Influence of Low Calcium Dialysate on Serum Parathyroid Hormone, Calcium and Phosphate in Patients under Hemodialysis

Abstract:Objective:To observe the change of intact parathyroid hormone, serum calcium and serum phosphate in hemodialysis patients treated with the dialysate of low calcium (1 25mmol/L) for 3 months. Methodology:Six uremic patients with routine hemodialysis 3 times per week were enrolled in this study. All of them received hemodialysis with the dialysate of low calcium (1 25mmol/L) for 3 months. During this period, the patients maintained stable for diet and medication. Before and after using the low calcium dialysate therapy, the serum calcium, phosphate and intact parathyroid hormone were measured in pre and post dialysis. For a single hemodialysis, the serum calcium, phosphate and intact parathyroid hormone were measured in pre , post and the next pre dialysis. Results:For a single dialysis, the serum calcium decreased in post dialysis [(2 50±0 23) mmol/L] compared with in pre dialysis (2 63±0 16 mmol/L), and the iPTH increased significantly in post dialysis compared with pre diaysis [(219±143) ng/L vs (158±107) ng/L, P 0 05]. However, the serum calcium [(2 64±0 15) mmol/L] and iPTH [(158±125)ng/L] came back in the next pre dialysis. When patients being dialyzed with the low calcium dialysate (1 25 mmol/L) for 3 months, the serum iPTH increased markedly [(158±107)ng/L vs (82 5±43 7) ng/L, P 0 05)] Conclusion:When patients were dialyzed with the dialysate of low calcium for 3 months, the serum iPTH might increase. The iPTH should be detected regularly if the low calcium dialysate (1.25 mmol/L) was used for a long time.
