Results Values of completion time and assistance ratio by SGRQ were significantly higher than those by CAT,MMRC and CCQ.There were significant differences in forced vital capacity (FVC),forced expiratory volume in one second (FEV1) and FEV1/FVC among SGRQ,CAT,MMRC and CCQ in different GOLD grades (P ＜ 0.05).SGRQ,CAT,MMRC and CCQ scores were positively correlated (P ＜ 0.05),and every questionnaire score was negatively correlated with its lung function indexes (P ＜ 0.05).CAT and MMRC were consistent in evaluation of high-risk patients,while there was significant difference in evaluation of low-risk patients (P ＜0.001,P ＜0.05).CCQ and CAT were consistent in evaluation of low-risk and high-risk patients (P ＜ 0.001).Conclusion SGRQ,CAT,MMRC and CCQ are correlated with pulmonary function in patients with COPD,and therefore it can be used as important methods to evaluate quality of life of the patients.SGRQ is more complex,and it needs assist to complete the evaluation with time-consuming.CCQ or CAT score is better in evaluation of the symptoms of patients with COPD.
