EVALUATION OF INTERVENTIONS BY PHARMACISTS IN VANCOMYCIN TREATMENT IN CHINESE ADULT PNEUMONIA PATIENTS: A RANDOMIZED CONTROLLED TRIAL

Abstract:OBJECTIVE To build a new work pattern to reduce VI-AKI which pharmacists used PPK models and vancomycin TDM guidelines to intervene the treatment of vancomycin in adult pneumonia patients. The new work pattern was compared with the traditional work pattern to verify the effectiveness. METHODS The research is a single center prospective cohort study from January1 th, 2017 to August 31 th, 2018 which was approved by Peking University First Hospital Clinical Research Ethics Committee. Patients was divided in the control group and intervention group by a random-cross reference table base on some pre-survey. In the intervention include the following project:(1) Pharmacists predicted the steady trough concentrations of vancomycin in different dosing regimens using adult(18-64 years) and elderly(≥65 years) PPK models, and(2) recommended the initial treatment dosage to doctors;(3)Pharmacists reminded doctors of TDM at the point-time. Pharmacists do nothing with the control group. RESULTS 55 patients were included in the control group, and 51 patients were included in the intervention group. The average prediction error of adult formula and elderly formula is 30.06% and 29.00%,respectively. Compared with the control group, the incidence of VI-AKI decreased(11.76% vs 21.62%, P=0.349). The adoption rate of clinical recommendations was 91.18%. CONCLUSION A new pharmacist work pattern was built to reduce VI-AKI which contains PPK and TDM. Intervention of pharmacists in pneumonia treatment with vancomycin can improve the rationality of vancomycin usage and reduce the incidence of VI-AKI.
